SummaryPrevious surveys in Myanmar indicate that soil-transmitted helminths (STH) are a significant public health problem. However the majority of these surveys are small scale or hospital based. To date there has been no countrywide assessment of the prevalence and intensity of STH infection. We conducted a survey (June 2002-June 2003 to assess the magnitude of STH infection in school age children in each of the different ecological areas of the country. In each area, five schools were selected and from each one, 50 children were randomly chosen. Parasitological and nutritional data were collected from each child. We found an overall prevalence of infection of 69%: 18% had a moderate-heavy intensity of infection, 22% were anaemic. A pilot exercise to treat 25 000 school age children in the 200 schools of Nyangdone Township tested the strategy before scaling up and to generate some rough costings. A crude calculation of the expenditure to buy and deliver the drugs, including the costs incurred in the teacher training, concluded that the cost per child was approximately 0.05 USD.
SummaryLymphatic filariasis (LF) and the major soil-transmitted helminth (STH) infections are co-endemic in many countries, particularly in Asia. Control strategies for both groups of infections have increasingly focused on the use of mass chemotherapy. With the use of albendazole, there is now a tool that is common to both. However, there are also important differences in their modes of transmission and epidemiology, and as a result, in the overall control strategies. The Global Programme for the Elimination of Lymphatic Filariasis aims to eliminate LF through time-limited, Mass Drug Administration programmes. STH control activities are more diffuse, aiming to piggyback de-worming onto existing services such as school health activities; controlling morbidity, rather than eliminating infection, is the stated goal. In order to maximize health benefits to communities that are endemic for one or both of these infections, it is vitally important that policy makers and programme managers have a clear understanding of both commonalities and differences, and implement control programmes that allocate available resources in an optimal manner.

vector species. LF prevalence declined very appreciably as a result of chemotherapeutic intervention measures implemented in 1977, but low levels of infection persisted. Along with other Pacific Island countries and in partnership with the Pacific Programme to Eliminate LF (PacELF), Tonga implemented a programme to eliminate LF as a public health problem.
Methods
On the basis of historical data and baseline survey, all the divisions of the country were declared as endemic. Five to six consecutive rounds of effective MDA were implemented in all the divisions during 2001–2006. The impact of MDA was assessed through interim and post-MDA antigen (Ag) detection surveys among adults and transmission assessment surveys among children. The chronic disease burden was assessed by health workers through observation.
Results
The base-line Ag prevalence was 2.70%. The treatment coverage was > 80% in all MDA rounds. The mid-term surveys showed an Ag prevalence of 2.46%. The pre-stop MDA Ag survey revealed an Ag prevalence of 0.34%. The stop MDA survey and transmission assessment surveys among children showed Ag prevalence at < 0.05%, indicating transmission is negligible. Health workers concluded that filarial lymphedema or hydrocele condition in the communities is absent or very rare.
Conclusion
Tonga had successfully met the criteria for elimination of LF as a public health problem. The accomplishment was acknowledged by the WHO in 2017. Tonga looks forward to work with stakeholders to eliminate transmission of LF and achieve zero incidence of infection.
We discuss the experience of some Pacific island countries in introducing the new WHO-recommended treatment protocol for lymphatic filariasis—a triple-drug therapy composed of ivermectin, diethylcarbamazine, and albendazole. The successful rollout of the new treatment protocol was dependent on strong partnerships among these countries’ ministries of health, WHO, and other stakeholders. Effective communication among these partners allowed for lessons learned to cross borders and have a positive impact on the experiences of other countries. We also describe various challenges confronted during this process and the ways these countries overcame them.
