Die Brust gilt als sichtbares Symbol der Weiblichkeit und trägt entscheidend zur weiblichen Identität und zum Selbstwertgefühl bei. Ziel der plastischen Mammachirurgie ist es deshalb, das weibliche Erscheinungsbild wiederherzustellen und das Körpergefühl nachhaltig zu verbessern. Im Bereich der ästhetischen plastischen Chirurgie sind die Übergänge zwischen medizinisch indizierten und rein kosmetischen Eingriffen fließend. Durch Schönheitsideale und Mode beeinflusst, wünschen sich viele Frauen eine Verbesserung des Körperbildes. Bedingt durch Schwangerschaft, Stillzeit, starke Gewichtsabnahme, Bindegewebsschwäche oder durch den natürlichen Alterungsprozess stimmt oft die persönliche Idealvorstellung einer attraktiven Brust mit der Realität nicht mehr überein. Anlagestörungen und Fehlbildungen Anlagestörungen und Brustfehlbildungen manifestieren sich im Teenageralter. Für die junge Frau bedeutet die deformierte Brust eine erhebliche ästhetische Beeinträchtigung, die eine soziale Isolierung mit Verhaltensauffälligkeiten zur Folge haben kann. Eine Korrektur sollte möglichst früh nach abgeschlossener Brustentwicklung erfolgen. Gleiches gilt für die operative Behandlung der juvenilen Makromastie oder Hyperplasie in den verschiedenen Lebensabschnitten. Diese Patientinnen sind durch chronische orthopädische Beschwerden, oft in Kombination mit Dermatosen in der Brustumschlagsfalte gesundheitlich stark beeinträchtigt. Eine Verkleinerung des Brustvolumens stellt für die Betroffenen eine lang erhoffte Lösung ihrer funktionellen, ästhetischen und psychischen Probleme dar. Die im Einzelfall beste individuell angepasste Operationsmethode wird mitbestimmt durch die anatomische Ausgangssituation und der von der Patientin gewünschten Brustgröße und -form. Die Operationstechniken reichen von der periareolären Mastopexie über vertikale Techniken bis hin zu den Reduktionsverfahren mit invertiertem T. Ausgiebige Aufklärung der Patientin unverzichtbar Vor jeder plastisch-chirurgischen rekonstruktiven Intervention ist es erforderlich, die Patientinnen umfassend über Risiken, Früh-und Spätkomplikationen aufzuklären. Die so genannte schonungslose Aufklärung bei ästhetischen Eingriffen bezieht sich bei der Implantatchirurgie zusätzlich auf die Haltbarkeit, Sicherheit, Formstabilität, Überwachung der Brust und Risiken der Kapselfibrose. Notwendig ist es ebenfalls, über Details der gewählten Operationstechnik mit eventueller axillärer, periareolärer oder einer Narbenbildung in der Inframammarfalte zu informieren. Nach heutigem Stand des Wissens sind die auf dem Markt befindlichen Silikonbrustimplantate nicht gesundheitsschädigend und haben ihren festen Platz sowohl im Rahmen der Brustrekonstruktion nach Mastektomie als auch bei der ästhetischen Augmentation. An ART sparing the uterine arteries and hypogastric plexus was performed. Surgical radicality was compared with an historical control group of C1 radical hysterectomies. The OS& DFS rates, complications, pregnancies, recurrences, and follow-up,were analyzed. Results The ART was performed in 25/27 patients (95,2%). Age: 27,5 years (25-35). Five patients received NCH. In 2 cases the procedure had to be completed with a C1RH because of positive margins in the surgical specimen. Radicality (surgical specimen): tumor size 2,36 (1,-4,1) cm; Vaginal length 2,62 (0,8-3,1) cm; Right parametrium 3,2 (1,8-3,9) cm; left parametrium 3,04 (2,3-3,5) cm; cervix length 2,8 (2,3-3,3) cm; Lymph nodes 13,5 (2-22). No statisticaly differences were observed in surgical radicality between ART vs C1RH. Complications: 1 case of dyspareunia + cervical polyposis. 1 case of dysmenorrhea. Four pregnancies were observed. Three healthy newborns and 1 abortion were registered,all in NCH cases). Recurrences: 2/27 (8,4%). Deaths: 2/ 27 (7,3%). DFS & OS: 92,5%. Follow-up: 127 months (85-180). Conclusions The ART sparing the uterine arteries and hypogastric plexus (C1 ART), showed to be feasible and oncologicaly safe. This variant of ART, could be considered as an alternative to C1RH for these initial FIGO stages.
Results Tomography and ultrasound evidenced of complex cystic mass with multiple trabeculations and calcifications involving the entire abdomen, massive cystic multissepted, with thick septa and peritoneal nodules, which presented contrast enhancement, diffusely lining the peritoneal cavity, from the subdiaphragmatic surfaces to the pelvis. Tumoral marker (CA 125) presented alteration (3,630 U/ ml). Pathological and immunohistochemistry exam showed malignant neoplasm of epithelial origin, Low-Grade Endometrial Stromal Sarcoma (ESS-LG). The patient was submitted in January 2018 to a cytoreductive surgery with preservation of the fertility, performed appendectomy, left salpingo-oophorectomy with resection of multiples peritoneal implants. Patient is currently in clinical follow-up, using megestrol acetate since March 2018, with no pregnancy schedule so far. Conclusions ESS-LG are rare malignancies, it affects women in perimenopause and young people. The standard treatment is total hysterectomy and bilateral salpingo-oophorectomy, but in young patients consider the possibility of fertility-sparing to desire for gestation. The viability and safety are still limited due to few studies. Patients who chose to maintain fertility, had low recurrence rates and most of them presented a successful pregnancy, naturally, cesarean delivery, without complications.
