Resumo: O cortisol é conceituado como hormônio do estresse, produzido pelas Glândulas Suprarrenais. Este atua regulando funções metabólicas, imunológicas, homeostáticas, e também na relação de comportamentos sociais. Com isso, níveis desregulados alteram o Sistema Homeostático, trazendo malefícios para a saúde. Objetivo: Identificar a influência dos níveis de cortisol na imunidade e na saúde mental. Método: Revisão integrativa da literatura, desenvolvida a partir de 17 artigos obtidos da base de dados PubMed®, e 1 artigo encontrado na base de dados Periódicos CAPES em que recorreram-se aos mesmos Descritores em Ciências da Saúde, totalizando 18 trabalhos analisados, com um recorte temporal contemplando artigos dos últimos 5 anos. Resultados: Conforme os achados, o Cortisol em níveis altos está mais relacionado aos estressores psicossociais e também a depressão, quando baixo, pode afetar aspectos cognitivos e comportamentais. Sob o âmbito da imunidade, alto nível do hormônio está relacionado com a redução do Sistema Imunológico, e em níveis baixos exacerba a reposta imunológica, fazendo com que haja uma resposta inflamatória exagerada, aumentando assim os riscos de suscetibilidade a alguns patógenos, além disso, trazendo riscos para a evolução de câncer. Conclusão: Diante dos níveis indicados do Cortisol, vale salientar que os níveis elevados e reduzidos alteram tanto a saúde mental como o equilíbrio imunológico.
