OBJETIVO: descrever os indicadores de risco para perda auditiva presentes em neonatos e lactentes que realizaram a Triagem Auditiva Neonatal no Hospital das Clínicas da Universidade Federal de Pernambuco, nascidos em 2008. MÉTODOS: foram pesquisados os 787 neonatos e lactentes que realizaram a Triagem Auditiva Neonatal no citado Hospital, nascidos em 2008. Foi montado um banco de dados com informações do formulário com histórico familiar e clínico dos pesquisados e resultado da triagem, para análise dos indicadores de risco. RESULTADOS: os indicadores de risco mais prevalentes na população estudada foram presença de hiperbilirrubinemia, nascimento pré-termo, baixo peso ao nascimento, uso de medicamento durante o período gestacional, permanência em Unidade de Terapia intensiva e presença de infecções intra-uterinas durante a gestação. Os indicadores de risco para perda auditiva com associação estatisticamente significante com o resultado falha na triagem foram nascimento pré-termo, baixo peso, permanência em Unidade de Terapia Intensiva, uso de ventilação mecânica e uso de medicamento ototóxicos. CONCLUSÃO: houve ocorrência de indicadores de risco pré, peri e pós-natais, porém apenas foi encontrada significância estatística entre alguns indicadores peri e pós-natais e a falha na triagem.
Objective The purpose of this study was to assess age-related changes in temporal resolution in listeners with relatively normal audiograms. The hypothesis was that increased susceptibility to non-simultaneous masking contributes to the hearing difficulties experienced by older listeners in complex fluctuating backgrounds. Design Participants included younger (n = 11), middle-aged (n = 12), and older (n = 11) listeners with relatively normal audiograms. The first phase of the study measured masking period patterns for speech-shaped noise maskers and signals. From these data, temporal window shapes were derived. The second phase measured forward-masking functions, and assessed how well the temporal window fits accounted for these data. Results The masking period patterns demonstrated increased susceptibility to backward masking in the older listeners, compatible with a more symmetric temporal window in this group. The forward-masking functions exhibited an age-related decline in recovery to baseline thresholds, and there was also an increase in the variability of the temporal window fits to these data. Conclusions This study demonstrated an age-related increase in susceptibility to non-simultaneous masking, supporting the hypothesis that exacerbated non-simultaneous masking contributes to age-related difficulties understanding speech in fluctuating noise. Further support for this hypothesis comes from limited speech-in-noise data suggesting an association between susceptibility to forward masking and speech understanding in modulated noise.
OBJETIVO: Descrever alguns aspectos demográficos e socioeconômicos de mães de neonatos e lactentes atendidos no serviço de Triagem Auditiva Neonatal, do Hospital das Clínicas, da Universidade Federal Pernambuco (HC-UFPE), nascidos no ano de 2007. MÉTODOS: A coleta de dados foi constituída por leitura e digitação das informações contidas no banco de dados referente ao programa de Triagem Auditiva Neonatal do HC- UFPE. Participaram da pesquisa 1.193 mães. RESULTADOS: Entre as variáveis demográficas do estudo destaca-se que o maior percentual (47,9%) das mães tinha entre 20 e 29 anos de idade e 69,2% eram casadas. Dentre as variáveis socioeconômicas, 36,1% possuíam o 1° grau incompleto, 55,7% eram donas de casa, 38,6% não possuíam renda pessoal, 60,6% dispunham de renda familiar entre um a três salários mínimos e 92,5% tiveram seus filhos nascidos em maternidades públicas. CONCLUSÃO: A população estudada encontra-se em condições consideradas desfavoráveis para a saúde e desenvolvimento global dos neonatos e lactentes, pois as gestantes com este perfil pertencem a uma categoria de risco. Esses resultados servem de alerta para o desenvolvimento de ações que visem à promoção da saúde na população investigada.
