Objectives:to estimate the incidence of dry eye, to identify risk factors and to establish a risk prediction model for its development in adult patients admitted to the intensive care unit of a public hospital. Method:concurrent cohort, conducted between March and June, 2014, with 230 patients admitted to an intensive care unit. Data were analyzed by bivariate descriptive statistics, with multivariate survival analysis and Cox regression. Results:53% out of 230 patients have developed dry eye, with onset mean time of 3.5 days. Independent variables that significantly and concurrently impacted the time for dry eye to occur were: O2 in room air, blinking more than five times per minute (lower risk factors) and presence of vascular disease (higher risk factor). Conclusion:dry eye is a common finding in patients admitted to adults intensive care units, and care for its prevention should be established.
Objective: to estimate the prevalence of falls, identify the associated factors and establish a prediction model for their development in patients treated by a home care program. Methods: a cross-sectional study conducted between June 2017 and January 2018, with 131 patients treated by Melhor em Casa Program, in a city in the north of Minas Gerais. We collected socio-demographic and clinical data through an instrument. Descriptive analysis was performed and the adjusted prevalence ratios were obtained by multiple Poisson regression analysis with robust variance. Results: 72 (55%) of the 131 patients were female, the average age was 66.8 years old and 43.5% had a fall. The independent variables that had a significant and joint impact on the fall outcome were: type of fall -accidental; fall location -bedroom; fall location -dining-room; fall location -kitchen; fall location -yard; fall location -street; and fear of further falls. Conclusion: falls are a common finding with home care patients and care for their prevention should be established.
Objectives:to estimate the prevalence of impaired tactile sensory perception, identify risk factors, and establish a risk prediction model among adult patients receiving antineoplastic chemotherapy. Method:historical cohort study based on information obtained from the medical files of 127 patients cared for in the cancer unit of a private hospital in a city in Minas Gerais, Brazil. Data were analyzed using descriptive and bivariate statistics, with survival and multivariate analysis by Cox regression. Results:57% of the 127 patients included in the study developed impaired tactile sensory perception. The independent variables that caused significant impact, together with time elapsed from the beginning of treatment up to the onset of the condition, were: bone, hepatic and regional lymph node metastases; alcoholism; palliative chemotherapy; and discomfort in lower limbs. Conclusion:impaired tactile sensory perception was common among adult patients during chemotherapy, indicating the need to implement interventions designed for early identification and treatment of this condition.
Objetivo: caracterizar clínica e epidemiologicamente os pacientes atendidos por um programa público de atenção domiciliar na cidade de Montes Claros, Minas Gerais, Brasil. Métodos: Estudo transversal e exploratório, realizado entre junho de 2017 e janeiro de 2018, com 131 pacientes cadastrados e atendidos pelo Serviço de Atenção Domiciliar - Melhor em Casa. Para o levantamento dos dados utilizou-se um instrumento contendo variáveis sociodemográficas, clínicas e funcionais. Os dados foram analisados por estatística descritiva. Resultados: A maioria dos pacientes eram idosos (67,9%), do sexo feminino (55%), pardos (47,3%), casados (31,3%) e/ou solteiros (31,3%), restritos ao leito (71%) e alimentavam-se via oral (68,7%). As doenças vasculares (42%) foram as mais prevalentes, 45,8% dos pacientes apresentavam lesão por pressão e o atendimento domiciliar foi realizado principalmente pelo enfermeiro (83,2%) e médico (82,4%). O artigo analisa as práticas de médicos e enfermeiros da Atenção Primária à Saúde (APS) no cuidado a idosos com demência. Foi conduzido um estudo transversal e analítico, na Atenção Primária de municípios do norte de Minas Gerais, Brasil. Aplicou-se o instrumento Atenção Sanitária às Demências: a visão da Atenção Básica, nas versões para médicos e enfermeiros. Dos 316 participantes, 138 eram médicos e 178, enfermeiros. Encontrou-se associação estatisticamente significativa entre tempo de experiência profissional no serviço e participação em atividade de capacitação em demência (médico p = 0,026; enfermeiros p = 0,049) e entre a formação dos profissionais e participação em atividade de capacitação em demência (médico p = 0,028; enfermeiro p = 0,003). Constatou-se uma prática incipiente de médicos e enfermeiros da APS no cuidado a idosos com demência, apontando para a necessidade do desenvolvimento de estratégias educativas de modo a qualificar a assistência.
