Aim. To improve quality and duration of life in patients with locally advanced pancreatic cancer. Material and methods. Cryosurgery through laparotomy has been performed in 36 patients with locally advanced pancreatic cancer since 2012. There were 14 (38.9%) men and 22 (61.1%) women (mean age 58 ± 6.8 years). Dimensions of pancreatic tumors were from 4 to 10 cm. Domestic devices “CRYO-MT”, “CRYO-01”, “ELAMED” and cryoapplicators with a diameter 2–5 cm were applied. Target temperature was about 186 °С, time of exposure – 3–5 min. There were 1–5 sessions of cryoablation (mean 2.4) and their number depended on tumor dimensions. Local cryodestruction was supplemented by bypass anastomoses in 18 patients (50%). All patients subsequently underwent adjuvant chemotherapy with additional regional chemoembolization in 10 of them. Results. There were no lethal outcomes during cryodestruction and in postoperative period. Early postoperative complications occurred in 14 (38.8%) patients, severe complications – in 13.6%. Cryodestruction was followed by complete regression (39.2%) or significant improvement (41.6%) of pain syndrome. 6-, 12-, 24- and 36-month survival was 92%, 84%, 48% and 14%, respectively. Median survival was 18.2 months. Conclusion. Cryodestruction is able to improve patients’ quality of life due to reduced pain syndrome in case of locally advanced pancreatic cancer. Certain increase of survival was observed in additional chemotherapy.

Цель: улучшение результатов лечения терминальных неоперабельных стадий рака поджелу-дочной железы.Материалы и методы. С 2012 г. криодеструкция выполнена у 45 больных раком поджелудоч-ной железы. Мужчин было 20 (44,4%), женщин -25 (55,6%). Средний возраст составил 59±3 года. Головка ПЖ была поражена в 20 (44,4%) наблюдениях, головка-тело -в 13 (28,9%), тело -в 12 (26,7%). Было отмечено следующее распределение больных по стадиям: IV стадия -34 пациента, III -11. У 8 больных раком ПЖ с метастазами в печень применяли одновременное криовоздействие на опухоль ПЖ и образования печени. Локальная криодеструкция была допол-нена формированием обходных анастомозов в 29 наблюдениях (64,4%). В одном случае криоде-струкция была произведена на сегменте воротной вены после выполнения ПДР. Всем больным в последующем была проведена адъювантная химиотерапия, в 10 случаях дополненная региональ-ной химиоэмболизацией.Результаты. Определены показания и противопоказания к выполнению криодеструкции. Определены особенности послеоперационного ведения пациентов после проведения криоде-струкции. Прослежены ближайшие и отдаленные результаты. Внутрибрюшное кровотечение диагностировано в 2 (4,4%) случаях. Острый панкреатит был выявлен в 5 (11,1%) случаях. Нагноение в зоне манипуляции было отмечено у 2 (4,4%). Асцит выявлен у 9 (20,0%) больных (купирован в течение 5-8 дней терапевтически). Нагноение послеоперационной раны было отмечено в 2 (4,4%) наблюдениях. После криодеструкции отмечалось либо полное купирование болевого синдрома (38%), либо значительное снижение его интенсивности (41%). Выживаемость по Каплан-Мейеру у больных раком поджелудочной железы: 6 мес -56%, 12 мес -28%, 24 мес -15%, 28 мес -4,3%.Заключение. Криодеструкция при раке поджелудочной железы у неоперабельных больных является вмешательством, в значительной степени улучшающим качество жизни онкологичес-ких пациентов, в первую очередь, за счет снижения болевого синдрома. Cryosurgery used on patients with unresectable pancreatic cancer improved their quality of life, but mainly because of the pain relief. In postoperative patients, multifaceted changes in immunity were found, and the state of the immune system prior to surgery often was a decisive factor to indicate whether further disorders in the postoperative period would develop, or by contrast, it would boost its recovery. Some patients receiving cryosurgery showed immune system imbalance and activation, and of antitumor immunity in particular. It has been suggested that the advisability of immunotropic therapy for specific treatment algorithms should be predicted or the therapy should be suspended at some pathologic stage, and this has been immunologically confirmed. Cryosurgery should be considered as a reasonable alternative to the existing types of surgery for pancreatic cancer or as an essential component of multimodal therapy, consisting of topical cryosurgery, chemotherapy, and immunotropic therapy, to boost antitumor immunity and to discontinue cytoreductive therapy due to its toxic effects.

Minimally invasive ablative technologies is a promising direction in the treatment of some cancer patients including pancreatic cancer. Cryodestruction, radiofrequency ablation, irreversible electroporation and ultrasound ablation show encouraging results regarding destruction of tumor tissue, cytoreduction. These methods are associated with small number of complications and relatively easy to tolerate by patients. However, there is no single approach to their use in the complex treatment of these patients. Accumulation of data followed by comparative analysis of various ablation techniques is being carried out in many specialized clinics of the world including national hospitals. Ablation mechanisms of technologies, literature data and the authors' own experience in the treatment of pancreatic cancer are reported in the article. Further randomized prospective trials are required to determine the role of ablation methods in the complex treatment of tumors of parenchymal organs including pancreatic cancer.
