El hombro es una de las regiones anatómicas de mayor movilidad en la vida cotidiana, siendo una de las causas de consulta más frecuentes en el área de fisioterapia para su rehabilitación, pues la primera línea de acción es el tratamiento conservador del hombro; por ello, es de suma importancia conocer y evaluar el complejo articular del hombro, así como también de la región cervicotorácica, con los resultados de la valoración establecer una estrategia de tratamiento que pueden ir desde la terapia manual, el masaje terapéutico, cambios de temperatura hasta el empleo de otros agentes físicos. Paciente femenino de 42 años, con ocupación de asistente odontológica; no reporta discapacidad previa. Como antecedente médico se presenta accidente de tránsito en motocicleta sin producir fractura de hueso ni luxación, ocurrido ocho años antes de la consulta en fisiatría, la paciente tiene afectado el desarrollo de las actividades de la vida cotidiana. Recibió serie de tratamientos durante 10 sesiones, en las cuales se aplican agentes físicos, dado que anteriormente recibió tratamiento farmacológico sin resultados favorables. Se aplica protocolo de rehabilitación fundamentado en las técnicas de propiocepción y al finalizar la terapia la paciente reporta dolor leve, y mejora en la realización de actividades de la vida diaria.
Introducción: El método POLD, basada en la movilización resonante, ha mostrado tener un efecto positivo tanto en la disminución del dolor como del grado de incapacidad en pacientes con lumbalgia. Objetivo: Determinar la eficiencia del método POLD en la reducción del dolor en pacientes adultos mayores con lumbalgia Materiales y método: Se realizó una investigación cuasi experimental incluyendo 30 adultos mayores con dolor lumbar, quienes firmaron un consentimiento informado. Se usó la escala de EVA y Oswestry para determinar el nivel de dolor e incapacidad, respectivamente. La técnica POLD fue aplicada durante aproximadamente 20 minutos. El nivel de dolor e incapacidad antes y después de la intervención fueron comparados mediante la prueba de Wilcoxon con un nivel de probabilidad de 5%. Resultados: La aplicación del método POLD mostró tener un efecto positivo sobre la disminución del dolor evidenciado por la disminución del dolor de 8 a 7 en la escala de EVA, mientras que el grado de incapacidad de los adultos mayores se mantuvo dentro del rango de incapacidad moderada, la cual va desde 21 a 40%. Además, se observó que el nivel de dolor e incapacidad fue mayor en mujeres que en hombres. Conclusión: Se demuestra el efecto positivo de la aplicación del Método POLD para problemas de lumbalgia en adultos mayores, por lo que se sugiere repetir este tipo de intervenciones en otros centros geriátricos del Ecuador
Introducción: Los trastornos musculoesqueléticos están asociados con el trabajo repetitivo, derivando en una serie de problemas de salud que van desde pequeñas molestias hasta lesiones con discapacidad permanente del paciente. Objetivo: Determinar la efectividad de un programa de prevención de lesiones musculoesqueléticas en estudiantes que reciben clases de modalidad virtual. Material y métodos: Se realizó una investigación cuasi experimental que incluyó a 34 estudiantes quienes firmaron el consentimiento informado, se aplicó una encuesta donde se solicitó información sobre variables sociodemográficas y evaluación de las condiciones del sitio donde reciben clases. Además, se determinó la presencia de síntomas musculoesqueléticas mediante la aplicación del cuestionario Nórdico. Posteriormente se aplicó un programa de intervención que consistió en charlas ergonómicas, estiramientos y pausas activas durante cuatro semanas consecutivas. Resultados: El 90,9% de los estudiantes reportó algún tipo de dolor musculoesquelético, siendo más frecuente en el cuello (94,1%), en la región lumbar (79,45%) y el hombro (70,6%), de la cual la mayor parte de los entrevistados describieron niveles de dolor intermedio en cuello (41,2%), hombro (29,4%) y dorsal o lumbar (29,4%), mientras que en codo y antebrazo y muñeca y mano la mayoría no presento molestias. Posterior a la intervención, el nivel de dolor disminuyó en las diferentes partes del cuerpo evaluadas (p< 0,05). Conclusión: Con base en los resultados se demuestra el efecto positivo del plan de prevención sobre la mejora de dolor musculoesqueléticas de los estudiantes intervenidos, por lo que se sugiere aplicar este tipo de programas en otras carreras.
Introducción: La Cervicalgia causa dolor de cuello como consecuencia de nódulos palpables denominados puntos gatillo, los cuales pueden ser tratados mediante la técnica de liberación miofascial. Objetivo: Determinar la efectividad de la aplicación de la técnica de liberación miofascial sobre el punto gatillo del músculo trapecio para aliviar el dolor cervical. Material y métodos: Investigación de tipo descriptivo longitudinal, cuasiexperimental, para determinar la efectividad de la técnica de liberación miofascial. Se analizó los datos de forma numérica, antes y después de la aplicación del tratamiento de rehabilitación. Se empleó la escala analógica visual del dolor (EVA), para determinar y diferenciar el grado de intensidad; el cuestionario del índice de discapacidad de cuello con el que identificamos el dolor cervical, y cómo éste puede interferir en las actividades de la vida diaria; otro instrumento aplicado fue el test goniómetro para determinar el grado de función a nivel cervical. La muestra estuvo constituida por 27 pacientes, con edad promedio de 33,59 ±8,19 años. Para el análisis de los resultados se utilizó el sistema informático SPSS versión 21.0. La estadística fue univariada para las variables sociodemográficas, y bivariada para la comparación longitudinal. Los niveles de significancia adoptados fue 0.05 para las comparaciones generales del control interno. Resultados: La media de las valoraciones EVA pasó de 7,59±1,37 antes del tratamiento terapéutico a 2,22±1,48 después del tratamiento; el índice de discapacidad de cuello cambió de 32,59±4,81 al inicio del estudio a 14,48±2,74 después de la terapia; mientras que, todas las mediciones con el goniómetro en los movimientos de flexión y extensión mejoraron de un 50 % de limitación a un 85 % de función, aumentando el rango articular en todos sus movimientos después de la aplicación del protocolo de rehabilitación en donde incluía la terapia de liberación miofascial, indicando mayor movilidad del cuello. Todos los cambios fueron estadísticamente significativos (p<0.05). Conclusión: La técnica de liberación miofascial es efectiva para la reducción del dolor cervical.