Purpose To analyze the effect of masking on the Cortical Auditory Evoked Potential with speech stimulus in young adults. Methods Fourteen individuals aged between 19 and 28 years of both sexes with no hearing loss participated in the study. The Cortical Auditory Evoked Potential examination was performed with synthetic speech stimulus /ba/ simultaneous to Speech Shaped Noise presented under three conditions: steady noise with a 30 dB SPLep intensity (weak steady noise), steady noise with a 65 dB SPLep intensity o (strong steady noise) and modulated noise with 30 dB SPLep and 65 dB SPLep intensities at 25Hz and modulation period of 40 ms. Results Higher latencies were observed in the cortical components, except P2, in the condition of strong steady noise and more meaningful measures of amplitude of the cortical components P1, N1 and P2 in the condition of modulated noise with statistically significant difference in comparison to the strong steady noise condition. There was worse wave morphology in the condition of strong steady noise, when compared to the other records. The average electrophysiological thresholds for the conditions of strong steady noise and modulated noise were 60 dB SPLep and 49 dB SPLep, respectively, showing a 11.7 dB mean difference. Conclusion We could infer that there was a lower masking effect of modulated noise when compared to the strong steady noise condition, in the amplitude measurements of the cortical components and an average difference of 11.7 dB between the electrophysiological thresholds (interpreted as the measure of the Masking Release).
Forward masking of the sABR provides an objective measure of the susceptibility of the auditory system to prior stimulation. Failure of recovery functions to return to baseline suggests an interaction between forward masking by the prior masker and temporal effects within the stimulus itself.
Objective The Brazilian-Portuguese Hearing In Noise Test (HINT) was used to investigate the benefit to speech recognition of listening in a fluctuating background. The goal was to determine whether modulation masking release varied as a function of the speech-to-masker ratio at threshold. Speech-to-masker ratio at threshold was manipulated using the novel approach of adjusting the time-compression of the speech. Design Experiment 1 measured performance-intensity functions in both a steady speech-shaped noise masker and a 10-Hz square-wave modulated masker. Experiment 2 measured speech-to-masker ratios at threshold as a function of time-compression of the speech (0, 33, and 50%) in both maskers. Study Sample Participants were normal-hearing adults who were native speakers of Brazilian Portuguese (Experiment 1: N = 10; Experiment 2: N = 30). Results The slope of the performance-intensity function was shallower in the modulated masker than in the steady masker for both words and sentences. Thresholds increased with increasing time-compression in both maskers, but more markedly in the modulated masker, resulting in reduced modulation masking release with increasing time-compression. Conclusions Speech-to-masker ratio at threshold varies with time-compression of speech. The results are relevant to the issue of whether degree of masker modulation benefit depends on speech-to-masker ratio at threshold.
OBJETIVO: Este estudo investigou o efeito das diferentes taxas de modulações do mascaramento na magnitude do masking release. MÉTODOS: Quinze indivíduos jovens, com audição normal, foram submetidos ao teste de reconhecimento de sentença na presença de ruído, utilizando as listas de sentenças do HINT-Brasil. Foram obtidos limiares de reconhecimento de fala em presença de ruído estável e ruído modulado, em diferentes taxas de modulação (4, 8, 16, 32 e 64 Hz). A magnitude do masking release foi obtida para cada modulação e foi realizada a análise comparativa dos resultados. RESULTADOS: Os achados demonstraram melhores limiares de reconhecimento de sentenças quando o ruído mascarante foi modulado em 4, 8, 16 e 32 Hz e piores limiares quando o ruído mascarante estava estável e em 64 Hz. No que diz respeito à análise da relação sinal/ruído, foram observados, no presente estudo, maiores valores para as tarefas que envolvem reconhecimento de sentenças com ruído estável, seguidos das tarefas que envolvem reconhecimento de sentenças com ruído modulado em 64 Hz, e menores valores para as tarefas que envolvem reconhecimento de sentenças com ruído modulado em 32, 16, 8 e 4 Hz, respectivamente. CONCLUSÃO: A magnitude do masking release para sentenças não se diferencia com taxas de modulação em amplitude entre 4 e 32 Hz. No entanto, quando a taxa de modulação é elevada a 64 Hz, a magnitude do masking release diminui.
