As the bacillary load is very high in these patients, they can form a potential reservoir of the infection in the community especially in the postleprosy elimination era. Contrary to the earlier belief in the dapsone era, most of our patients manifested disease without any history of inadequate or incomplete antileprosy therapy.
Abstract:Striae distensae are common innocuous skin lesions. We report a patient of nephrotic syndrome with corticosteroid-induced large striae, which suddenly developed bulging and fluid accumulation. There were no associated cutaneous or systemic symptoms. Analysis of aspirated fluid showed it was a transudate and culture was sterile. It was concluded that edema fluid, as part of anasarca, had preferentially accumulated within the striae, probably due to their altered dermal mechanical properties. Recognizing this entity is important to avoid unnecessary alarm or irrational use of antibiotics.
Plica neuropathica is a rare acquired condition of sudden onset characterized by irreversible tangling of hair. It is sparsely reported in the literature, though it does not seem to be uncommon, at least in Hindu-religious persons of India. However, it has not been previously reported in patients as young as 2 years of age, and consequently this fact warranted reporting.
Deep learning have paved the way for scientists to achieve great technical feats. In an endeavor to hone and perfect these techniques, quantum deep learning is a promising and important tool to utilize to the fullest. Using the techniques of deep learning and supervised learning in a quantum framework, we are able to propose a quantum convolutional neural network and showcase its implementation. We keep our focus on training of the ten qubits system in a way so that it can learn from labeling of the breast cell data of Wisconsin breast cancer database and optimize the circuit parameters to obtain the minimum error. Through our study, we also have showcased that quantum convolutional neural network can outperform its classical counterpart not only in terms of accuracy but also in the aspect of better time complexity.
A 12-year-old boy presented with a progressive chronic ulcer over the left hand of 6 yearsÕ duration. The lesion had started as a skin-coloured papule over the dorsum of the left hand. It later ulcerated and gradually extended over a number of years to the adjacent area at one end and healing with scarring at the other. This led to fusion of the middle three fingers of the left hand, causing difficulty with movement. The lesion was not preceded by any local trauma or bite. He was from poor socioeconomic background and his immunization was incomplete. There was a history of weight loss for past 2 years.On physical examination, the patient was found to have mild pallor and bilateral submandibular and posterior cervical lymphadenopathy, with no systemic abnormalities. There was atrophic, hypopigmented, Ôcigarette paperÕ-like scar involving the dorsum of the left hand merging with the ulcer. A large well-defined, irregularly bordered, painless ulcer with adhesion to the underlying bone was present, involving the dorsum of the left hand, and the index, middle and ring fingers, extending to the distal part of the palm. (Fig. 1a,b) The involved three fingers were fused with each other proximally due to extensive granulation tissue and necrotic slough, distorting the finger morphology. The patient also had pediculosis capitis and pyoderma over his face.
