Purpose It was aimed to investigate the frequency of the risk of diabetes-specific eating disorder (DSED) in adolescents with type 1 diabetes mellitus (T1DM) and to reveal the accompanying psychopathologies. Methods Adolescents with T1DM aged 12–18 who applied to the pediatric diabetes outpatient clinic between July 2021 and March 2022 were included. Diabetes Eating Problem Survey-Revised (DEPS-R) was applied to all patients to determine the risk of DSED. In order to detect accompanying psychopathologies, Eating Disorder Examination Questionnaire (EDE-Q), Child Anxiety and Depression Scale-Child version (RCADS) and Parenting Style Scale were applied. After completing the scales, semi-structured interviews were conducted with all patients by a child and adolescent psychiatrist. Results Ninety-two adolescents (45 boys, 47 girls) were included. DSED risk was found in 23.9% of the cases. A positive correlation was found between DEPS-R and EDE-Q scores (p = 0.001, rho = 0.370). RCADS mean scores were significantly higher in the group with DSED risk (p < 0.001). When the Parenting Style Scale was evaluated, psychological autonomy scores were significantly lower in the group with DSED risk (p = 0.029). As a result of the psychiatric interviews, 30 (32.6%) patients had at least 1 psychiatric disorder. Of these, 2 patients were diagnosed with eating disorder. Conclusion Almost one-fourth of adolescents with T1DM were found to be at risk of DSED. Routine screening of adolescents with T1DM with the DEPS-R scale may provide early detection of DSED, and referral of those at risk to child psychiatry enables early diagnosis and intervention for both eating disorders and accompanying psychopathologies. the effects of ımpaired fasting glucose and ımpaired glucose tolerance on diabetic development risks on hba1c levels: A retrospective study.
Amaç: Tip 1 diabetes mellitus, insülin eksikliği ve hiperglisemi ile karakterize kronik bir hastalıktır. İyi bir glisemik kontrolün için yaşam boyunca günde ortalama en az sekiz defa kan şekeri ölçümü, sağlıklı besinlerin seçimi, porsiyon kontrolü ve düzenli egzersiz gerekmektedir ve tüm bunlar diyabetli bireylerin birçoğunda kaygıya neden olmakta ve yeme bozuklukları riskini arttırmaktadır. Diabulimia, diyabetli bireylerde, vücut ağırlığında kayıp sağlama amacıyla, kasıtlı olarak insülin dozunu azaltmak ve/veya atlamayı ifade eder. Bu çalışmanın amacı diyabete özgü yeme bozukluğu olan diabulimia açısından riskli adölesanların sıklığını araştırmak, yeme bozukluğu gelişmesi riski ile metabolik kontrol ve vücut kitle indeksi arasındaki ilişkisini değerlendirmektir. Gereç ve Yöntem: Çalışmaya Tip 1 diabetes mellitus tanısıyla izlenen 10-18 yaş arası 129 olgu dahil edildi. Katılımcılara DEPS-R (Diabetes Eating Problem Survey, Diyabette Yeme Sorunları Anketi) yüz yüze uygulandı. Demografik, antropometrik parametrelere ilişkin veriler ve HbA1c değerleri hastaların tıbbi dosyalarından elde edildi. Bulgular: Katılımcıların DEPS-R puanları incelendiğinde riskli olguların %53,2‟si (n=25) kız, %46,8‟i (n=22) erkek, toplamda 47 olguda (%36,4) bozulmuş yeme davranışı riski saptandı. Olgular DEPS-R puanına göre yeme davranışı bozukluğu açısından risksiz ve riskli iki gruba ayrılarak incelendiğinde HbA1c ve vücut kitle indeksi riskli grupta anlamlı olarak yüksek saptandı. DEPS-R skoru ile diyabet yaşı, vücut kitle indeksi ve HbA1c değeri arasında zayıf korelasyon saptandı. Sonuç: Riskli diyabetli bireylerde diabulimia gelişmesini önlemek ve hali hazırda yeme bozukluğu geliştirmiş çocukların saptanmasında rutin tarama önem kazanmaktadır. Bu konu ile ilgili daha fazla çalışmaya ihtiyaç olmakla beraber özellikle açıklanamayan kilo kaybı ve kötü metabolik kontrolde diabulimia olasılığı akılda tutulmalıdır ve etkili tedavi için klinisyenlerin farkındalığı arttırmalıdır.
