Kalium auf 13 mg°/o 3,32 inval/L. Abgesehen davon blieb der Patient bis jetzt ausreichend kompensiert.Uns bot sich bei wiederholten Klinikaufenthalten seit 1955, wohl infolge der seit Jahren verabfolgten Halocortine, nie das Vollbild einer Nebennierenrinden-Insuffizienz. Erwähnt seien der pathologische Ausfall des Robinson-Power-Kepler-Tests (A = 10,2) und Eosinophilen-Senkungen von etwa 30°/o bei wiederholten 4-Stunden-ACTH-Tests. Die im Ham untersuchten Fraktionen neutraler Steroide lagen unterhalb der altersentsprethenden Norm. Die schon früher festgestellten walnußgroßen Kalkschatten kranial beider Nieren ließen sich durch Schichtaufnahmen und i.v. Pyelographie mit Sicherheit den Nebennieren zuordnen (Abb. 5). Im rechten Lungenhilus fanden sich kalkdichte Herde. Die Tuberkulinprobe fiel stark positiv aus.Eine subtile Prüfung der Nebennierenrinden-Funktionsreserve erfolgte im Januar 1959. Der Mineralstoffwechsel wurde gesondert durch einen modifizierten Cutler-Power-Wilder-beziehungsweise Harrop-Test (Literatur bei Labhart) untersucht. Dabei fand sich nach Kochsalzentzug ein nur unbedeutender Rückgang der Natriumchlorid-Ausscheidung (durchschnittlich 0,5 g/die) bei deutlichem Absinken des Natrium-und Ansteigen des Kaliumspiegels un Blut. Einzelheiten siehe Abb. 6. Der Versuch läßt somit auf eine fast maximale Ausnützung der mineralocorticoiden NebennierenrindenFunktion schließen.
