QUAL É O SEU DIAGNÓSTICO?HISTÓRIA DA DOENÇA Paciente do sexo feminino, 80 anos, branca, apresentando lesão nodular exofítica, endurecida, com bordos nítidos e coloração enegrecida, medindo 1,5 x 1,2cm, localizada na região malar direita, com um ano de evolução (Figura 1). Não se observaram linfonodomegalias ao exame físico e à ultrassonografia de partes moles da região cervical. Foram considerados os diagnósticos diferenciais de melanoma maligno nodular e carcinoma basocelular pigmentado. A paciente foi submetida à biópsia incisional e posterior exérese completa da lesão, com margem de segurança de 4mm. A análise histopatológica evidenciou neoplasia epitelial maligna, conectada à epiderme, composta por queratinócitos pleomórficos, com queratinização individual e em grupo, assumindo arranjo exofítico, com invasão da derme superficial (Figura 2). As células neoplásicas apresentavam algumas figuras de mitose de aspecto atípico. Dispersa entre os queratinócitos neoplásicos, havia uma segunda população de células, constituída por melanócitos dendríticos típi-cos. Foi observado pigmento de coloração acastanhada nos melanócitos, nos queratinócitos e em histióci-tos localizados na derme subjacente, que se corou positivamente pelo método de Fontana-Masson, tratando-se, portanto, de melanina (Figura 3). A reação imuno-histoquímica com anticorpos para citoqueratinas de alto e baixo peso molecular foi fortemente positiva em todas as células neoplásicas e negativa nos melanócitos. As reações imuno-histoquímicas para proteína S-100 e HMB 45 foram positivas somente nos melanócitos. A paciente continua em acompanhamento clínico, sem evidências de recorrência local ou metástases após seis meses de evolução.