Vitamin B12 ve depresyon-aksiyete bozuklukları ilişkisi: Retrospektif kohort çalışma. Arch Clin Exp Med. 2017;2(1):6-8. ÖzAmaç: Vitamin B12 eksikliğinde bilişsel fonksiyonlarda gerileme ile depresyon ve anksiyete bozukluklarının geliştiği bildirilmiştir. Çalışmamızın amacı sık karşılaşılan depresyon ve anksiyete bozuklukları ile vitamin B12 eksikliği ilişkisini araştırmaktır. Yöntemler: 2011-2015 yılları arasında vitamin B12 düzeyi ölçülen hastalar, hastane bilgi sistemi üzerinden tarandı. B12 vitamin düzeyi elektrokemilüminesans immunoassay yöntemi ile ölçüldü, 200 pg/ml altı B12 vitamin eksikliği, 200 pg/ml ve üzeri normal olarak değerlendirildi. Hastalar başvuru tanılarına göre iki gruba ayrıldı. Depresyon veya anksiyete bozukluğu tanıları alan hastalar grup 1, bu tanıları almayan hastalar ise grup 2 olarak ele alındı. Grup 1 ve grup 2'de vitamin B12 eksikliği görülme açısından karşılaştırıldı. Verilerin analizinde tanımlayıcı istatistikler, sürekli değişkenler için ortalama ve standart sapma, kategorik veriler için ise sayı ve yüzde kullanıldı. Karşılaştırmalar için Ki-kare testi kullanıldı. Anlamlılık %95 güvenlik aralığında değerlendirilip, p<0.05 anlamlı kabul edildi. Bulgular: Çalışma döneminde hastanemize başvuran hastaların 255.852'sinde vitamin B12 düzeyi ölçüldüğü tespit edildi. Grup 1'deki hastaların 217'sinde (%16,1), grup 2'deki hastaların ise 40.442'sinde (%15,9) vitamin B12 eksikliği (<200 pg/ml) saptandı. Grup 1'deki 1.129 ve grup 2'deki 214.064 hastanın vitamin B12 düzeyleri normal olarak değerlendirildi. Görülme sıklığı arasında istatistiksel olarak anlamlı bir fark saptanmadı (p=0,864). Sonuç: Çalışmamızda depresyon veya anksiyete bozukluğu olan hastalarda vitamin B12 eksikliğinin daha fazla oranda görüldüğüne dair anlamlı bir veri saptanmadı. Konu üzerine daha kesin yorum yapılabilmesi için prospektif, kontrollü çalışmalara ihtiyaç vardır. Aim of our study is to investigate the relationship between these disorders and vitamin B12 deficiency. Methods: Patients whose vitamin B12 levels were measured between 2011 and 2015 were searched through the hospital information system. Vitamin B12 levels were measured by electrochemiluminescence immunoassay. The values <200 pg/mL and ≥200 pg/mL were considered as B12 vitamin deficiency and normal, respectively. The patients were divided into two groups according to their diagnosis. The patients who were diagnosed with depression or anxiety disorder were treated as group 1 and the patients without these diagnoses as group 2. Objective: Early detection of malnutrition risk in hospitalized children can improve health outcomes and quality of life; however, the number of studies where the pediatric screening tool is appropriate for Turkish children is limited. Therefore, this article aims to determine the prevalence of malnutrition risk in pediatric patients evaluated with Screening Tool for Risk on Nutritional Status and Growth, Screening Tool for the Assessment of Malnutrition in Pediatrics, Pediatric Yorkhill Malnutrition Score, and Simple Pediatric Nutrition Screening Tool with original and adjusted cutoffs and to evaluate which pediatric screening tool is appropriate for Turkish children. Methods: In this cross-sectional study, four published nutritional risk screening tools (Screening Tool for Risk on Nutritional Status and Growth, Screening Tool for the Assessment of Malnutrition in Pediatrics, Pediatric Yorkhill Malnutrition Score, Pediatric Nutrition Screening Tool) were applied to pediatric inpatients (n=604) aged 1 month to 17 years, admitted to a pediatric ward for at least 24 hours. Results: Pediatric Nutrition Screening Tool with adjusted cutoffs had the greatest recognition rate (94.2%) of acute malnutrition. Having a high nutritional risk by Pediatric Yorkhill Malnutrition Score was associated with an increased risk of acute (OR: 6.57 for Screening Tool for Risk on Nutritional Status and Growth, 5.84 for Screening Tool for the Assessment of Malnutrition in Pediatrics, and 20.35 for Pediatric Yorkhill Malnutrition Score) and chronic malnutrition (OR: 1.27 for Screening Tool for Risk on Nutritional Status and Growth, 3.28 for Screening Tool for the Assessment of Malnutrition in Pediatrics, and 1.72 for Pediatric Yorkhill Malnutrition Score). Classifying the at-risk category by the Pediatric Nutrition Screening Tool was related to raised odds of malnutrition (OR: 2.64 for original and 5.24 for adjusted cutoffs). This positive association was also observed for acute (OR: 4.07 for original cutoffs, and 28.01 for adjusted cutoffs) and chronic malnutrition (OR: 1.14 for original cutoffs, and 1.67 for adjusted cutoffs). Conclusion: Pediatric Nutrition Screening Tool with adjusted cutoffs and Pediatric Yorkhill Malnutrition Score have higher diagnostic accuracy than other screening tools in assessing the nutritional status of hospitalized Turkish children and detecting children, particularly with acute malnutrition.
