The impact of previous stillbirth, miscarriage, or preterm delivery on anxiety in pregnant women in various subgroups at high risk for fetal abnormality in comparison with a nonrisk control group of women with healthy uncomplicated pregnancies was studied longitudinally. The level of anxiety in women (n ¼ 674) during early pregnancy was assessed by questionnaire. Data were collected at three points in time: immediately before the ultrasound scanning, at 5-6 weeks, and at 10-12 weeks after the prenatal examination for fetal malformation. In general, all women with high-risk pregnancies (n ¼ 506) showed high levels of anxiety immediately before ultrasound scanning. There was a significant decrease in anxiety over the following 10-12 weeks. By contrast, level of anxiety was not raised in the no-risk control group (n ¼ 168). However, pregnant women
ZusammenfassungGemäß Leitlinienempfehlungen ist Psychotherapie ein wesentliches Behandlungsmittel bei psychischen Erkrankungen. In den Einrichtungen der Psychiatrie lassen sich psychotherapeutische Behandlungskonzepte jedoch nur begrenzt umsetzen, da die personellen Ressourcen, wie sie auf der Basis der Psychiatrie-Personalverordnung (Psych-PV) ermittelt werden, hierfür nicht ausreichen. In den neuen Personalanforderungen, die der Gemeinsame Bundesausschuss (G-BA) bis zum 1. Januar 2020 für die psychiatrischen und psychosomatischen Einrichtungen erarbeitet, muss der Stellenwert der Psychotherapie für die Behandlung deshalb angemessen berücksichtigt werden. Circa 5 600 Psychologische Psychotherapeuten und Kinder- und Jugendlichenpsychotherapeuten (PP/KJP) sind in Krankenhäusern tätig, die Mehrzahl von ihnen in der Psychiatrie und Psychosomatik. Dort haben sich Organisationsmodelle und -strukturen etabliert, in denen PP/KJP neben den Fachärzten eine verantwortliche Rolle für die Patientenbehandlung übernehmen. Diese Rolle muss auch in den neuen Personalanforderungen des G-BA verbindlich festgeschrieben werden. Doch auch die Qualifizierung der PP/KJP steht vor einem Wandel, von dem ihre Rolle in der stationären Versorgung in Psychiatrie und Psychosomatik profitieren wird. Die dringend notwendige Reform des Psychotherapeutengesetzes wird zu Änderungen der Personalstrukturen in den psychiatrischen und psychosomatischen Krankenhäusern führen, an deren Umsetzung die Bundespsychotherapeutenkammer gerne mit allen Beteiligten gemeinsam arbeiten würde.
