По стати-стическим данным у 57 % больных, которым выпол-нена радикальная цистэктомия, имеется первичная мышечная инвазия [2, 16]. Около 30 % пациентов с мышечно-инвазивным РМП на момент выявления за-болевания имеют недиагностированные отдаленные метастазы, а 25 % больных подвергаются радикаль-ному оперативному лечению при уже имеющемся по-ражении лимфатических узлов [13, 14].Радикальная цистэктомия предполагает регио-нарную лимфодиссекцию. Тазовая лимфаденэктомия позволяет уменьшить риск местного рецидива и по-тенциально улучшить раково-специфическую выжи-ваемость. Липецка в период с 2005 по 2016 год. Больные были разделены на две группы в зависимости от способа обезболивания в послеоперационном периоде. В 1-й группе (n=96) при-менялась только системная анальгезия. Во второй группе (n=30) наряду с системной анальгезией проводили преперитонеальную блокаду. Эффективность послеоперационной анальгезии определяли с помощью ВАШ.Из ранних послеоперационных осложнений в обеих группах чаще наблюдалась динамическая кишечная непроходимость. Во второй группе больных выявлено снижение динамической кишечной непроходимости до 10 %, раневые воспалительные осложнения наблюдались в 2 раза реже, чем в первой группе.Таким образом, радикальная цистэктомия позволяет получить удовлетворительные функциональные ре-зультаты. Однако количество послеоперационных осложнений остается высоким. Для снижения и профилак-тики послеоперационных осложнений у больных после радикальной цистэктомии при РМП эффективно при-менение преперитонеальной блокады. Patients were divided into two groups depending on the method of analgesia in the postoperative period. In the first group (n=96) only systemic analgesia was used. The objective of this study was to evaluate the immediate results of the use of ureterointestinal anastomosis according to the Bricker technique at radical cystectomy (RC) for bladder cancer (BC). Materials and Results:The study included 96 patients (11.5% women and 88.5% men) with BC, aged from 31 to 74 years (mean age 63.8±7.2), who underwent RC in the Lipetsk Regional Oncology Center, in the period from 2005 to 2014.Among the early postoperative complications, we identified dynamic ileus (16.7%), inflammatory complications of the surgical wound (12.5%), acute pyelonephritis (10.4%), and failure of ureterointestinal anastomosis (4.2%). The frequency of postoperative acute pyelonephritis corresponded to the findings of other authors. Two (2.1%) patients died from early postoperative complications because of concomitant diseases (ischemic heart disease, myocardial infarction); thus, postoperative mortality in the early postoperative period was 4.2%. Chronic pyelonephritis with chronic renal failure detected in 15(15.6%) patients after one year after surgery was the most frequent late postoperative complication. The stricture of ureterointestinal anastomosis in 9(9.4%) patients has been eliminated through relaparotomy and resection of anastomosis. The development of urolithiasis in 12(12.5%) patients after one year after surgery has required the implementation of contact lithotripsy and litholytic therapy. (Int J Biomed. 2016;6(2):110-113.).
СПОСОБ ФОРМИРОВАНИЯ МОЧЕТОЧНИКОВО-КИШЕЧНОГО АНАСТОМОЗАФГБОУ Российская ФедерацияЦель. Создание способа мочеточниково-кишечного анастомоза, который позволит обеспечить гер-метичность, достаточную клапанную функцию анастомоза и отсутствие рефлюкса, снижение ранних и поздних послеоперационных осложнений.Материал и методы. Проведено исследование результатов формирования анастомоза в эксперименте на 12 кроликах породы шиншилла обоего пола на протяжении 14 суток. В 1-ой серии эксперимента на 6 кроликах мочеточниково-кишечный анастомоз выполняли по способу Bricker. Во 2-ой серии на 6 кроликах анастомоз выполняли предложенным способом. Оценку клапанной функции мочеточниково-кишечного анастомоза проводили на 7 и 14 сутки после операции и выведения кроликов из эксперимента на изоли-рованном препарате мочеточниково-кишечного анастомоза, сформированном по предложенному способу и по способу Bricker.Результаты. Установлено, что во 2-ой серии эксперимента интраоперационно после формирования мочеточниково-кишечного анастомоза кровотечения и выделения мочи в зоне анастомоза не наблюдалось. Давление, при котором возник рефлюкс в изолированном препарате мочеточниково-кишечного анастомо-за, сформированного по способу Bricker, составило 58,3±12,2 см вод.ст., а по предложенному способу -75,6±5,4 см вод.ст. При макроскопическом и микроскопическом исследовании в 1-ой серии эксперимента через 7 суток в двух случаях выявлена несостоятельность анастомоза, у других животных в данной серии на 7 и 14 сутки наблюдали в области анастомоза явления воспаления с отеком и инфильтрацией, деформации, секвестрации лигатур.Во 2-ой серии эксперимента несостоятельности анастомозов не наблюдали, через 7 суток в области анастомоза выявлены умеренная гиперемия, образование налета фибрина, деформация отсутствовала, через 14 суток признаков воспаления не было, отмечалась эпителизация зоны анастомоза. В мышечной оболочке развивалась грануляционная ткань.Заключение. Предлагаемый способ формирования анастомоза позволяет создать инвагинационный мочеточниково-кишечный анастомоз, обладающий достаточной герметичностью и более эффективной клапанной функцией.