Background Critically ill patients are susceptible to the development of dry eye. Few studies have been conducted on how to best prevent and treat this condition. Objective To compare the effectiveness of 2 nursing interventions in preventing dry eye in adult intensive care unit patients: liquid artificial tears (Lacribell; Latinofarma) and artificial tears gel (Vidisic Gel; Bausch and Lomb). Methods In this randomized controlled trial, 140 participants were randomly assigned to 1 of 2 treatment groups: a liquid artificial tears group (n = 70) and an artificial tears gel group (n = 70). The study inclusion criteria were as follows: admission to the intensive care unit, age of 18 years or older, no diagnosis of dry eye at admission, receipt of mechanical ventilation, blink rate of less than 5 times per minute, and a score of 7 or less on the Glasgow Coma Scale. On 5 consecutive days, a single researcher who was unaware of the treatment assignment assessed the participants’ eyes using the fluorescein eye stain test and the Schirmer test for dry eye. Results Dry eye developed in 21% of participants who received liquid artificial tears versus 9% of participants who received artificial tears gel (P = .04). Conclusions In this study, artificial tears gel was superior to liquid artificial tears in preventing the development of dry eye. These results may help nurses deliver evidence-based eye care aimed at reducing the risk of dry eye in critically ill patients.
Resumo Objetivo identificar diagnósticos de enfermagem da Taxonomia da NANDA-I em idosos institucionalizados. Método estudo descritivo, conduzido com 116 idosos de uma instituição de longa permanência, realizado a partir da identificação de diagnósticos de enfermagem da Taxonomia da NANDA-I em um instrumento fundamentado no modelo teórico das Necessidades Humanas Básicas e validação por especialistas com suporte no raciocínio diagnóstico de Risner. Resultados identificou-se 1.555 títulos de diagnósticos de enfermagem para os 116 idosos, com uma média de 13,4 diagnósticos por idoso. Após exclusão de repetições foram obtidos 39 títulos distintos de diagnósticos. Entre eles, 26 (66,7%) são títulos de diagnósticos reais, 13 (33,3%) títulos de diagnósticos de risco e estão classificados em 8 dos 13 domínios da taxonomia da NANDA-I. Os diagnósticos de enfermagem mais frequentes foram: risco de quedas (94,8%); síndrome do idoso frágil (91,3%) e processos familiares disfuncionais (90,5%). Conclusão e implicações para a prática o perfil de diagnósticos de enfermagem identificado pode contribuir para o incremento de indicadores sensíveis à prática de enfermagem, com o planejamento e implementação de medidas assistenciais direcionadas as reais necessidades dos idosos institucionalizados impactando na autonomia, independência e consequentemente na qualidade de vida.
Objective: to identify Nursing diagnoses in patients in home care by cross-mapping terms obtained in the anamnesis and clinical examination, with NANDA-I Taxonomy. Method: descriptive exploratory study, transversal type, performed with 131 patients from a home health care program in northern Minas Gerais State. With the aid of an instrument based on the theoretical model of Basic Human Needs, the terms indicators of conditions that required nursing interventions were extracted. The cross-mapping of these terms was also performed with NANDA-I Taxonomy diagnoses. Results: 378 terms and expressions referring to 49 different diagnoses were identified in 11 of the 13 domains of NANDA-I Taxonomy. Conclusion: the profile of identified nursing diagnoses can contribute to care management and organizational processes of nurses who provide care to patients in home care.
