Osteoma do etmóide com invasão orbitária: relato de três casos e revisão da literatura INTRODUÇÃO Osteomas são tumores benignos raros, lesões ósseas encapsuladas de crescimento lento e ilimitado, etiologia desconhecida, podendo estar associados a traumas prévios (5 a 29%); afetam mais comumente o nariz e os seios paranasais, sendo mais comuns nos seios frontal (57%) e etmóide e acometendo mais raramente os seios esfenoidal e maxilar (1)(2)(3)(4) . Ocorre mais no sexo masculino 2:1 -3,75:1, com incidência variável de 0,01 a 3% (5)(6) , ocorrendo mais na terceira e quarta décadas de vida (média= 38,4 anos) (3) , ou quinta e sexta décadas (3,5,(7)(8)(9)(10) . A dor é o sintoma mais freqüente, geralmente localizada na face (70%) ou região frontal, surgindo após 6 meses a 3 anos de crescimento tumoral.Parece que o osteoma se desenvolve sobre a junção do osso frontal, de origem membranosa e do osso etmoidal, de origem endocondral. Entretanto, a etiologia embrionária como causa isolada dos osteomas do seio frontal é pouco provável, uma vez que a localização destes tumores na junção destes ossos é encontrada em apenas 10% a 20% dos pacientes (5) . Além da etiologia traumática e embrionária admite-se a teoria genética e infecciosa. Na teoria genética existe a possibilidade de participação de um padrão de penetrância, expressividade e dominância desconhecidos pela ocorrência de osteomas múltiplos na síndrome autossômica dominante de Gardner, na comunidade Mórmon de Utah (11)(12) . A etilogia infecciosa é discutida devido a 30% dos pacientes com osteoma apresentarem infecção sinusal. Entretanto, alterações infecciosas crônicas da mucosa como causa do osteoma são altamente questionáveis, podendo ser conseqüência do osteoma e não sua causa (10)(11)13) . A invasão orbitária é relativamente incomum, ocorrendo em 0,9% a 5,1% dos casos (5,(7)(8)(9)(10)(11)(12)(13)(14)(15)(16) . Quando invadem a região da fossa lacrimal, podem simular ou evoluir com quadro de obstrução das vias lacrimais, condição que é pouco freqüente (17) . São apresentados três raros casos de osteoma do etmóide, com extensão para o quadrante medial da órbita e que apresentavam algumas particularidades que os tornavam ainda mais inusitados, como terem acontecido em mulheres, em faixa etária não usual e com queixa de epífora. Os achados radiográficos foram típicos da afecção e os casos foram operados, com resolução do problema. RESUMO RELATOS DE CASOS
INTRODUÇÃONa gênese do pterígio, a inflamação crônica pode ser um fator que gera crescimento da conjuntiva sobre a córnea, supondo-se que os linfócitos T possam estar envolvidos (1) .O tacrolimus é um antibiótico macrolídeo, extraído da fermentação de um caldo do microrganismo Streptomyces tsukubaensis. Em animais, demonstrou-se que o tacrolimus (FK-506) causa supressão da imunidade humoral e inibe a ativação do linfócito T, apesar de seu exato mecanismo de ação não ser conhecido. Evidências experimentais sugerem que o tacrolimus se liga a uma proteína intracelular (proteína de ligação do FK-506), formando um complexo que se liga a calcineurina e inibe a atividade da serina-treonina, inibindo assim a expressão de linfocinas, a apoptose e a degranulação (2) . Exposição de fibroblastos provenientes de pterígios recidivados e da cápsula de Tenon normal ao tacrolimus (FK-506)Descritores: Pterígio/patologia; Fibroblastos/patologia; Técnicas de cultura de células; Proliferação de células; Recidiva; Tracolimus/uso terapêutico Objetivo: Avaliar a taxa de proliferação de fibroblastos provenientes de pterígios recidivados e da cápsula de Tenon normal, quando expostos in vitro ao tacrolimus (FK 506). Métodos: Foi realizado estudo prospectivo, controlado, avaliando-se 8 amostras de explantes de cápsula de Tenon de pterígios recidivados e 6 de cápsula de Tenon normal, obtidas na Faculdade de Medicina de Botucatu -UNESP. A cápsula de Tenon normal foi colhida da região temporal inferior, do mesmo portador de pterígio. As amostras foram cultivadas em meio específico e posteriormente expostas ao tacrolimus 1M (FK 506), em única exposição, com avaliação da taxa de proliferação celular 1, 5, 12 e 19 dias após a exposição. Resultados: Avaliando-se a proliferação dos fibroblastos provenientes de cápsula de Tenon de pterígios recidivados e da cápsula de Tenon normal, os fibroblastos expostos ao tacrolimus tiveram taxa de proliferação significativamente menor (p<0,05) do que quando não houve exposição a droga, quando a avaliação foi feita 1 dia após a exposição. Quando a avaliação foi feita 19 dias após a exposição, a taxa de proliferação foi maior nos grupos expostos a droga. Conclusão: Os fibroblastos provenientes da cápsula de Tenon de pterígios recidivados apresentaram taxa de proliferação significativamente menor um dia após a exposição ao tacrolimus. Novos estudos devem ser realizados para definir dose e tempo de exposição dos fibroblastos a droga, com o intuito de definir se o tacrolimus pode ser útil como tratamento coadjuvante do pterígio. INTRODUÇÃOO receio da recidiva do pterígio após exérese, leva ao uso de medicações adjuvantes. A triancinolona é um corticosteroide potente, de depósito, que atua inibindo a síntese de prostaglandinas e leucotrienos, com consequente efeito anti-inflamatório. Utilizada há anos em vários campos da medicina, a triancinolona é um corticosteroide que vem sendo utilizado na Oftalmologia recentemente, no tratamento de inflamações intraoculares, edema macular associa do à pseudofacia, retinopatia diabética, oclusão de veia central da retina, vasculites retinianas e membrana neovascular sub-retiniana de várias etiologias. Efeitos adversos significativos, tais como, catarata, hipertensão ocular, descolamento de retina e endoftalmite, já foram relatados (1) .Porém, pouco ainda se sabe sobre o uso da triancinolona como medicamento adjuvante no tratamento do pterígio, o que motivou o presente estudo, realizado com o objetivo de avaliar a atividade pro liferativa dos fibroblastos da cápsula de Tenon, provenientes de explantes de pterígios recidivados, em cultura de células, comparando-os com a atividade proliferativa dos fibroblastos da cápsula de Tenon normal, quando expostos à triancinolona.
