Objective Medication non-adherence is a major public health problem that has been called an “invisible epidemic”. Globally, non-adherence rates among patients with severe mental illness ranged between 30 and 65%. It greatly increases the risk of illness exacerbation and hospitalizations. However, there is a paucity of studies examining treatment non-adherence and its associated factors among patients with severe mental disorder. Therefore, this study was aimed at determining the magnitude of medication non-adherence and associated factors among patients with severe mental disorder. Results A total of 409 study subjects were participated making a response rate of 92%. The overall prevalence of medication non-adherence was found to be 55.2% (95%, CI; 49.9%, 60.2%). Study participants whose age group of (25–34) [AOR = 3.04 (95% CI 1.27, 7.29)], study subjects taking their medication twice per day [AOR = 4.60 (95% CI 2.25, 9.43)], no social support [AOR = 4.4 (95% CI 1.78, 11.08)] and no insight for their treatment [AOR = 5.88 (CI 2.08, 16.59)] were significantly associated with medication non-adherence. The result of this study showed that non-adherence among patients with severe mental disorder was found to be high. Psychiatry health care providers have to consider the frequency of medication become once per day, continual awareness creation among professionals and engaging significant others for good social support system and continual treatment alliance is strongly commended for adherence. Background People with severe mental disorders are more likely to engage in high-risk sexual behaviors. As a result of these high-risk behaviors, they might contract sexually transmitted infections and become pregnant unintentionally. Despite the high burden of this problem, very little is known about the association between mental disorders and high-risk sexual behaviors; for this reason, the current study aimed at determining the association between these two behaviors in patients with mental disorders attending an outpatient clinic at the University of Gondar Comprehensive Specialized Hospital, Psychiatric Clinic. Methods A total of 223 study participants were recruited via a stratified sampling followed by a systematic sampling technique. An institutional-based cross-sectional study was conducted from April to May 2018. Data were collected using a pretested interviewer-based questionnaire. A four-item questionnaire was adopted from a behavioral surveillance survey and different literature sources. A multivariable logistic regression model was fitted to assess the strength in addition to the direction of the association between risky sexual behavior and independent variables. An interpretation was made based on the adjusted odds ratio and p-value at a 95% confidence interval (CI). Result Nearly half of the study participants (49.8, 95% CI; 43.9–56.5%) presented risky sexual behavior. The study found that male gender (2.98; adjusted odds ration [AOR] = 2.98; 95% CI; 1.49–5.95), no ability to read and write (3.99; AOR = 3.99; 95% CI: 1.53–10.4), history of hospitalization (3.95; AOR = 3.95; 95% CI: 1.87–8.32), perceived internal stigma (2.45, AOR = 2.45; 95% CI: 1.18–5.11), and poor social support (3.07, AOR = 3.07; 95% CI: 1.29–7.30) were significant predictors of risky sexual behaviors among patients with severe mental disorder. Conclusion Risky sexual behavior among patients with a severe mental disorders was high (49.8%). Special attention should be given to male patients, incorporating people with severe mental disorders into the adult education programs, continuous health education regarding risky sexual behavior and utilization of condoms, building self-esteem, and engaging others to provide good social support systems are strongly recommended to alleviate this type of behavior in this population.
Background A child suffering from attention deficit hyperactivity disorder (ADHD) faces many difficulties in social as well as academic performances. School teachers’ knowledge and attitude towards ADHD play a vital role in early detection and referral of the child to treatment centers. Few existing reports, however, indicate the alarming rate at which the problem is highly neglected in sub-Saharan Africa. The present study is designed to determine the knowledge, attitude, and factors that affecting elementary school teachers about ADHD. Methods An institutional-based cross-sectional study design was conducted in Gondar town and other towns nearby Gondar from February 24 to March 24, 2020. Data were collected through structured self -administered questionnaires using the Knowledge of Attention Deficit Disorders Scale and ADHD-specific attitudes measurement tools. Then, it was entered into Epi-info version 7 and exported to SPSS version 20 for analysis. Bivariable and multivariate logistic regressions were fitted to identify factors associated with the knowledge and attitude of elementary school teachers. Variables having a p-value < 0.05 at 95% CI were considered statistically significant. Result Of 636 respondents, about 44.8% (95% CI 41.2, 48.4) and 84.1% (95% CI 81.0, 86.8) of elementary school teachers had good knowledge and a favorable attitude towards ADHD, respectively. Having a diploma and above (AOR = 3.028, 95% CI 1.630–5.625), reading ADHD leaflets (AOR = 2.035, 95% CI 1.391, 2.950) and search ADHD on the internet (AOR = 1.793, 95% CI 1.090, 2.950) were significantly associated with teachers knowledge to ADHD; whereas, working experience in teaching a child with ADHD (AOR = 1.852, 95% CI 1.195–2.87) and watching ADHD on mass media (AOR = 1.72, 95% CI 1.056–2.8) were positively predicts teachers attitude towards ADHD. Conclusion the proportion of teachers’ knowledge towards ADHD was low; in contrast, their attitude was relatively satisfactory. Strengthening teachers’ educational upgrading system, frequent and fair distribution of leaflets written to address ADHD, installation of an internet system to the schools, and continuous ADHD awareness creation programs through mass media are highly recommended.
