Nota HistóricaAté parte do século XVI, não foram feitos esforços visando ajudar os indivíduos com surdez profunda no sentido do desenvolvimento da linguagem oral, pois admitia-se, sob a influência da autoridade magna de Hipócrates e Aristóteles, que eles apresentavam lesão orgânica irreparável. Na segunda metade do século XVI, o monge beneditino Pedro de Ponce tornou-se famoso por ensinar surdos-mudos de famílias espanholas nobres a falar 5 . A partir deste começo, seguiram-se avanços significativos na área educacional de crianças surdas. E, no século XIX, a surdez infantil passou a ser abordada como matéria de grande interesse no campo das ciências médicas.Em meados do século XIX, William Wilde (o pai de Oscar Wilde), eminente otorrinolaringologista irlandês, realizou ampla investigação de surdez em seu país e encontrou famílias com os padrões de herança que seriam reconhecidos, com o advento do mendelismo, como autossômico dominante, autossômico recessivo e ligado ao X 6,7 . Se profunda, a hipoacusia afeta os mais diversos aspectos da vida de seus portadores, incluindo preferências educacionais, estraté-gias de comunicação, relações familiares e sociais, e oportunidades profissionais 2 .Na surdez condutiva, o defeito localiza-se no ouvido médio ou externo, enquanto que a surdez neurossensorial (perceptiva ou neural) caracteriza-se pela ocorrência de anormalidades situadas entre os receptores do ouvido interno e as regiões auditivas no cérebro 3 . ResumoObjetivo: Investigar as causas de surdez pré-verbal em uma população institucionalizada, com enfoque sobre a etiologia gené-tica, considerando-se a escassez de dados nacionais nessa área.Métodos: Com base nos prontuários de 658 alunos, matriculados em 18 instituições para deficientes auditivos de Recife, foram obtidas informações relativas a testes audiológicos, exames laboratoriais, etiologia, anomalias associadas, consangüinidade e presença de outros surdos na família; exame dismorfológico foi realizado em 557 alunos; todas as famílias com casos recorrentes de surdez foram investigadas, tendo sido constituídas as genealogias pertinentes.Resultados: Cerca de 13% da população estudada, provavelmente, manifestam surdez genética; os padrões de herança autossô-mico recessivo, autossômico dominante e recessivo ligado ao X foram observados, respectivamente, em 87,7%, 8,8% e 3,5% dos casos familiais; entre os alunos examinados, 4,3% apresentavam síndromes dismórficas específicas ou anomalias associadas à surdez; as causas adquiridas e a etiologia desconhecida estavam representadas por 41,5% e 45,5% respectivamente.Conclusões: As freqüências que obtivemos para os distintos fatores causais de surdez pré-verbal situam-se dentro do espectro de variação das freqüências observadas em trabalhos similares, salientando-se que, em grande parte dos casos, a etiologia é desconhecida.J. pediatr. Relatamos a história clínica de dois irmãos com a síndrome de Cockayne clássica. As principais manifestações consistiam de nanismo caquético, retardo mental, calcificações intracranianas, microcefalia, enoftalmia, aparência senil, hipomotilidade articular e fotossensibilidade cutânea. Em uma dessas crianças, que faleceu aos 10 anos de idade de broncopneumonia, os estudos de necrópsia revelaram uma variedade de anomalias, principalmente encefálicas, que incluíam um cisto aracnóideo na base do cerebelo, defeito aparentemente não descrito previamente em pacientes com esta síndrome.
