Introducción: La pandemia por coronavirus 2019 (COVID-19) es una emergencia de salud pública con impactos significativos en la salud mental de la población general. El aislamiento social y el confinamiento en el hogar pueden estar relacionados con el desarrollo de trastornos del sueño. Materiales y métodos: El presente estudio descriptivo prospectivo fue realizado durante los meses de febrero y marzo del 2021. Se aplicó una encuesta dirigida a identificar trastornos del sueño en 155 niños y adolescentes. Resultados: La prevalencia de trastornos del sueño en niños y adolescentes fue del 35%. Por medio de la escala de trastornos del sueño de Bruni se determinó tres principales trastornos del sueño: trastorno de inicio y mantenimiento del sueño (37%), hiperhidrosis del sueño (20%), somnolencia diurna excesiva (17%). Conclusiones: Si bien estos resultados no pueden ser extrapolados a la población general, pensamos que la importancia del mismo radica en concientizar a la población sobre los trastornos del sueño en los niños y adolescentes.
The case of a fourteen-year-old patient with Systemic Lupus Erythematosus with severe activity index and immunosuppressive treatment is described. He went to the Pediatric Emergency Department of the Carlos Andrade Marín Specialty Hospital, for presenting fever, headache, nausea, auditory hallucinations and paresis in the lower extremities. Cerebrospinal fluid was studied by Polymerase Chain Reaction, in which the presence of Human Herpes Virus type 6 or also called Roseolovirus was detected. Ganciclovir was started with a favorable clinical response in 72 hours. Conclusions: The presence of HHV-6 encephalitis should be considered in immunocompromised patients with encephalopathy and use of ganciclovir as directed therapy.
El síndrome de Alström (ALMS) es un raro desorden autosómico recesivo, caracterizado por una progresiva afectación multiorgánica que puede ocasionar una muerte precoz. Se trata de un preescolar masculino de tres años y siete meses de edad que presenta fotofobia, nistagmo, cardiomiopatía ventricular dilatada, retraso en el desarrollo psicomotor, trastorno del sueño, obesidad y epilepsia. Entre los diagnósticos diferenciales del síndrome genético se planteó al ALMS. Se realizó la secuenciación del exoma completo, en el gen ALMS1 se identificaron dos nuevas variantes c.5024deIT (p.Phe1675SerfsTer16) en el exón 8 del gen ALMS1 y c.11717_1172 (0delGTTG p.Val3906GlysfsTer) en el exón 18 del gen ALMS1. El ALMS, es una enfermedad extremadamente rara de expresión clínica heterogénea el estudio molecular es determinante para su diagnóstico.
