Lower extremity deformities of patients with arthrogryposis multiplex congenita present a wide spectrum of severity and deformity combinations. Treatment goals range from merely ensuring comfortable seating and shoe wear, to fully independent and active ambulation, but the overarching intention is to help realize the patient's greatest potential for independence and function. Treatment of hip and knee contractures and dislocations has become more interventional, whereas treatment of foot deformities has paradoxically become much less surgical. This article synopsizes the treatment strategies presented in September 2014 in Saint Petersburg, Russia at the second international symposium on arthrogryposis.

Буллезный эпидермолиз (БЭ)-это редкое наследственное заболевание, его главный признак -образование пузырей и мокнущих ран (эрозий) на коже и слизистых оболочках, возникающих при незначительном травмировании. Клинические проявления заболевания могут варьировать от локализованных пузырей на руках и стопах до генерализованных высыпаний по всему кожному покрову, а также с поражением слизистой оболочки внутренних органов. В настоящее время выделено четыре основные группы БЭ: простой, промежуточный, дистрофический и синдром Киндлера. Мутации вызывают изменения в структуре белков, ответственных за адгезию между слоями дермы, что и приводит к образованию везикул. Лечение БЭ представляет собой сложную задачу вследствие отсутствия возможности прямого воздействия на патогенез заболевания, и его основной целью является купирование существующих кожных проявлений и предотвращение появления новых элементов. В статье приводится описание основных типов БЭ, видов современной диагностики и лечения заболевания, а также представлен клинический случай редкого сочетания двух тяжелых патологий -дистрофического буллезного эпидермолиза и артрогрипоза с поражением верхних и нижних конечностей.Ключевые слова: буллезный эпидермолиз, артрогрипоз, сгибательные контрактуры конечностей.
Background. Severe hypoplasia (or aplasia) of the biceps brachii is a primary cause of restriction in activities of daily living in patients with arthrogryposis. Aim. To estimate the possibility of restoring elbow active flexion via a latissimus dorsii transfer in patients with arthrogryposis. Materials and methods. From 2011 to 2018, we restored active flexion of the elbow via a latissimus dorsi transfer to the biceps brachii in 30 patients with arthrogryposis (44 upper limbs). We used different regimes including clinical examinations, EMG donor and recipient sites, and CT of the chest wall and shoulder. Results. The mean age of the patients was 4.0 ± 2.4 years, and the follow-up period was 3.2 ± 1.9 months. Follow-up results were available for 26 patients (30 upper limbs). The active postoperative elbow motion was 90.5 ± 14.9°. Elbow extension limitation occurred in 51% of cases (12.8 ± 4.3°) without any problems in activities of daily living. In total, 55.6% of patients had good results, 33.3% had satisfactory results, and 11.1% had poor results. Discussion. Our latissimus dorsi transfer results were comparable with those of other authors. Transposition of the latissimus dorsi to the biceps brachii restores sufficient flexion of the elbow without severe elbow flexion contractures. Conclusions. We suggest pedicle monopolar latissimus dorsi transfer as a reliable therapeutic option to restore active elbow flexion in patients with arthrogryposis having passive elbow flexion of 90° or higher before operation and donor muscle strain grade 4 or higher.

