Kedelai merupakan salah satu kebutuhan pokok yang dibutuhkan oleh masyarakat umum. PT. FKS Multi Agro merupakan industri yang bergerak dibidang pengolah distribusi kacang kedelai. Untuk mencapai tingkat produktivitas dan keuntungan yang tinggi, suatu industri perlu melakukan sebuah monitoring dan evaluasi terkait capaian yang telah ditentukan sebelumnya. Dari hasil studi lapangan diketahui bahwa terdapat hambatan yakni kecacatan pada kemasan kedelai terdapat barang reject akibat karung yang jahitannya kurang kuat. Sehingga untuk mengatasi masalah tersebut dibutuhkan metode yang dapat mengidentifikasi permasalahan yang ada serta dapat memberikan usulan yang tepat guna meminimalisir permasalahan tersebut. Metode six sigma digunakan untuk mencari permasalahan yang terdapat pada alur produksi beserta mengurangi cacat. Sedangkan fishbone diagram digunakan untuk analisa penyimpangan pada produk cacat dan mencari penyebab terjadinya permasalahan produk cacat. Setelah dilakukan analisis diketahui nilai besaran sigma PT. FKS MULTIAGRO berada pada nilai 4-sigma yang setara dengan dunia di Negara Amerika. Namun demikian masih terdapat hal hal minor yang menjadi faktor penghambat PT. FKS MULTIAGRO untuk dapat mendekati nilai zero defects, hal tersebut terkait dengan CTQ dari produk kedelai kemasan itu sendiri yakni kemasan yang digunakan memiliki kecacatan pada karung yang mengakibatkan Produk yang dihasilkan dianggap reject serta rawmaterial harus kembali diolah ke tahapan awal
