A sífilis é causada pelo Treponema pallidum e transmitida por via sexual, hematogênica ou vertical (TV) durante qualquer período da gravidez. Por orientação do Ministério da Saúde, seu rastreio seu e tratamento são oferecidos de rotina a todas as gestantes que realizam o pré-natal. No entanto, as taxas de morbidade materna, infecção congênita e mortalidade perinatal permanecem altas, representando ainda desafio à saúde pública. A persistência da alta prevalência de sífilis congênita, apesar dos programas específicos de prevenção, é inquietante. Esta revisão visa contribuir para a divulgação do conhecimento atual no país e a elaboração de medidas que possam reduzir a transmissão vertical e a morbimortalidade materno-infantil da sífilis. A prevalência de sífilis em gestantes no Brasil é de 1,6%. A região Sudeste possui a maior taxa de notificação de sífilis congênita, e o Estado do Rio de Janeiro tem as maiores taxas de incidência por nascidos vivos (9,8/1.000), segundo dados do Ministério da Saúde. O custo da prevenção da infecção congênita, incluindo teste e tratamento, é menor que US$ 1,50 por pessoa, e o risco de transmissão vertical na gravidez varia de 70 a 100%, nos casos de sífilis recente, e 30 a 40%, na sífilis tardia. Nesta revisão, apresentamos uma discussão sobre o diagnóstico e o tratamento precoce da sífilis na gravidez, que, quando realizado adequadamente, pode prevenir resultados gestacionais desfavoráveis para o binômio mãe-filho. Um melhor entendimento da doença permite buscar formas de melhorar as políticas públicas, colaborando com a diminuição da prevalência da sífilis e suas complicações no Brasil.
BackgroundHTLV-1/2 infection can cause severe and disabling diseases in children and adults. The aim of the study was to estimate the prevalence of HTLV-1/2 infection in pregnant women living in the metropolitan area of Rio de Janeiro.Methodology/Principal Findings1,204 pregnant women were tested upon hospital admission for delivery in two public hospitals in the cities of Rio de Janeiro and Mesquita, between November, 2012 and April, 2013. The samples were screened by chemiluminescent microparticle immunoassay (CMIA) and reactive ones were confirmed by Western blot (WB). Epi-info software was used for building the database and performing the statistical analysis. Eight patients had confirmed HTLV-1/2 infection (7 HTLV-1, one HTLV-2), equivalent to a prevalence rate of 0.66%. Two further reactive screening tests had negative Western blot results and therefore were considered negative in the statistical analysis. All HTLV-1/2-positive patients were born in Rio de Janeiro, most were non-Caucasian (87.5%), in a stable relationship (62.5%), had at least ten years of formal education (62.5%) and a monthly family income of up to US$600.00 (87.5%). There was only one case of coinfection with syphilis and none with HIV. The mean age of the infected women was 28.4 (SD = 6.3) years and of the seronegative ones was 24.8 (SD = 6.5) (p = 0.10). The median number of pregnancies were 3.0 and 1.0 (p = 0.06) and the median number of sexual partners were 3.5 and 3.0 (p = 0.33) in the seropositive and negative groups, respectively. There were no statistically significant differences between the groups.Conclusions/SignificanceA significant prevalence of HTLV-1/2 was found in our population. The socio-epidemiological profile of carrier mothers was similar to the controls. Such findings expose the need for a public health policy of routine HTLV-1/2 screening in antenatal care, since counselling and preventive measures are the only strategies currently available to interrupt the chain of transmission and the future development of HTLV-1/2-related diseases.
