Objective: The present study aims to evaluate the prevalence of and factors associated with the use of tobacco and alcohol among elderly people living in the northern part of the city of Juiz de Fora, Minas Gerais, Brazil. Method: A cross-sectional study conducted through a home survey was performed with a sample of 423 elderly citizens. Interviews were conducted through a questionnaire including the Fagerström and Audit-C tests. The data were subjected to descriptive statistical analysis and multinomial regression. Results: The prevalence of elderly smokers was 9.0%, 32.0% were former smokers, 26.7% of the sample consumed alcoholic beverages and 3.2% used alcohol and smoked. In the multinomial logistic regression model, the factors that were significantly associated with smoking were the male gender, an age of 60 to 70 years old; the presence of self-reported health problems; while the consumption of alcohol was associated with the male gender and frailty. Conclusion: There was a low prevalence of elderly people living in the community who consumed alcoholic beverages and/or were smokers. Such individuals, however, almost exclusively suffered from impaired health and potentially a poor quality of life. Regarding the profile of such elderly persons, there were similarities between the socio-demographic and health variables, suggesting the possibility of a more targeted approach to these individuals.
Resumo Objetivo identificar e mensurar fatores associados à qualidade de vida relacionada à saúde de pacientes renais crônicos em hemodiálise e analisar a associação da qualidade de vida relacionada à saúde e a adesão ao regime terapêutico hemodialítico. Métodos Estudo transversal com 258 pacientes em terapia hemodialítica. Avaliou-se a qualidade de vida relacionada à saúde através do questionário Kidney Disease Quality of Life Short Form. Foram analisados os componentes específicos da doença renal crônica e os componentes sumarizados físico e mental. A análise da não adesão ao regime terapêutico hemodialítico teve como base os indicadores do estudo The Dialysis Outcomes and Practice Patterns Study. Utilizaram-se, para a análise os testes de Mann Whitney e Kruskal Wallis. Resultados Indivíduos do sexo feminino, com menos de 60 anos, brancos, baixo nível socioeconômico, que necessitam de acompanhante, em terapia hemodialítica por menos de 5 anos, que possuem prescrição medicamentosa com dez ou mais fármacos, com baixos níveis séricos de albumina e hemoglobina e que não aderiram à restrição hídrica e à terapia apresentaram piora na qualidade de vida relacionada à saúde. Conclusão A avaliação da qualidade de vida relacionada à saúde dos pacientes em hemodiálise apresentou associação com as características sociodemográficas, clínicas e terapêutica. A monitoração desses parâmetros, bem como a busca do alcance dos padrões recomendáveis poderão subsidiar a prática clínica, aprimorar o planejamento assistencial e trazer melhorias em diversos aspectos da vida dos pacientes.
Introdução: O crescente número de pacientes que necessitam de terapia renal substitutiva impacta o Sistema Único de Saúde. A complexidade do tratamento hemodialítico requer cuidados especializados. Objetivo: Analisar o fluxo assistencial de pacientes em terapia hemodialítica a partir da distribuição geográfica estabelecida no Plano Diretor de Regionalização do estado de Minas Gerais. Método: Estudo descritivo de mapeamento dos fluxos assistenciais de paciente em terapia hemodialitíca no estado de Minas Gerais. Resultados: As macrorregiões Nordeste e Jequitinhonha possuem as maiores necessidades de deslocamento médio até as clínicas dialíticas com 60,67 km e 50,29 km, respectivamente. Em relação ao fluxo de pacientes entre as macrorregiões para realização de hemodiálise, Jequitinhonha e Nordeste obtiveram maior percentual de escape com 4,43% e 3,35%, respectivamente. Conclusão: Garantir o acesso a terapia próximo à residência é de fundamental importância para minimizar os impactos psicossociais, físicos e econômicos e ainda melhorar aspectos relacionados à qualidade de vida, pois possibilitará menor tempo gasto exclusivamente com tratamento.
Resumo Introdução A doença renal crônica apresenta-se como um problema de saúde pública por causa de sua prevalência, dos custos envolvidos no tratamento e da alta taxa de morbimortalidade. Objetivo Avaliar a não adesão ao regime terapêutico de pacientes em hemodiálise e fatores associados. Método Estudo transversal com pacientes que realizam hemodiálise em um hospital universitário e duas clínicas privadas conveniadas ao Sistema Único de Saúde. Padrões para avaliação da não adesão ao regime terapêutico hemodialítico tiveram como base os indicadores estabelecidos pelo The Dialysis Outcomes and Practice Patterns Study: restrição hídrica, regime dietético, regime medicamentoso e terapia hemodialítica. Não conformidade em pelo menos um dos aspectos do tratamento foi considerada não aderência à terapêutica. Resultados Indivíduos com idade inferior a 60 anos tiveram maior probabilidade de não aderir à restrição hídrica, à terapêutica e aos regimes dietético e medicamentoso. Pacientes anúricos tiveram maior chance de não adesão à restrição hídrica e à terapêutica. Indivíduos com hipoalbuminemia e hipoemoglobinemia tiveram maior probabilidade de não aderir à restrição hídrica, enquanto pacientes que se autodeclararam não brancos apresentaram maior chance de não aderir à terapêutica. Nenhuma variável explicativa da adesão à terapia hemodialítica se manteve significativa no modelo final da regressão logística. Conclusão A inconformidade relacionada aos aspectos do tratamento pode resultar em aumento de hospitalização e gastos com saúde, piora do estado clínico, maiores intercorrências no tratamento e aumento da taxa de mortalidade.
