Modifications in oral biofilms of irradiated patients showed association with xerostomia and hygiene conditions, which reinforces the necessity of improving patient compliance to oral health care programs.
ResumoAs osteomielites dos maxilares são bastante comuns nos países em desenvolvimento, sendo que em alguns casos o diagnóstico, assim como a terapêutica, são por vezes comprometidos em virtude da variedade microbiana associada a essas condições clínicas além de terapêuticas incorretas. Assim, foi objetivo dos autores discutir os principais aspectos de diagnóstico sejam eles clínicos e imaginológicos assim como as terapêuticas associadas por meio de um caso clínico de paciente de 42 anos de idade do gênero masculino queixando-se de dor em região mandibular esquerda e presença de secreção purulenta que persistia há mais de um mês. Ao exame clínico extrabucal pôde-se constatar a presença de três fístulas em região de ângulo mandibular esquerdo. A ortopantomografia evidenciou lesões líticas e espessamento cortical com aspecto de "roído por traça" associada a áreas compatíveis com seqüestros ósseos. Baseado nas características anteriores, o diagnóstico foi de osteomielite crônica mandibular. A terapia incluiu combinação de antibioticoterapia e procedimentos cirúrgicos como a sequestrectomia, debridamento, decorticação com resultados satisfatórios. Assim, o tratamento cirúrgico local adquire uma grande importância no resultado da terapia, associado com o uso de antimicrobianos como coadjuvantes, fundamental importância no êxito da terapêutica, sem deixar de notar a importância de um tratamento precoce para o sucesso da mesma. AbstractOsteomyelitis of the jaws are quite common in developing countries, and in some cases the diagnosis and therapy are sometimes compromised because of the variety microbial associated with these clinical conditions associated with therapeutic incorrect. Thus, it was the objective of the authors discuss the main aspects of diagnosis whether clinical and imaging as well as therapeutic linked through a clinical case of a 42 year-old male complaining of pain in left mandibular region and the presence of secretion pus that has persisted for over 1 month. On examination Clinical headgear could be verified the presence of 3 fistulas in left mandibular angle region. The panoramic radiography showed osteolytic lesions and cortical thickening with the appearance of "motheaten" associated with areas compatible with bone sequestration. Based on previous features, the diagnosis was chronic osteomyelitis of the mandible. Therapy included combination of antibiotics and procedures of sequestrectomy surgical, debridement, decortication with satisfactory results. Thus, treatment surgical site becomes more important in result of therapy, associated with the use of antimicrobials as adjuncts, fundamental importance to the success of therapy without noticing the importance of early treatment for successful therapy. The aim of this study was to compare the efficacy of two different treatment of odontogenic cellulitis in pediatric patients: conservative treatment (just intravenous antibiotic administration and tooth root treatment or tooth extraction) and surgical treatment (intravenous antibiotic administration, incision, drainage and tooth root treatment or tooth extraction). The search strategy was conducted based on the model of systematic review adopted by PRISMA guidelines. The search strategy included: Pubmed/MEDLINE, Web of Science, Scopus and Cochrane Library databases seeking clinical trials with, unless, ten pediatric patients, in English and available on-line. The key-words used in the screening were “Infection Control, Dental and Cellulitis OR odontogenic infection and cellulitis”. A total of 404 studies were obtained and the selection of manuscripts was performed based on the defined inclusion and exclusion criteria and four manuscripts were according with review. It was verified that the surgeons of these studies commonly choose the conservative treatment, varying the adherence rate between 75% and 95.2% and no complications were associated with this treatment. Likewise, 75% of the author support the conservative treatment. The conservative treatment of cellulitis in pediatric patients, presents efficacy without having to submit the patient to surgical drainage.
