Publié par l’Association américaine de psychiatrie (APA), le Manuel diagnostique et statistique des troubles mentaux , mieux connu sous l’abréviation « DSM », s’impose comme l’instrument par excellence, tant dans le monde de la recherche scientifique que dans celui des pratiques cliniques. Depuis la première version de 1952 jusqu’à nos jours, le manuel a subi des révisions successives. La publication récente sur un site internet officiel du projet de la future nouvelle version (DSM-V), annoncée pour 2013, constitue un événement dit « historique » parce qu’il rendrait enfin transparent le processus de révision et s’ouvrirait ainsi à la critique du plus grand nombre, censée garantir la construction démocratique d’un manuel toujours plus rigoureux. Si la critique apparaît à la fois vive et polémiste dans le monde anglo-saxon, elle se montre beaucoup plus discrète dans le monde francophone alors que le manuel y est très utilisé et constitue indéniablement une référence incontournable. Le but de l’article est de poser les jalons d’une théorie critique du DSM, de ses conceptions et développements en vue d’un tant soit peu relativiser son caractère monopolistique.

Résumé Le propos de cet article consiste à présenter la naissance et le développement de la criminologie clinique à l’École de criminologie de l’Université de Louvain à Louvain-la-Neuve, depuis sa création en 1929. Plus précisément, il se centre sur l’émergence d’une criminologie clinique d’inspiration phénoménologique, pour en montrer les prolongements jusqu’à la fin du xxe siècle. Il développe certains aspects de la carrière et de l’oeuvre d’Étienne De Greeff (1898-1961). Il présente ensuite le travail clinique réalisé dans le cadre pénitentiaire par une équipe universitaire pluridisciplinaire. Il montre ensuite comment Christian Debuyst a repensé le sens de la clinique criminologique, compte tenu du développement des sciences sociales et des politiques de répression. Puis, comment, sous l’impulsion de Jean Kinable, l’apport de la psychanalyse a enrichi cet héritage. Enfin, il conclut sur l’importance, en criminologie, du maintien d’un mode de pensée dialectique qui vise une compréhension systématique et ouverte des phénomènes qui se donnent toujours à connaître dans l’intersubjectivité.

L e terme instrument, un emprunt du latin instrumentum, « mobilier, ameublement, matériel », désigne un objet fabriqué servant à exécuter un travail. Au sens figuré, il s'agit d'un acte juridique servant à établir un droit, une convention ; il est aussi employé pour désigner une chose ou une personne « qui sert à obtenir un résultat » et, par analogie, un « objet utilisé pour une fin déterminée » (Rey, 1994, p. 1035-1036). L'adjectif « instrumental » s'applique à ce qui touche aux moyens. Le verbe « instrumentaliser » enfin renvoie à l'idée de considérer quelqu'un ou quelque chose comme un instrument en vue d'obtenir un résultat.
