INTRODUÇÃO: a análise facial tem sido um recurso diagnóstico valorizado desde os primórdios da Ortodontia. Vários autores tentaram estabelecer referências de normalidade na direção das quais os pacientes ortodônticos deveriam ser tratados. Essa preocupação da Ortodontia está em concordância com a expectativa do paciente, cuja principal motivação para o tratamento ortodôntico é a melhora estética. Para que os objetivos do profissional possam solucionar a queixa do paciente é fundamental que o ortodontista conheça os parâmetros utilizados pela sociedade na avaliação estética. Sugerimos, por meio desse trabalho, uma nomenclatura que permita a realização da Análise Facial Subjetiva, estética e morfológica. OBJETIVO: avaliar a aplicação prática da análise. METODOLOGIA: solicitou-se a um grupo heterogêneo de avaliadores (14 ortodontistas, 12 leigos e 7 artistas) que dessem notas ao perfil facial de 100 indivíduos (50 de cada gênero) classificando-os como esteticamente desagradáveis (notas 1, 2 ou 3), esteticamente aceitáveis (notas 4, 5 ou 6) e esteticamente agradáveis (notas 7, 8 ou 9). RESULTADOS: 89% dos perfis foram esteticamente aceitáveis, 8% desagradáveis e 3% agradáveis. Em 38,35% das justificativas, o nariz foi a estrutura responsável pela estética desagradável, seguida pelo mento ("queixo") em 18,9% dos relatos. CONCLUSÃO: foi possível observar, portanto, que a Análise Facial Subjetiva é mais um instrumento diagnóstico, que tem sua importância aumentada por ser o parâmetro pelo qual o paciente e as pessoas com as quais ele convive vão avaliar os resultados do tratamento.
Objetivo: avaliar as características numéricas do perfil facial. Metodologia: avaliou-se uma amostra de 50 indivíduos, brasileiros, adultos, leucodermas, portadores de selamento labial passivo, classificados pela avaliação morfológica do perfil em 41% Padrão II e 9% Padrão III. A análise facial numérica do perfil foi realizada em fotografias padronizadas. As medidas obtidas para as variáveis estudadas nos dois padrões foram comparadas entre si e com as do Padrão I por meio da Análise de variância (ANOVA) complementada pelo Teste de Tukey. Resultados e Conclusões: não foram observadas diferenças estatísticas entre os Grupos Padrão I, II e III nas medidas obtidas para os ângulos nasolabial e do sulco mentolabial e a proporção entre as alturas faciais média e inferior. O ângulo interlabial foi mais obtuso no Padrão III. Esse Padrão também apresentou menor convexidade facial e menor proporção do terço inferior da face. O ângulo do terço inferior da face, que avalia a protrusão mandibular, foi mais obtuso no Padrão II. A classificação do Padrão é realizada pela avaliação da face nas visões frontal e lateral. Os indiví-duos podem ser classificados como Padrão I, II, III, Face longa ou Face curta. O Padrão I é identificado pela normalidade facial. A má oclusão quando presente é apenas dentária não associada a qualquer discrepância esquelética sagital ou vertical. A r t i g o i n é d i t oOs Padrões II e III são caracterizados pelo degrau sagital respectivamente positivo e negativo entre a maxila e a mandíbula. Nos Padrões face longa e face curta a discrepância é vertical. Nos pacientes com erros esqueléticos, as más oclusões são geralmente conseqüentes dessas discrepâncias.Na avaliação do perfil, o Padrão I é caracterizado por um grau moderado de convexidade. A expressão da maxila na face é identificada pela presença da projeção zigomática e depressão infraorbitária, que podem ser verificadas também na visão frontal. A linha de implantação do nariz, levemente inclinada para anterior, denota adequada posição maxilar.
Objetivo: o objetivo do presente estudo foi determinar as medidas do perfil facial de brasileiros portadores de equilíbrio facial, denominados Padrão I. Metodologia: a amostra foi constituída por 50 indivíduos (32 femininos e 18 masculinos), brasileiros, adultos, leucodermas, com idade média de 23 anos. Fotografias padronizadas do perfil foram obtidas. Sobre essas foram executados os traçados por dois avaliadores, que verificaram: 1) ângulo nasolabial; 2) ângulo mentolabial; 3) ângulo interlabial; 4) ângulo de convexidade facial; 5) ângulo de convexidade facial total; 6) ângulo do terço inferior da face; 7) proporção entre a altura facial anterior média e a altura facial anterior inferior e 8) proporção do terço inferior da face. Resultados e Conclusões: não houve diferença estatística relevante entre as duas medidas realizadas. Os valores de média, desvio-padrão, valores máximos e mínimos obtidos para cada variável estudada foram: 1) ângulo nasolabial: 108,13° ± 9,75° (81° a 127°); 2) ângulo do sulco mentolabial: 132,37° ± 9,82° (110,5° a 152°); 3) ângulo interlabial: 135,35° ± 11,14° (116,5° a 159,5°); 4) ângulo de convexidade facial: 12,32°± 3,93° (4° a 19,5°); 5) ângulo de convexidade facial total: 137,85° ± 4,08° (129,5° a 147,5°); 6) ângulo do terço inferior da face: 103,41° ± 8,12° (88° a 124°); 7) proporção entre os terços médio e inferior da face: 0,93 ± 0,10 (0,80 a 1,21) e 8) proporção do terço inferior da face: 0,45 ± 0,06 (0,30 a 0,67). Propõe-se esse conjunto de medidas como um padrão de referência para avaliação facial numérica de adultos, brasileiros, brancos.
