Introducción y objetivos. El tumor tritón maligno (TTM) es un tipo de tumor poco frecuente, de características agresivas, que se asocia con altas tasas de enfermedad metastásica y recurrencia. La caracterización de la neoplasia por medio de inmunohistoquímica es fundamental para valorar el componente neural y la diferenciación rabdomioblástica. No hay suficiente soporte en la literatura con respecto al manejo óptimo dada la rareza de su presentación; se discute el caso de una paciente con un TTM metastásico inicialmente manejado con radioterapia.
Se presenta el caso de una paciente de 46 años de edad con cinco años de evolución de episodios intermitentes de hemoptisis cuyo diagnóstico final fue hemosiderosis pulmonar idiopática. Su presentación y características clínicas son comparadas con los otros casos reportados en la literatura.
