Background: Enterovirus D68 (EV-D68) has been detected infrequently and has not been associated with severe disease in Canada. In the early fall of 2014, following an unusual case increase in the United States, clusters of EV-D68 among children and some adults manifesting severe symptoms were reported in Canada.
Objective: To provide a surveillance update on overall and age-related pertussis trends in British Columbia (BC), Canada, spanning the 20-year period from 1993-2013.
Contexte : L'entérovirus D68 (EV-D68) est rarement détecté et n'a pas été associé à des cas graves au Canada. Au début de l'automne 2014, à la suite d'une augmentation inhabituelle de cas aux États-Unis, des grappes d'EV-D68 chez des enfants et certains adultes montrant des symptômes graves ont été signalées au Canada. Objectif : Fournir un résumé épidémiologique initial des cas pédiatriques hospitalisés avec l'EV-D68 au Canada. Méthodologie : Projet pilote de surveillance d'une durée limitée visant à recueillir des renseignements sur les cas pédiatriques (enfants et adolescents de moins de 18 ans) d'EV-D68 hospitalisés entre le 1 er et le 30 septembre 2014. Résultats : Total de 268 cas signalés en Ontario (n = 210), en Alberta (n = 45) et en Colombie-Britannique (n = 13). Parmi les 268 cas signalés, 64,9 % (n = 174) étaient des garçons; la différence relative au sexe était statistiquement significative (p < 0,01). L'âge a été signalé chez 255 cas, la moyenne d'âge chez les garçons étant de 5,4 ans et chez les filles de 5,3 ans. Parmi les cas ayant des données disponibles, 6,8 % (18/266) ont été admis dans une unité de soins intensifs. Parmi les cas de maladie clinique documentée, 98,3 % (227/231) présentaient seulement une maladie respiratoire, 0,4 % (n = 1), seulement une maladie neurologique, et 0,9 % (n = 2), les deux maladies; les cas sans maladie respiratoire ni neurologique étaient rares (n = 1). Parmi les 90 cas accompagnés d'informations cliniques supplémentaires, 43,3 % présentaient de l'asthme. Aucun des 268 cas n'a entraîné le décès du patient. Conclusion : L'éclosion de l'EV-D68 au Canada en septembre 2014 représente une nouvelle éclosion associée à une maladie grave chez les enfants. Ces observations constituent le premier résumé épidémiologique de cas graves liés à l'EV-D68, un agent pathogène respiratoire émergent au Canada. L'investigation continue de cet agent pathogène est nécessaire si l'on veut s'appuyer sur ces résultats et comprendre le spectre complet de sa maladie associée.
Fournir une mise à jour sur la surveillance des tendances relatives à la coqueluche dans l'ensemble et par groupes d'âge en Colombie-Britannique (C.-B.), au Canada, sur une période de 20 ans, soit de 1993 à 2013. Méthodes : Des données sur la surveillance des cas confirmés de coqueluche à l'échelle provinciale ont été extraites pour la période entre le 1er janvier 1993 et le 31 octobre 2013. Des taux d'incidence annuels et propres à l'âge ont été calculés à partir d'estimations dans les populations provinciales et régionales. Résultats : La C.-B. a connu une épidémie importante de coqueluche à la fin des années 1990 et au début des années 2000, avec des taux d'incidence globaux variant entre 20 et 40 cas pour 100 000 habitants et atteignant un sommet de plus de 200 cas pour 100 000 préadolescents de 10 à 13 ans au cours de l'épidémie de l'année 2000. Entre 2005 et 2011, le taux d'incidence global a chuté à des niveaux historiquement bas variant entre 1 à 6 cas pour 100 000 habitants. Cette faible activité a été suivie, en 2012, par une résurgence attribuable principalement aux éclosions dans les régions des basses-terres continentales de la C.-B., pendant laquelle l'incidence globale à l'échelle provinciale a atteint 10 cas pour 100 000 habitants. L'incidence propre à l'âge la plus élevée en 2012 a été observée chez les nourrissons de moins de 1 an (64 cas pour 100 000 habitants) et chez les enfants de 12 et 13 ans (entre 56 et 57 cas pour 100 000 habitants), alors qu'un changement dans la répartition selon l'âge, soit une diminution du nombre des enfants d'âge préscolaire et une augmentation du nombre de préadolescents et d'adolescents, était observé depuis l'année 2000. L'incidence chez les adultes est restée à moins de 10 cas pour 100 000 habitants pendant toute la période et était de 5 cas pour 100 000 habitants en 2012. Le taux d'incidence provincial cumulatif global en 2013 était de 6 cas pour 100 000 habitants, alors qu'une activité asynchrone continue était observée principalement sur l'île de Vancouver. Conclusion : L'activité de la coqueluche en C.-B. a connu des fluctuations cycliques attendues. Un sommet d'incidence a été observé en 2012, principalement chez les nourrissons et chez les préadolescents et adolescents, mais à des niveaux inférieurs aux sommets précédents. Après une épidémie importante dans les années 1990 et au début des années 2000 et l'intégration d'un vaccin anticoquelucheux acellulaire au programme de vaccination systématique, l'immuno-épidémiologie de la coqueluche pourrait être encore en transition. D'autres efforts de surveillance et d'évaluation sont requis pour déterminer les changements éventuels à apporter aux programmes de la Colombie-Britannique.
