Este artigo buscou reunir, avaliar e sintetizar estudos acerca dos sintomas e dos cuidados gerais em mulheres com Transtorno disfórico pré-menstrual (TDPM), bem como a interferência de tais sintomas nos campos de atuações sociais e na qualidade de vida dessas mulheres. O diagnóstico do TDPM é feito com base no Manual Diagnóstico e Estatístico de Transtornos Mentais (DSM-V) e caracteriza-se por um grupo de manifestações com exacerbação grave na semana anterior à menstruação. As consequências do TDPM na qualidade de vida das mulheres são, sobretudo, nos quesitos de relacionamentos e trabalho, com possibilidade de desfechos graves, como o suicídio. Os resultados apontam que o TDPM é um distúrbio grave que influencia a vida particular e profissional da paciente e, por isso, deve ser feita entrevista e anamnese precisa a fim de reconhecer o quadro clínico e, por conseguinte, dar seguimento ao tratamento adequado para um melhor prognóstico e bem estar global da paciente.
Este artigo buscou enfatizar a importância de uma abordagem precoce da SOP nos diversos setores da medicina. Concomitantemente, expôs o quadro clínico das pacientes afetadas, nos âmbitos psicológico, endócrino e dermatológico. Pode-se destacar como principais mudanças estéticas a acne, o hirsutismo, a dermatite seborréica, a alopecia androgênica, a acanthosis nigricans e a obesidade, todas influenciadas por fatores genéticos e ambientais. Além disso, a infertilidade também é uma consequência agravante para quadros depressivos. Para essas manifestações, respectivamente, a isotretinoína, o anticoncepcional combinado, mudanças dos hábitos de vida e a metformina devem ser utilizados para minimizar os efeitos deixados pela síndrome, podendo, ou não, serem usados em associação, conforme a necessidade individual de cada paciente. Nesta revisão bibliográfica, concluiu-se que um tratamento prévio e multidisciplinar evita fármacos desnecessários e transtornos psicológicos (estresse, ansiedade, depressão e quadros psicóticos), advindos das sequelas deixadas por esta doença, quando não há o acompanhamento conveniente.
Objetivo: Ressaltar e revisar os principais fatores de risco para a Síndrome dos Ovários Policísticos (SOP) e explorar os métodos terapêuticos capazes de promover uma melhora na qualidade de vida das pacientes. Revisão bibliográfica: É possível verificar a abordagem das suas principais alterações fisiopatológicas, sendo a hipersecreção de LH patognomônica da doença e, em consequência, o aumento de hormônios androgênicos. A obesidade, o desequilíbrio hormonal, o aparecimento precoce de pelos e o baixo peso em recém-nascidos e o processo de resistência à insulina são descritos como alguns dos fatores predisponentes dessa patologia. A SOP cursa com manifestações clínicas variadas dos sistemas endócrino e ginecológico, podendo se somar a sinais e sintomas do sistema cardíaco e psicológico. Sua terapêutica deve ser baseada na sintomatologia, conforme a individualidade de cada paciente, buscando benefícios na vida reprodutiva e mudança no estilo de vida para melhor promoção de saúde das pacientes. Considerações finais: O artigo conclui que a Mudança do Estilo de Vida (MEV), com a melhora da alimentação e a prática de exercícios físicos, é a base terapêutica para essa síndrome. Aliada ao MEV, a Metformina tem mostrado bons resultados nos transtornos menstruais e indução ovulatória, importante na vida reprodutiva dessas mulheres.
Esse artigo buscou analisar, por meio de uma revisão de literatura, a produção científica acerca da abordagem aos pacientes com doença trofoblástica gestacional (DTG), a fim de discutir os conhecimentos atuais sobre seu diagnóstico e tratamento. As DTGs constituem-se um grupo de tumores relacionados à gestação, e que apresentam em comum, uma hiperproliferação trofoblástica. Dentre as DTGs, a condição mais frequentemente observada é a mola hidatiforme (MH). A MH apresenta desafios para a realização do diagnóstico precoce, tratamento imediato, controle de complicações iniciais e tardias, e seguimento pós-molar adequado. O diagnóstico na maioria das vezes é feito através do exame clínico, da dosagem do hormônio beta hCG e da avaliação ultrassonográfica, mas em alguns casos pode ser necessário o exame histopatológico, considerado padrão ouro. É fundamental no segmento pós gestação molar, uma anticoncepção eficaz e avaliação do risco de transformação maligna, se fazendo de extrema importância no segmento das pacientes com mola hidatiforme, identificar aquelas que apresentam persistência da doença.
Objetivo: Comparar e revisar as diversas características do Dispositivo Intrauterino (DIU) de cobre com o DIU hormonal, apontando seus mecanismos de ação, eficácias, contraindicações, vantagens e desvantagens, além de sua relação com o HIV. Revisão Bibliográfica: O DIU é um método anticoncepcional altamente eficaz, com taxas de falha em torno de 1%. Não é abortivo e não interrompe a gravidez. O principal mecanismo de ação do DIU de cobre é prejudicar a fecundação do oócito pelo espermatozóide. O DIU hormonal, por sua vez, constituído de levonorgestrel, tem como principal mecanismo de ação o espessamento do muco cervical (impedindo a passagem do esperma pelo colo do útero) e a diminuição da motilidade dos espermatozóides. A taxa de eficácia do DIU hormonal é muito alta, podendo ser representada através do índice de Pearl 0,1. Já o DIU de cobre, que é provavelmente o mais utilizado em todo mundo, tem durabilidade de dez anos e o índice de Pearl muito baixo, ocorrendo uma gravidez ou menos em cada 100 mulheres durante o primeiro ano de uso. Considerações finais: Os dispositivos intrauterinos hormonais e de cobre apresentam características específicas em relação à sua indicação, eficácia, repercussões sistêmicas e principalmente os efeitos colaterais.
