Aim To compare hemorrhagic safety of ticagrelor and clopidogrel in patients with ST-segment elevation acute coronary syndrome (STEACS) after thrombolytic therapy (TLT).Material and methods This nonrandomized study included 183 patients followed up for 30 days. Hemorrhagic safety was compared in a group of patients with STEACS (n=71) after a thrombolytic treatment with alteplase and early ticagrelor treatment (180 mg followed by switching to 90 mg twice daily) and in a group of patients (n=112) with STEACS receiving TLT with alteplase and clopidogrel (loading dose, 600 mg followed by switching to 75 mg daily). Primary endpoint was hemorrhage associated with TLT; patients were followed up for 30 days.Results During the follow-up period, TLT-associated hemorrhages were observed in 11.3% of patients in the ticagrelor treatment group and in 10.7% of patients in the clopidogrel treatment group (p=0.9; odds ratio, 1.06 at 95 % confidence interval, from 0.41 to 2.73). Intracranial hemorrhages and fatal hemorrhages were absent in both groups.Conclusion There were no significant differences in hemorrhagic safety between patients with STEACS after the TLT treatment with alteplase and early treatment with ticagrelor or clopidogrel.

Ежегодно, по данным мировой статистики, на 1 млн жителей выполняются до 500 больших и малых ампутаций конечностей [9]. В России ежегодно от 50 до 70 тыс. ампутированных больных становятся инвалидами, что представляет собой не только медицинскую, но и социальную проблему [2]. Наиболее частой причиной развития критической ишемии являются распространенные окклюзионно-стенотические поражения бедренно-подколенно-берцового сегмента артериального русла нижних конечностей. При поражении артерий голени и стопы и неудовлетворительном состоянии дистального сосудистого русла большинство хирургов предпочитают отказываться от реконстуктивных операций и выполняют непрямую реваскуляризацию ишемизированной конечности [1,5]. При хорошем или удовлетворительном состоянии путей оттока выполняется бедренно-подколенное или берцовое шунтирование в зависимости от распространенности поражения артерий выше или ниже щели коленного сустава. Золотым стандартом выбора шунта при реконструктивных операциях на инфраингвинальных артериях был и остается аутовенозный трансплантат из большой подкожной вены [6,9]. Применение в качестве шунта синтетического или биологического протезов значительно ухудшает результаты лечения больных [10]. Результаты аутовенозного бедренно-подколенного шунтирования выше щели коленного сустава вполне удовлетворительны. Через 5 лет функционируют от 75 до 90% шунтов, конечность приходится ампутировать не более чем в 2-5% случаев [1,7]. Значительно хуже результаты шунтирующих аутовенозных операций ниже щели коленного сустава. Через 4-5 лет функционируют от 48 до 85% шунтов, избежать ампутации конечности удается лишь у 55-70% больных [6,7].Свободный аутовенозный трансплантат путем реверсии большой подкожной вены давно применяется для шунтирования артерий бедренно-подколенно-берцового сегмента ниже щели коленного сустава. К сожалению, данная методика имеет 2 существенных недостатка. При наложении проксимального анастомоза между бедренной артерией диаметром 8 мм с дистальным концом большой подкожной вены диаметром 4 мм развивается гемодинамически значимый стеноз. На выходе из стеноза возникает зона разделения потока крови с его замедлением, что приводит к развитию низкого напряжения силы сдвига между слоями крови и стенкой сосуда. Низкое напряжение силы сдвига стимулирует секреторную активность миоцитов и провоцирует адгезию тромбоцитов, что может вызывать в дальнейшем сужение просвета аутовенозного трансплантата и приводить к его тромбозу [8]. Второй недостаток заключается в том, что реверсированная вена при дистальном бедренно-подколенном шунтировании не обеспечивает удовлетворительный уровень