Currently, there is a constant search for new chemical compounds through plant extracts and phytochemical compounds that do not cause so many side effects in the oral cavity and that have good antimicrobial properties. Therefore, the present study evaluated the synergistic antimicrobial efficacy of mouthwashes containing thymol and carvacrol when compared with 0.12% chlorhexidine mouthwash after bilateral lower third molar extraction. Nineteen healthy (male and female) patients (n = 19) aged between 18 and 30 years, of both sexes, treated at the Faculty of Dentistry of Mato Grosso do Sul (UFMS, MS, Brazil) participated in this study. All patients were submitted to two different therapeutic treatments, divided into two groups: group 1 (control) - patient used 0.12% chlorhexidine mouthwash solution; group 2 (experimental) - patient used 0.05% thymol and 0.025% carvacrol solutions. The suture was removed at seven postoperative days and aseptically transferred to a tube containing sterile potassium buffer saline solution. Samples were submitted to microbiological analysis at the Microbiology Laboratory at UFMS. The mean and standard deviation of colony forming units/ml of isolate microorganisms in the suture grown in BHI agar obtained from the control and experimental groups were 4.766 x 106 ± 4.069 and 4.847 x 106 ± 3.971, respectively. In conclusion, the thymol and carvacrol based mouthwash showed antimicrobial effectivity in biofilm adherence of the silk thread after extraction of third molars.
O tumor odontogênico queratocístico (TOQ) é uma patologia benigna, que apresenta controvérsias em seu diagnóstico e tratamento. Ocorre principalmente em região de ângulo mandibular, podendo estar ou não relacionada a um elemento dentário, comportamento agressivo e alta taxa de recorrência. A descoberta destas lesões muitas vezes é dada em exames de rotina, já que a maioria das lesões é assintomática e o diagnóstico definitivo é dado somente através do anatomohistopatológico. O objetivo deste trabalho é direcionar a melhor conduta ao cirurgião diante do tumor odontogênico de maior relevância clínica na odontologia através de diferentes formas de tratamento. Paciente do sexo feminino, 16 anos, feoderma, atendida no Hospital Universitário Maria Aparecida Pedrossian – UFMS, pela equipe de Cirurgia e Traumatologia Buco Maxilo-Facial acometida por um tumor odontogênico queratocístico com transformação ameloblástica em região posterior de mandíbula tratado com ressecção segmentar. A reconstrução no mesmo tempo operatório utilizou enxerto ósseo autógeno de crista ilíaca. Após quatro meses de acompanhamento observou-se oclusão satisfatória, sem queixas álgicas, sem mobilidade da placa de reconstrução, taxa de reabsorção do enxerto com valores normais. O tratamento dos ameloblastomas deve ser individualizado de acordo com tamanho da lesão, localização anatômica, idade do paciente, agressividade da mesma e imagem radiológica, sendo que as lesões multicísticas devem ser submetidas a ressecções mais amplas, com tecido sadio como margem de segurança para evitar recidivas. Portanto, não se deve advogar um único tipo de tratamento para pacientes com ameloblastoma.Descritores: Ameloblastoma; Diagnóstico; Etiologia; Terapêutica.