Resumo A prevalência do câncer de mama gestacional (CMG) é 1:3.000-10.000 gestações. O objetivo é conhecer os fatores de risco (FR) associados ao CMG. Trata-se de estudo caso-controle entre janeiro de 2004 e dezembro de 2014, em maternidade de referência para gravidez de alto risco no Rio de Janeiro. Para cada um dos casos foram selecionados dois controles, totalizando 21 casos de CMG e 42 controles. Os dados foram coletados a partir de revisão de prontuários e sumários de internação e parto. Características reprodutivas, obstétricas, sociodemográficas e relativas à saúde foram investigadas. Resultados: A idade média das gestantes dos dois grupos foi 35,5 anos. A média de idade da menarca também se mostrou equivalente (12,3 anos). A idade materna na primeira gravidez foi > 30 anos em 28,6% da pacientes com CMG e em 2,4% do grupo controle (p = 0,03). Utilizando regressão logística condicional pareada por idade da mãe, calcularam-se as razões de chance brutas e ajustadas e os respectivos IC95%. Os resultados apontaram que a chance de CMG aumenta 27% para cada ano a mais na idade materna na primeira gravidez (p < 0,02) e que mães com baixa escolaridade tiveram maior chance de apresentar câncer de mama (OR = 8,49). Conclusão: Nossos dados confirmam a associação entre primiparidade a partir de 30 anos e baixa escolaridade como CMG.
HTLV-1/2 are transmitted sexually, by whole cell blood products and from mother-to-child (MTC), mainly through breastfeeding. HTLV-1/2 prevalence in pregnant women is high in Rio de Janeiro, however there were no local studies addressing the rate of adverse pregnancy outcomes (APO) and MTC transmission. The aim was to study sociodemographic characteristics which may be associated to HTLV-1/2 infection and describe pregnancy outcomes and MTC transmission in HTLV-1/2-positive women. The cross-sectional study screened 1,628 pregnant women in of Rio de Janeiro (2012–2014) and found 12 asymptomatic carrier mothers (prevalence = 0.74%). Pregnancy outcome information was retrieved from medical records. Sociodemographic characteristics were similar between the positive and negative groups except for maternal age, which was higher in carrier mothers. The incidence of adverse pregnancy outcomes was similar in infected and non-infected patients (p = 0.33), however there was a high rate of premature rupture of membranes (PROM) amid infected mothers (3/12). Multilevel logistic regression found that for each additional year of age, the chance of being HTLV-1/2-positive increased 11% and that having another sexually transmitted infection (STI) increased 9 times the chance of being infected. Carrier mothers had more antenatal visits (OR = 5.26). Among the children of HTLV-1/2-positive mothers there was one fetal death, one infant death and one loss of follow-up. After two years of follow-up there was one case of MTC transmission (1/9). The mother reported breastfeeding for one month only. Knowledge about factors associated to HTLV-1/2 infection, its impact on pregnancy outcomes and the MTC transmission rate is important to guide public health policies on antenatal screening and management.
HTLV-1/2 são retrovírus com três vias de transmissão: sexual, hematogênica e vertical (TV). Em áreas endêmicas, a TV tem o papel principal na cadeia de transmissão, principalmente pelo aleitamento materno. A infecção é perene e 90% dos pacientes são portadores assintomáticos, porém existe associação com uma gama de doenças inflamatórias, de dermatites inespecíficas até patologias graves e de prognóstico reservado, como a leucemia/ linfoma de células T do adulto (LTA) e a mielopatia associada ao HTLV-1, ou paraparesia espástica tropical (MAH). Até hoje não existem formas de prevenção ou tratamento eficaz para a infecção por HTLV-1/2 nem para as doenças decorrentes da mesma. Os dados sobre a infecção por HTLV-1/2 em gestantes brasileiras ainda são escassos e isolados, pois a maioria dos artigos estuda doadores de sangue, a infecção não tem notificação compulsória e o rastreio não faz parte da rotina pré-natal preconizada pelo Ministério da Saúde. O objetivo deste trabalho é reunir o conhecimento disponível sobre o tema na literatura internacional. Ainda não há estudos publicados sobre a prevalência da infecção em nosso estado, mas o projeto "Estudo da prevalência e da transmissão vertical de HIV, HTLV e Sífilis em gestantes da Região Metropolitana do Rio de Janeiro", de professores da UERJ com financiamento da FAPERJ, identificou prevalência de 0,66% em parturientes na região metropolitana do Rio de Janeiro. Com base neste resultado, foi proposta a inclusão do exame para o HTLV na rotina pré-natal do HUPE/ UERJ.O melhor conhecimento sobre a prevalência de HTLV-1/2 em gestantes nas diferentes regiões brasileiras é fundamental para orientar a elaboração de políticas educacionais, sociais e de saúde pública que possam conscientizar a população sobre a doença e reduzir as taxas de transmissão vertical.
