Objetivo: Descrever os achados de um programa de detecção de glaucoma em São Paulo dando-se ênfase para dados de prevalência de glaucoma e doenças associadas. Métodos: Durante uma semana, em agosto de 1997, foi realizada no Hospital do Servidor Público Estadual de São Paulo, uma campanha de detecção de glaucoma. Durante este período, 1438 pacientes com idade variando entre 40 e 87 anos (média de 58,24 ± 10,88 anos), foram examinados quanto ao glaucoma. Idade mínima de 40 anos foi utilizada como critério de inclusão. O exame de triagem consistiu em um questionário padronizado, medida da pressão intra-ocular (PIO) e avaliação do nervo óptico por oftalmoscopia direta. Pacientes com pressão intra-ocular elevada e/ou disco óptico suspeito foram encaminhados para exame oftalmológico detalhado, campo visual e estereofoto de papila. As fichas clínicas dos pacientes foram revistas por dois experientes observadores. Resultados: Cento e cinco pacientes (7,3%) foram diagnosticados como portadores de glaucoma sendo 86,7% desses (6,3% do total examinado) portadores de glaucoma primário de ângulo aberto (GPAA). Cinqüenta e seis (61,5%) dos pacientes com glaucoma primário de ângulo aberto apresentaram pressão intra-ocular de triagem abaixo de 22 mmHg, e somente 7 pacientes estavam em uso de medicação ocular hipotensora. Outros 65 pacientes (4,5%) apresentavam disco óptico suspeito, sem definitivo defeito de papila ou campo visual. Quarenta e nove pacientes (3,4%) foram classificados como hipertensos oculares (pressão intra-ocular superior a 21 mmHg com disco óptico normal). Pacientes com glaucoma primário de ângulo aberto eram significantemente mais idosos que indivíduos sem glaucoma. Como esperado, a média de pressão foi significativamente mais alta entre os pacientes com glaucoma primário de ângulo aberto em relação aos normais. Nenhuma relação significativa foi encontrada entre diabete melito, hipertensão sistêmica, doença coronariana, enxaqueca e história familiar de glaucoma e pacientes com glaucoma primário de ângulo aberto ou normais. Conclusões: Foi encontrada alta prevalência de glaucoma na população estudada. A maioria dos novos diagnósticos apresentavam pressão intra-ocular inferior a 22 mmHg. Idade avançada e pressão intra-ocular elevada foram associados ao glaucoma primário de ângulo aberto. (1) .