Introduction Depression and anxiety are the most prevalent mental disorders in the general population and are expected to be the number one global burden of disease by the year 2030. They are also common comorbid conditions for patients with tuberculosis. Objective This study aimed to assess the prevalence of symptoms of depression, and anxiety and their associated factors among patients with tuberculosis attending Gondar city health facilities. Methods An institution-based cross-sectional study was conducted from September 01 to 30/2020. A census sampling technique was employed to select 390 patients. A structured interviewer-administered questionnaire was used to collect data, and a standardized hospital anxiety and depression scale was used to measure the symptoms of anxiety and depression. Data were entered in Epi-Info version 7 and analyzed using SPSS version 23. Binary and multivariable logistic regressions were computed to identify factors associated with the symptoms of depression and anxiety. P-value < 0.05 and adjusted odds ratios were used to declare the significance and strength of the association. Results The overall prevalence of symptoms of depression and anxiety were found to be 35.8% with 95% CI (34.6, 36.6). Perceived stigma and duration of illness > 12 months were associated positively ([AOR = 3.60; 95% CI (2.74, 4.43)], and [AOR = 3.19; 95% CI (2.17, 4.19)]) for both depression and anxiety respectively. Separate analyses revealed that the prevalence of symptoms of depression was 55.9% (95% CI (51.0%, 60.3%) and was significantly associated with duration of illness 4–6 months and > 12 months (AOR = 1.21; 95% CI (1.17, 2.73)] and [AOR = 2.36; 95% CI (2.16, 3.79)], comorbid chronic disease (AOR = 0.12; 95% CI (0.08, 0.91)] and perceived stigma [AOR = 0. The prevalence of anxiety symptoms was 39.5 percent, with 95% confidence intervals of 34.6% and 44.6%, and it was significantly associated with comorbid chronic disease [AOR = 2.53; 95% CI (1.96, 6.32)] and perceived stigma [AOR = 3.31; 95% CI (1.22, 7.74)]. Conclusion The prevalence of symptoms of depression and anxiety was high. Duration of illness, comorbid chronic disease, and perceived stigma were significantly associated with symptoms of depression. Comorbid chronic disease and perceived stigma were significantly associated with symptoms of anxiety.
Introduction Post Traumatic Stress Disorder (PTSD) was more common in children who had suffered physical trauma than in adults. Despite its prevalence, the prevalence and factors associated with PTSD in pediatric patients with physical trauma are unknown in Ethiopia. As a result, the purpose of this study was to determine the prevalence of PTSD and associated factors among pediatric patients with physical trauma who attended Northwest Amhara referral hospitals. Methods An institutional-based cross-sectional study design was used in 422 pediatric patients with physical trauma aged 8–18 years from March 15 to May 15/2021. Using a systematic random sampling technique, data were collected from a sample of selected trauma patients via interviews and chart review. A standardized, pre-tested Child PTSD Symptom Scale was used to assess the severity of PTSD. Epidata 4.6 was used to enter the data, and Stata 14.0 was used to analyze it. Bivariable and multivariable binary logistic regression models were used to identify PTSD determinants. Result The study included 422 paediatric patients who had suffered physical trauma, with a response rate of 97.87 percent. PTSD was found in 22.03 percent of paediatric patients with physical trauma in Northwest Amhara referral hospitals. The study discovered that female gender (AOR = 3.04, 95 percent CI: 1.58–5.84), age of 8 to 10 years old (AOR = 3.70, 95 percent CI: 1.39–9.87), having a chronic medical illness (AOR = 5.99, 95 percent CI: 2.60–13.77), having severe pain (AOR = 3.17, 95 percent CI: 1.12–8.99), low social support (AOR = 8.97, 95 percent CI: 4.04–19 were associated with PTSD. Conclusion and recommendation The prevalence of PTSD was found to be high among pediatric patients who had experienced physical trauma. Special attention should be given to female patients, aged 8 to 10 years old, who have a chronic illness, for those who complain of severe pain and engaging others to provide good social support systems, are strongly recommended to alleviate PTSD in this segment of population.