ObjectiveTo compare dental and skeletal anchorages in mandibular canine retraction by means of a stress distribution analysis. MethodsA photoelastic model was produced from second molar to canine, without the first premolar, and mandibular canine retraction was simulated by a rubber band tied to two types of anchorage: dental anchorage, in the first molar attached to adjacent teeth, and skeletal anchorage with a hook simulating the mini-implant. The forces were applied 10 times and observed in a circular polariscope. The stresses located in the mandibular canine were recorded in 7 regions. The Mann-Whitney test was employed to compare the stress in each region and between both anchorage systems. The stresses in the mandibular canine periradicular regions were compared by the Kruskal-Wallis test. ResultsStresses were similar in the cervical region and the middle third. In the apical third, the stresses associated with skeletal anchorage were higher than the stresses associated with dental anchorage. The results of the Kruskal-Wallis test showed that the highest stresses were identified in the cervical-distal, apical-distal, and apex regions with the use of dental anchorage, and in the apical-distal, apical-mesial, cervical-distal, and apex regions with the use of skeletal anchorage. ConclusionsThe use of skeletal anchorage in canine retraction caused greater stress in the apical third than the use of dental anchorage, which indicates an intrusive component resulting from the direction of the force due to the position of the mini-implant and the bracket hook of the canine.
Objective: To evaluate the influence of age, gender, sagittal occlusal relationship, facial pattern and 8 facial profile measures on profile aesthetics. Methods: Contingency tables, chi-square test and Cramer's coefficient were used to evaluate the possible association between the scores assigned by 32 examiners (14 orthodontists, 12 laypeople and 6 artists) to the aesthetics of the profile of 100 Brazilian Caucasian adults, all patients with lip seal competence, and age, gender, sagittal occlusal relationship, facial pattern and the variables of the numerical analysis of the facial profile. Results: No association was found between age, gender and sagittal occlusal relationship and the aesthetics of facial profile. An association was observed between profile scores and facial pattern, facial convexity angle and lower face angle. Conclusions: Among the factors evaluated in this study, facial profile convexity and anterior chin projection were the key determinants of facial profile aesthetics. AbstractKeywords: Facial profile. O presente estudo teve como objetivo avaliar a espessura do processo alveolar da região anterior da maxila e mandíbula em pacientes portadores de discrepâncias ântero-posteriores. Após a seleção de telerradiografias laterais de cinqüenta e dois pacientes entre idades de sete e 13 anos, mediu-se a espessura do processo alveolar da região anterior da maxila e mandíbula. Todos os pacientes apresentavam o ângulo do plano mandibular entre vinte e trinta graus e discrepância óssea ântero-posterior entre maxila e mandíbula. Após a análise estatística (teste qui-quadrado) observou-se que não houve dependência entre a espessura do processo alveolar da maxila e mandíbula e a idade do paciente. Todavia houve dependência entre o tipo de má oclusão e a espessura de osso vestibular na região anterior da maxila. Os pacientes portadores de má oclusão de Classe III apresentaram maior porcentagem de redução de osso vestibular da região anterior da maxila quando comparados aos pacientes Classe II. Os pacientes com tendência ao crescimento vertical apresentaram a dimensão reduzida de osso lingual da maxila e osso vestibular da mandíbula. ResumoPalavras-chave: Espessura do processo alveolar. A R T I G O I N É D I T O INTRODUÇÃOO delineamento dos limites do tratamento ortodôntico é fator importante a ser considerado, principalmente em casos ortodônticos-cirúrgicos limítrofes. Este delineamento realizado antes do tratamento seria extremamente benéfico. O planejamento do tratamento ortodôntico poderia ser bem valorizado, especialmente em situações nas quais a discrepância esquelética é severa ou onde um ou ambos os arcos podem acomodar somente reposicionamento limitado do dente. As tábuas corticais vestibulares e linguais ao nível do ápice dos incisivos podem representar os limites anatô-micos da movimentação dentária 5 .
The aim of the study was to assess the tensions generated in the long axis of the implants and the interimplants in the cervical, middle, and apical regions when subjected to different loads with or without interposition of the flat occlusal plane splint. A photoelastic model was made with 2 external hexagon implants located in the space corresponding to the second premolar and molar inferiors. A screw-retained metal superstructure was installed on the implants with a torque of 20 N·cm, and the set (photoelastic model with superstructure) were positioned in the circular polariscope in the dark-field configuration to observe the distribution of isochromatic fringes around the implants and interimplant areas. Photographic records were obtained before the application of the occlusal load with the following loading conditions: 300, 600, and 900 N, with and without interposition of the occlusal plane splint. The decrease in stress with the application of the flat occlusal plane splint became more evident after the application of the 600-N load. Generally, the major stress magnitude occurred in the cervical region for interimplant areas and in the apical region around implants. Use of occlusal splint in the loading of 300, 600, and 900 N reduced tension 33.22%, 66.66%, and 73.33%, respectively, in the samples. It can be concluded that the interposition of the occlusal plane splint caused a reduction in tension resulting from the simulation of occlusal overload.
