El síndrome compartimental agudo (SCA) de la extremidad es una entidad que se basa en el aumento de la presión intracompartimental con disminución de la presión de perfusión, comprometiendo así la extremidad, lo cual provoca isquemia, que podría concluir en necrosis tisular e incluso pérdida de la extremidad. Ante la sospecha clínica debe evaluarse al paciente en caso de ser un paciente que colabore para recopilar información por medio de una anamnesis, y examen clínico en caso de ese ser imposible. Se debe realizar la sospecha ante los signos clínicos de dolor desproporcionado en la extremidad, compartimentos tensos, ausencia de pulso o palidez, los cuales se ven cuando el tiempo de compromiso e instauración del síndrome se prolonga. El pilar de tratamiento se basa en la descompresión de la presión intracompartimental por medio de una fasciotomía descompresiva. Entre las complicaciones del SCA se pueden presentar lesión nerviosa, lesión de tejidos blandos, y rabdomiolisis, que podría conllevar a liberación de marcadores que afecten a tal punto de generar un compromiso multisistémico. Un diagnóstico tardío se podría traducir en pérdida de la extremidad, por lo que se considera una emergencia quirúrgica.
Las lesiones de Morel- Lavallée son lesiones provocadas por trauma de alta energia que dan como resultado un desprendimiento interno de los tejidos y licuefacción de los mismos. En ciertas ocasiones se diagnostican de manera errónea y tardía lo que podrian generar un compromiso importante de la extremidad y/o del paciente. Su abordaje, reconocimiento y tratamiento continúa siendo un desafío, puede ser tratado con manejo conservador, mínimamente invasivo o quirúrgico, por la complejidad de dichas lesiones es importante el trabajo multidisciplinario lo que significa un reto para el equipo de ortoplástica e infectología en casos en los que estas se presenten.
En los últimos meses el mundo ha enfrentado cambios drásticos debido a la pandemia por el SARS-CoV2. La complejidad del virus ha llevado a que, para el personal de salud, sea difícil identificar la gran variedad de síntomas que puede conllevar el COVID-19 y de esta forma, muchos diagnósticos pueden atrasarse y no realizarse en el periodo óptimo. Gracias a la constante investigación científica se ha descrito una sintomatología muy variable, que va desde lo más frecuente, que incluye síntomas como la fiebre, dolor de garganta, tos seca y dificultad respiratoria, hasta síntomas no tan comunes como la diarrea, malestar estomacal, anosmia, hiposmia, entre otros. Un aspecto que ha llamado la atención del personal de salud, es como la presencia de la anosmia y disgeusia ha sufrido un aumento en los últimos meses y como se ha establecido una estrecha relación con el SARS-CoV2 como un síntoma precoz, que aparece incluso antes que la fiebre. Esto puede guiar al personal de salud a realizar un diagnóstico en el momento preciso para poder rastrear cadenas de contagio y así detenerlas. Claramente, también tiene una gran importancia en como la atención médica podría influir de manera positiva en el estado del paciente.