Сгибательные контрактуры коленных суставов у детей с артрогрипозом встречаются часто и значительно изменяют кинематику ходьбы, снижают эффективность передвижения или делают его невозможным . Из многообразия методов хирургического лечения -мягкотканный релиз с использованием аппарата Илизарова или без него, разгибательная надмыщелковая остеотомия бедренной кости -сложно выбрать наиболее эффективный, так как каждый метод имеет свои недостатки . Целью исследования было оценить результаты коррекции сгибательных контрактур коленных суставов с помощью метода управляемого роста у пациентов с артрогрипозом . Материалы и методы. В исследование было включено 12 пациентов с артрогрипозом со сгибательными контрактурами коленных суставов (20 коленных суставов), которым выполнялся временный гемиэпифизеодез передней части дистальной зоны роста бедренной кости с использованием 8-образных пластин . Средний возраст на момент операции составлял 6,5 ± 0,5 года (4,3-9,6) . Применялся клинический и рентгенологический методы исследования со статистической обработкой полученных данных . Результаты. Средняя величина дефицита разгибания коленного сустава до операции составляла 48,5 ± 4,04° (20-80°) . За период наблюдения от 18 до 36 месяцев после гемиэпифизеодеза дистальной зоны роста бедренной кости было отмечено уменьшение сгибательной контрактуры коленного сустава в 17 случаях (85 %) в среднем на 20 ± 2,67° (0-40°), p < 0,05 . Величина резидуальной деформации составила 28,5 ± 6,03° (0-60°) . Наиболее значительно (на 90 % по сравнению с исходной величиной) происходила коррекция у пациентов с контрактурами до 50° (p < 0,05) . В этой группе были пациенты с тяжелыми сгибательными контрактурами, которым до операции производилась попытка их коррекции гипсовыми повязками с дистракционным устройством, в результате чего величина контрактуры была значительно уменьшена . Выводы. Метод временного гемиэпифизеодеза является эффективным, безопасным и менее инвазивным по сравнению с другими методиками и может применяться для лечения детей с артрогрипозом . Сочетание гемиэпифизеодеза с дополнительными методами коррекции сгибательной контрактуры помогает значительно уменьшить ее величину, перевести ее из тяжелой в умеренную, делая тем самым лечение более эффективным и менее продолжительным, что позволяет в кратчайшие сроки достичь вертикализации пациента . Background. Patients with amyoplasia and hip dislocation have different variants of hip contractures and deformities, but there is no difference in the selection of the method. Aim. To justify the algorithm of rational selection of surgery methods in children under 3 years of age with hip dislocation in amyoplasia Materials and methods. Level of Evidence II. 70 patients were examined, including 21 children under the age of 12 months (main group) after 25 hip open reduction, 19 children under the age of 3 years (main group) after hip open reduction, Salter innominate osteotomy, femoral osteotomy and 30 patients aged 3 to 7 years (control group) who had not previously received conservative and surgical treatment. All patients were divided into two subgroups depending on the variant of hip contracture: flexion-extension-abduction-external rotation (frog-like) (subgroup 1) and flexion-extension-adduction-external rotation (subgroup 2). We used clinical, radiological and statistical methods. Results. In children of subgroup 1, after hip open reduction, good results were noted in 17% of cases, satisfactory in 50%, unsatisfactory in 33%, severe complications class III, IV according to the modified Clavien-Dindo-Sink classification were obtained in 83%. After hip open reduction, Salter innominate osteotomy, and femoral osteotomy performed in patients of subgroup 1 under the age of 3 years, good results were noted in 50% of cases, satisfactory and unsatisfactory in 25% with less severe complications (50%) (p=0.041). In children of subgroup 2, after hip open reduction, good results were obtained in 90% of cases, satisfactory in 10% with a rate of severe complications of 10%, and when this surgery was combined with Salter innominate osteotomy, femoral osteotomy, good results were noted in 75% of cases, satisfactory in 19% and unsatisfactory in 6%, with a rate of severe complications of 25% (p=0.05). Conclusion. A differentiated approach to the treatment of children with hip dislocation in amyoplasia, in our opinion, will increase the effectiveness of the treatment methods, and its introduction into clinical practice will help to improve the outcomes
Late results of treatment of thermal injury and its sequelae are presented. According the data obtained, the most severe deformities developing after a thermal injury are the flexion contractures of fingers. The results of the study are evidence of a cardinal importance of an appropriate immobilization of the affected segments and of early functional rehabilitation for prevention of contractures development. Without dynamic keeping the children under observation, secondary skeletal deformities develop which are changes in form of bones and their hypoplasia. This enhances the number and volume of reconstructive procedures needed as well as deteriorates the final outcome of the treatment.