The superior orbital fissure is a crack that communicates with the middle cranial fossa and is located between the greater and lesser wings of the sphenoid in the posterior region of the orbit. It presents a close relationship with several important structures such as the optic foramen and II, III, IV, V and VI cranial nerve and the sphenoid and ethmoid sinuses. Zygomatic-orbital fracture may trauma in the posterior region of the orbit and may result in the Superior Orbital Fissure Syndrome (SOFS) or Apex Orbital Syndrome (AOS). The superior orbital fissure syndrome results from compression structures that pass this region, resulting in paralysis of the cranial nerves III, IV and VI. Clinically, the patient has ptosis, mydriasis and ophthalmoplegia, it is very important that the diagnosis is made before the surgical manipulation of reduction and fixation of zygomatic fracture. Case report: A male patient, 37, hit and run victim was survived by Trauma Surgery Team. Diagnosed with head trauma and diffuse axonal injury, it was transferred to the Intensive Care Unit (ICU) where he stayed for three weeks. It was conducted by the Oral and Maxillofacial Surgery and was diagnosed zygomaticomaxillary fracture type Le Fort III and mandibular fracture associated with SOFS. The surgical treatment of mandibular and zygomaticomaxillary fractures was conducted. The ptosis devolved and postoperative 6 months was noted complete remission of the syndrome signals. ResumoA fissura orbitária superior é uma fenda que se comunica com a fossa craniana média e localiza-se entre as asas maior e menor do esfenoide na região posterior da órbita. Apresenta íntima relação com diversas estruturas nobres, como o forame óptico e os II, III, IV, V e VI pares cranianos e os seios esfenoidal e etmoidal. Fratura zigomático-orbitária pode desencadear trauma na região posterior da órbita, podendo resultar na Síndrome da Fissura Orbitária Superior (SFOS) ou na Síndrome do Ápice Orbitário (SAO). A síndrome da fissura orbitária superior é resultante da compressão de estruturas que passam nessa região, resultando na paralisia dos pares cranianos III, IV e VI. Clinicamente o paciente apresenta ptose palpebral, midríase e oftalmoplegia, sendo muito importante que o diagnóstico seja feito anteriormente a manipulação cirúrgica de redução e fixação da fratura de zigoma. Relato de caso: Paciente do gênero masculino, 37 anos, vítima de atropelamento, foi socorrido por Equipe de Cirurgia do Trauma. Com diagnóstico de traumatismo craniano e lesão axonal difusa, o mesmo foi transferido para Unidade de Terapia Intensiva (UTI) aonde permaneceu por 3 semanas. Foi reavaliado pela Cirurgia e Traumatologia Bucomaxilofacial e diagnosticou-se fratura zigomático-maxilar do Tipo Le fort III e fratura mandibular associado a SFOS. ResumoO assoalho orbital é um dos locais mais comumente acometidos nas fraturas faciais, podendo se apresentar como fratura de assoalho pura ou como parte de outras fraturas do terço médio da face. Sendo assim foi realizado um relato de caso de paciente vítima de acidente motociclístico que apresentava equimose periorbitária do lado esquerdo, degrau ósseo em pilar zigomático-maxilar ipsilateral assim como parestesia infra-orbital do mesmo lado. Ao exame oftalmológico, a acuidade visual, a motilidade ocular e o reflexo fotomotor estavam preservados, ausência de diplopia e enoftalmia. Foi constatado clinica e imaginologicamente fratura de assoalho orbital, sutura fronto-zigomática, pilar zigomático-maxilar, pilar canino esquerdo e septo nasal. Foi então realizado sob anestesia geral a redução e fixação das fraturas mencionadas meio do acesso subtarsal para a fratura orbital, associada a enxerto de cartilagem auricular removido no mesmo ato cirúrgico. Em pós-operatórios de 15 dias, 7 meses e 9 meses aspectos de normalidade e processo normal de cicatrização foram observados assim como manutenção da acuidade e motilidade ocular mantiveram-se preservados. AbstractThe orbital floor is one of the most commonly affected sites in facial fractures, which may present as pure floor fracture or as part of other middle third fractures of the face. So we conducted a case report of a patient victim of motorcycle accident that had periorbital ecchymosis on the left, bony step in zygomaticomaxillary ipsilateral pillar as well as infra-orbital parestesia the same side. On examination, visual acuity, ocular motility and reflection fotomotor were preserved, no diplopia and enophthalmos. clinical and imaginologicamente orbital floor fracture, fronto-zygomatic suture, zygomaticomaxillary pillar, left canine pillar and nasal septum was found. It was then performed under general anesthesia reduction and fixation of fractures mentioned through subtarsal access to orbital fracture associated with auricular cartilage graft removed during the same operation. At 15 days postoperatively 7 months and 9 months aspects of normality and normal healing process were observed as well as maintenance of visual acuity and ocular motility remained preserved.
