Objective:To investigate the association between handgrip strength (HS) and physical activity in physical frailty elderly. Method: Cross-sectional quantitative study with a sample of 203 elderly calculated based on the population estimated proportion. Tests were applied to detect cognitive impairment and assessment of physical frailty. Descriptive statistics and multivariate analysis by binary logistic regression were used, and also Student's t-test and Fisher's exact test. Results: A total of 99 (64.3%) elderly showed decreased handgrip strength and 90 (58.4%) elderly presented decrease in physical activity levels. There was a statistically significant difference between these two components (p=0.019), in which elderly who have decreased HS have lower levels of physical activity. For low levels of physical activity and decreased HS, there was no evidence of significant difference in the probability of the classification as frail elderly (p<0.001). Conclusion: The components handgrip strength and physical activity are associated with the frail elderly. The joint presence of low levels of physical activity and decreased handgrip strength leads to a significantly higher probability of the elderly to be categorized as frailty.
Objetivo: analizar la asociación entre la cognición y los resultados finales de los exámenes de aptitud física y mental para habilitación vehicular en adultos mayores.Método: Se trata de un estudio transversal realizado en clínicas de tránsito acreditadas para realizar exámenes de habilitación vehicular. La muestra del tipo probabilística fue constituida por 421 adultos mayores (≥ 60 años). Los datos fueron colectados por medio de Mini Examen del Estado Mental (MEEM) y consulta en el Registro Nacional de Conductores Habilitados. Para el análisis de los datos se utilizó estadística descriptiva y aplicación de los testes Kruskal-Wallis y Qui-cuadrado. Los valores de p≤0,05 indicaron significancia estadística.Resultados: Para el MEEM el puntaje promedio fue de 27,13 ± 2,53 puntos, 92,2 % (n =388) presentaron cognición normal y 7,8 % (n =33) comprometimiento cognitivo. En cuanto a los resultados de las pruebas de aptitud física y mental para habilitación vehicular 71,5 % (n =301) se consideraron aptos con restricción, el 21,9 % (n =92) aptos y el 6,7 % (n =28) inaptos temporales. Se observó una asociación significativa entre los resultados de la habilitación vehicular y los escores cognitivos (p<0,001) y no hubo asociación a las categorías cognitivas (p =0,172). Los escores más bajos de cognición se identificaron en ancianos aptos con restricción e inaptos temporalmente. Conclusión: Los resultados evidencian la inconsistencia del MEEM para evaluar la cognición. Se recomienda a las clínicas de tránsito la utilización del MEEM con cautela, como herramienta de evaluación cognitivo en conductores adultos mayores.
Aim: to analyze the association between physical frailty and the results of fitness capacity exams for driving vehicles among elder Brazilians. Methods: this is a cross sectional study, performed in traffic medicine clinics of the city of Curitiba (Brazil). The data was collected through the physical frailty tests, the use of a structured questionnaire, and searches on the records of the Brazilian National Register of Qualified Drivers.To analyze the information, the authors used descriptive statistics and non-parametrical tests. Results: One hundred seventy two elderly people, of whom 56.4% pre-fragile and 43.6% non-fragile. 25.0% were considered fit for driving, 68.6% were considered fit, but with some restrictions, and 6.4% were placed as temporarily unfit for driving. There was no association between frailty condition and the final results for driving fitness (p= 0.8934). Physical frailty was significantly associated to the restrictions observed for those who were fit under restrictions (p= 0.0313), according to the weekly amount of kilometers traveled (p= 0.0222), and to car accidents occurred after the age of 60 (p= 0.0165). Conclusion: Physical frailty was significantly associated to the restrictions related to driving, reason to which makes important to manage frailty over this group of drivers. However, no association observed between physical frailty and the final result for driving vehicles.
Objetivo: delinear o perfil dos pacientes assistidos em um Serviço de Atendimento Domiciliar da cidade de Curitiba. Métodos: desenho transversal, retrospectivo, abordagem quantitativa; coleta de dados dos pacientes assistidos no serviço em 2017; contendo as variáveis sociodemográficas e clínicas. Realizou-se análise estatística descritiva e modelos de regressão. Resultados: em 2017 houve 1798 atendimentos de 1436 pacientes. O perfil apontou maioria dos pacientes do sexo feminino, média de idade acima de 71 anos e raça branca; a procedência predominante dos assistidos foram as Unidades Básicas de Saúde ou Estratégia de Saúde da Família com tempo médio de atendimento superior a 38 dias; prevalência de distúrbios neurológicos; desfecho de atendimento foi alta. O custo médio mensal foi de R$ 1.403,56 por paciente. Conclusão: o perfil dos pacientes indicou maioria de mulheres, brancas, com até oito anos de estudo, idade acima de 71 anos e assistidos por distúrbios neurológicos.
The absence of physical frailty does not necessarily points out that the elderly are able to drive motor vehicles. Tracking the frailty subsidizes preventive interventions, which seek to interfere positively in the act of driving. This is an unprecedented study in nursing and it highlights an essential field for the performance of gerontological nursing.
INTRODUÇÃO: Escores prognósticos são cruciais para evitar medidas desproporcionais em cuidados paliativos, mas seu uso em pacientes não oncológicos, em cuidado domiciliar e em países subdesenvolvidos apresenta carência de evidência. OBJETIVO: Comparar a acurácia dos instrumentos preditivos de sobrevivência Palliative Performance Scale (PPS), Karnofsky Performance Status (KPS), Palliative Prognostic Index (PPI) e Palliative Prognostic Score (PaP) na população idosa do Serviço de Atenção Domiciliar (SAD) de Curitiba. MÉTODOS: Trata-se de estudo de acurácia diagnóstica de caráter prospectivo e observacional conduzido com idosos em cuidados paliativos no SAD de Curitiba. Aplicaram-se os escores PPS, KPS, PPI e PaP, e após 90 dias os pacientes foram avaliados quanto aos desfechos internamento hospitalar e óbito. RESULTADOS: A amostra final foi de 53 pacientes com idade ≥ 60 anos; desses, 28,3% (n = 15) tinham ≥ 85 anos. Na análise do desfecho binário (óbito ou não), foi identificado que a escala PaP teve melhor acurácia (79,2%). Na análise do desfecho ordinal (óbito, hospitalização ou nenhuma dessas opções), a escala com melhor acurácia foi o PPI (58,5%), mas todas as escalas avaliaram a amostra de forma semelhante, sem significância estatística. Em ambas as análises, os pacientes com idade ≥ 85 anos obtiveram resultados semelhantes aos demais. Apesar de o SAD ser um ambiente distinto, obteve resultados similares aos de outros estudos que incluíram hospitais e hospices. CONCLUSÃO: As escalas prognósticas apresentaram melhor acurácia na predição somente de óbito, mas quando usadas para relacionar a hospitalização, perdem sua acurácia. Não houve uma escala superior às demais, e não existiu distinção da acurácia referente à idade.
