Introdução: A endometriose é um distúrbio ginecológico caracterizado pela presença de glândula e/ou estroma endometrial fora da cavidade uterina e acomete mulheres em idade reprodutiva. Seu diagnóstico é feito por meio da laparoscopia e complementado pela anamnese e outros exames. Objetivo: Compreender a fisiopatologia e o diagnóstico da endometriose. Metodologia: Foi realizado uma revisão de literatura, por meio da busca nas plataformas Pubmed e Google Acadêmico, a partir dos descritores em inglês e em português endometriosis, diagnosis, infertility and pathophysiology, sendo selecionados 5 artigos. Resultado: A fisiopatologia da endometriose ainda é tema de discussão e apresenta teorias baseadas em evidências clínicas e experimentais, entre elas três serão discutidas. A primeira, chamada teoria da metaplasia celômica, aborda que da mesma superfície originam as células endometriais e as peritoneais. A metaplasia deveria aumentar com a idade, porém a endometriose está em maior parte relacionada à idade fértil. A segunda teoria, chamada teoria da indução diz sobre um derramamento de substâncias desconhecidas do útero e isso iria induzir à formação de tecido endometrial a partir de células mesenquimais indiferenciadas. Enfim, a teoria da menstruação retrógrada propõe que ocorra regurgitação transtubária da menstruação e as células endometriais disseminam e se implantam em diferentes locais, assim alterações anatômicas da pelve promovem o fluxo retrógrado e aumentam a chance de endometriose. O diagnóstico da endometriose é realizado a partir da anamnese e observa se a paciente relata dor pélvica, dispareunia, dismenorreia, disquezia, disúria e infertilidade. Dado que esses sintomas são usuais da endometriose, após deve ser feito o exame físico da pelve e caso haja mobilização uterina dolorosa deve-se suspeitar da doença. O exame padrão ouro para o diagnóstico definitivo é biópsia, realizada por laparoscopia, este exame é feito por do tecido lesionado e, posteriormente, sua análise possuindo como finalidade a determinação da endometriose. Ademais, são utilizados exames complementares tais como biomarcadores e de imagem. Conclusão: Assim, nota-se que a endometriose é uma doença muito prevalente no mundo, sendo que sua fisiopatologia ainda está em discussão com diversas teorias e o seu difícil diagnóstico representa um importante problema na saúde da mulher.
