OBJETIVO: Apresentar os resultados do Hospital Israel Pinheiro - IPSEMG em 41 duodenopancreatectomias realizadas para neoplasias. MÉTODO: Foram coletados dados referentes a 41 pacientes submetidos a duodenopancreatectomias entre 1997 e 2004. A principal operação realizada foi a Whipple "clássica" sem preservação do piloro. A anastomose pancreático-jejunal foi realizada por meio de sutura ducto-mucosa. Foram analisadas a mortalidade e as complicações pós-operatórias. Para avaliar se havia diferença de resultados com a maior experiência da equipe, o estudo foi dividido em dois períodos de quatro anos cada: 1997 a 2000 e 2001 a 2004. RESULTADOS: As complicações pós-operatórias ocorreram em 58% dos casos e a mortalidade foi de 22%. As principais complicações foram pneumonia e infecção de ferida operatória. Quatro pacientes (10%) evoluíram com fistulas pancreáticas, porém obteve-se sucesso com o tratamento conservador em todos os casos. A necessidade de hemotransfusão no peroperatório relacionou-se a um pior prognóstico. Observou-se uma redução das taxas de morbi-mortalidade no período de 2001 a 2004, entretanto sem significância estatística. Os pacientes apresentaram sobrevida global em cinco anos de 35% e de 26% quando considerados apenas aqueles com adenocarcinoma de cabeça de pâncreas. CONCLUSÕES: A duodenopancreatectomia é um procedimento cirúrgico complexo, com elevada morbi-mortalidade. Entretanto, com o aumento da experiência das equipes cirúrgica, anestésica e de medicina intensiva, observa-se uma redução nas taxas de complicação.
RESUMO: Objetivo:Os autores apresentam um estudo observacional, transversal, comparando a anastomose manual com a anastomose mecânica para reconstrução do trânsito digestivo em Y de Roux em pacientes submetidos à gastrectomia total para câncer gástrico. Métodos: O estudo foi realizado no Hospital Governador Israel Pinheiro, no período de janeiro de 1997 a março de 2002. Um total de 40 pacientes neste período foram submetidos à gastrectomia total com reconstrução em Y de Roux. O Grupo I formado por 16 pacientes foi submetido à anastomose mecânica e o grupo II por 24 pacientes foi submetido à anastomose manual. Os dois grupos foram comparados quanto à incidência de complicações pós-operatórias, necessidade de re-operação, alta hospitalar e mortalidade per e pós-operatória. Resultados: Quanto às complicações relacionadas à anastomose, fístula e abscesso intraabdominal, elas ocorreram em nove pacientes do grupo com anastomose manual e em nenhum paciente no grupo com anastomose mecânica. (p= 0,006). A morbidade e a permanência hospitalar pós-operatória foram maiores no grupo submetido à sutura manual. Houve um óbito em cada grupo e 37% dos pacientes em cada grupo não apresentaram quaisquer tipos de complicações. Conclusão: Considerando a fístula e o abscesso intra-abdominal juntamente como complicações da anastomose esôfago-jejunal, os resultados deste estudo sugerem uma maior viabilidade da sutura mecânica (Rev. Cir. 2008; 35(5): 298-303 INTRODUÇÃOA esofagojejunostomia em Y de Roux para a reconstrução do trânsito após gastrectomia total está associada a complicações como abscesso intra-abdominal, fístula e estenose. Considerando a dificuldade de exposição do mediastino posterior através do hiato esofágico diafragmático tornando o campo operatório estreito e profundo e o fato do coto esofágico ser de pequeno diâmetro e desprovido de serosa a confecção da anastomose torna-se difícil.A fístula anastomótica é a complicação mais grave ocorrendo em 0 a 41% dos casos e é responsável por até 90% dos óbitos pós-operatórios que ocorrem em cinco a 30% dos pacientes submetidos à gastrectomia total com esofagojejunostomia 1 . A técnica cirúrgica é fator fundamental relacionado à sua ocorrência 2,3 . Desde o surgimento dos grampeadores mecânicos de sutura dupla em 1977, argumenta-se que as suturas mecâ-nicas são mais seguras, menos traumáticas e associadas a menor morbimortalidade 4,5 . No entanto, alguns autores alegam alto custo e altas taxas de estenose pós-operatória 6 .Estudos retrospectivos e prospectivos comparativos entre a anastomose manual e mecânica concluíram que a última é tecnicamente menos trabalhosa e, segundo alguns autores, acompanha-se de menor incidência de complicações pós-operatórias 7 . Outros estudos não demonstraram diferença estatisticamente significativa entre as complicações após sutura mecânica e manual 2,3,8 . Os inibidores da colinesterase são medicamentos utilizados frente a doença de alzheimer, proporcionando altas concentrações do neurotransmissor acetilcolina, que irá atuar através do estímulo comunicação entre as células e auxiliando no processo da memória, mas que requer avaliação dos riscos para a sua utilização, principalmente os riscos cardiovasculares. Objetivou-se evidenciar os efeitos cardiovasculares decorrentes da utilização dos inibidores da colinesterase como tratamento para pacientes idosos com demência. Trata-se de uma pesquisa bibliográfica, do tipo revisão integrativa da literatura, realizada através de pesquisa nas bases de dados: Scientific Electronic Library Online, Medical Literature Analysis and Retrieval System Online e Literatura Latino-Americana e do Caribe em Ciências da Saúde, através dos Descritores em Ciências da Saúde (DeCS): “Inibidores da Colinesterase”; “ Demência”; “Idoso”; “Sistema Cardiovascular”, associados ao operador booleano AND. Chegou-se a totalidade de 6 documentos para compor a revisão. Dentre esses riscos cardiovasculares, cita-se: sinais e sintomas de bradicardia, efeitos de decaimento da pressão arterial, hipotensão postural e síncope, além do desenvolvimento de outras patologias, como: Acidente Vascular Cerebral, Hipertensão Arterial Sistêmica e Arritmias. Ou seja, são diversas as complicações relacionadas ao Sistema Cardiovascular, carecendo avaliação constante do paciente e do seu quadro de saúde, adaptando à terapêutica com base do seu histórico prévio de patologias, queixas e na evolução do estado de saúde.
Cholinesterase inhibitors are drugs used against hallucinations, providing high concentrations of the neurotransmitter acetylcholine, which will act by stimulating communication between cells and helping in the memory process, but which requires risk assessment for their use, especially cardiovascular risks . The objective was to highlight the cardiovascular effects resulting from the use of cholinesterase inhibitors as a treatment for elderly patients with dementia. This is a bibliographical research, of the integrative literature review type, carried out through research in the databases: Scientific Electronic Library Online, Medical Literature Analysis and Retrieval System Online and Latin American and Caribbean Literature in Health Sciences, through of Health Sciences Descriptors (DeCS): “Cholinesterase inhibitors”; “Dementia”; "Elderly"; “Cardiovascular System”, associated with the Boolean AND operator. A total of 6 documents were reached to compose the review. Among these cardiovascular risks, we mention: signs and symptoms of bradycardia, effects of blood pressure decay, postural hypotension and syncope, in addition to the development of other pathologies, such as: Stroke, Systemic Arterial Hypertension and Arrhythmias. That is, there are several complications related to the Cardiovascular System, requiring constant evaluation of the patient and his health condition, adapting the therapy based on his previous history of pathologies, complaints and the evolution of his health status.
