OBJETIVO: Apresentar os resultados do Hospital Israel Pinheiro - IPSEMG em 41 duodenopancreatectomias realizadas para neoplasias. MÉTODO: Foram coletados dados referentes a 41 pacientes submetidos a duodenopancreatectomias entre 1997 e 2004. A principal operação realizada foi a Whipple "clássica" sem preservação do piloro. A anastomose pancreático-jejunal foi realizada por meio de sutura ducto-mucosa. Foram analisadas a mortalidade e as complicações pós-operatórias. Para avaliar se havia diferença de resultados com a maior experiência da equipe, o estudo foi dividido em dois períodos de quatro anos cada: 1997 a 2000 e 2001 a 2004. RESULTADOS: As complicações pós-operatórias ocorreram em 58% dos casos e a mortalidade foi de 22%. As principais complicações foram pneumonia e infecção de ferida operatória. Quatro pacientes (10%) evoluíram com fistulas pancreáticas, porém obteve-se sucesso com o tratamento conservador em todos os casos. A necessidade de hemotransfusão no peroperatório relacionou-se a um pior prognóstico. Observou-se uma redução das taxas de morbi-mortalidade no período de 2001 a 2004, entretanto sem significância estatística. Os pacientes apresentaram sobrevida global em cinco anos de 35% e de 26% quando considerados apenas aqueles com adenocarcinoma de cabeça de pâncreas. CONCLUSÕES: A duodenopancreatectomia é um procedimento cirúrgico complexo, com elevada morbi-mortalidade. Entretanto, com o aumento da experiência das equipes cirúrgica, anestésica e de medicina intensiva, observa-se uma redução nas taxas de complicação.

É sabido que a esplenectomia causa propensão a infecções, principalmente aquelas causadas por microorganismos capsulados. Essas complicações acompanham-se de maior mortalidade e conseqüente menor sobrevida dos indivíduos asplênicos. O objetivo do presente trabalho foi avaliar a sobrevida em relação ao sexo de ratos submetidos a esplenectomia total. Foram estudados 34 ratos, distribuídos em dois grupos, sendo o Grupo 1 (n=14) controle, animais submetidos apenas a laparotomia e o Grupo 2 (n=20) submetidos a esplenectomia total. Os animais de cada grupo foram distribuídos em dois subgrupos iguais, sendo o Subgrupo A composto por machos e o Subgrupo B por fêmeas. Os ratos foram acompanhados por um período máximo de noventa dias com vista à sua sobrevivência. Observou-se que a mortalidade após esplenectomia total foi de 80% no Subgrupo dos animais machos e de 30% no das fêmeas. Não houve óbitos no Grupo 1. Os ratos esplenectomizados apresentaram sobrevida inferior à das ratas esplenectomizadas (p = 0,034). De acordo com os resultados obtidos, a esplenectomia diminui a sobrevida de ratos, e as fêmeas murinas apresentam maior resistência à asplenia e conseqüentemente menor mortalidade do que os machos. It is believed that sigmoid volvulus (SV) in Brazil is a frequent complication of megacolon caused by Chagas' disease (CD), differing in some characteristics from volvulus found in other countries. Bowel obstruction in patients with CD, principally when the cause is SV, may be sometimes difficult to diagnosis exclusively with plain abdominal radiograph. Fecaloma impacted in retossigmoidal area is one of the differential diagnoses. In addition, the huge amount of gas and feces, and distension of the colon normally increase the difficulty to make the correct diagnostic. The use of computer tomography (CT) scan can easy elucidate the picture of SV, and can be a great tool in cases of patients with CD and suspicion of this entity. A 62-year-old man showed bowel distention and stop disposal of gas for 5 days. He had previous diagnosis of CD. He also had been suffering from chronic constipation for several years, including impacted fecaloma, with the necessity of manual extraction. Plain abdominal radiographs showed an important colon dilatation and gross amount of feces in the sigmoid colon. Abdominal computer tomography sacan revealed dilated colon filled with feces, as well, the "whirl sign" composed of mesentery and twisted colon. When abdominal radiograph films reveal gross colonic dilatation of unknown etiology in patients with CD, a whirl sign on CT scans raises the possibility of colonic volvulus.
