Este estudo compete a um levantamento de literatura que tem como finalidade explorar trabalhos publicados no intervalo dos anos de 2015 a 2020, que façam explanação sobre a saúde da mulher em comunidades quilombolas e, com isso, mencionar os questionamentos frequentes aos artigos, proporcionando argumentação e reflexão para melhorar o acesso a saúde, a essas comunidades. Introdução: Gestantes de alto risco são mais propensas a complicações e partos prematuros, podem necessitar de hospitalização e farmacoterapia para controle pressórico e glicêmico sobretudo. As mudanças fisiológicas associadas a gravidez aumentam a vulnerabilidade a eventos adversos como aqueles decorrentes de interações medicamentosas (IM). A análise de rede compreende uma abordagem eficaz para identificar padrões complexos em saúde como a ocorrência de IM. Objetivo: caracterizar a ocorrência de IM, os principais medicamentos e desfechos envolvidos com IM graves via análise de rede. Métodos: Coorte prospectiva (set/19 a jun/22) incluindo 297 gestantes hospitalizadas acompanhadas diariamente em uma maternidade escola. As IM foram classificadas utilizando-se a base de dados Micromedex em leves, moderadas e graves. Foi feita análise descritiva (Stata© v15) e de rede (Gephi© 0.9.1). Na análise de rede, empregou-se o algoritmo de Yifan Hu e foi adotado o critério de betweenness centrality para identificar os fármacos mais implicados com interações graves. Resultados: As pacientes apresentaram idade média de 30,4 ± 6,6 com 30,9 ± 7,0 semanas de gestação com predomínio de hipertensão gestacional (32%) e diabetes gestacional (35,7%). O tempo médio de tratamento foi de 5,6 ± 4,2 e a média total de medicamentos prescritos por paciente foi 7,4 ± 2,9. Cerca de 85% apresentaram pelo menos uma IMP no período e uma taxa de incidência de 146,3 IMP por 1000 pacientes-dia. Os pares de IMP mais frequentes foram hidralazina/metildopa (10,6%) e hidralazina/ dipirona (5,6%), ambas moderadas. Em relação as IMP graves, a análise de rede destacou três clusters: a) levomepromazina associada a fármacos depressores e anticolinérgicos; b) dipirona associada a anti-inflamatórios e anticoagulantes e c) sulfato ferroso com fármacos com potencial para quelação. Conclusão: IMP em gestantes hospitalizadas são frequentes, sendo as IMP graves associadas ao risco de depressão cardiorrespiratória, hemorragia digestiva alta e anemia.
