Este artigo investiga a efetiva participação de Conselhos Municipais de Saúde na gestão das políticas de saúde e avalia o seu potencial quanto à reestruturação da atenção à saúde através da estratégia da saúde da família. Toma como lócus da pesquisa os nove municípios da Região Centro-Oeste incluídos nos Estudos de Linha de Base para a avaliação do Proesf. Parte do pressuposto de que o formato institucional é relevante, pois define oportunidades de participação e articulação. Foi examinada a legislação sobre os conselhos e foram entrevistados os conselheiros usuários integrantes da mesa diretora, abordando diversos aspectos do Conselho de Saúde. Na análise dos dados foram focalizadas, principalmente, a composição dos conselhos, as entidades que representam os usuários e as regras formais de funcionamento, além de problemas-chave, tais como o acesso seletivo de entidades aos conselhos e a legitimidade das decisões. Os resultados sugerem que os conselhos têm pouco impacto sobre a reestruturação dos serviços de saúde. Mesmo assim, exercem controle social sobre a gestão do sistema de saúde, incluindo novos atores na discussão de suas políticas, e permitem decisões legítimas, pois em consonância com o discurso sobre o SUS.
