We studied the integrity of the rotator cuff in both dominant and non-dominant shoulders of 90 asymptomatic adults between the ages of 30 and 99 years using ultrasound. The criteria for diagnosis had been validated on unembalmed cadaver specimens. We found no statistically significant difference in the incidence of impingement findings between dominant and non-dominant arms or between genders. The prevalence of partial-or full-thickness tears increased markedly after 50 years of age: these were present in over 50% of dominant shoulders in the seventh decade and in 80% of subjects over 80 years of age. Our results indicate that rotator-cuff lesions are a natural correlate of ageing, and are often present with no clinical symptoms. In a prospective study of 295 male Israeli military recruits a 31% incidence of stress fractures was found. Eighty per cent of the fractures were in the tibial or femoral shaft, while only 8% occurred in the tarsus and metatarsus. Sixty-nine per cent of the femoral stress fractures were asymptomatic, but only 8% of those in the tibia. Even asymptomatic stress fractures do, however, need to be treated. Possible explanations for the unusually high incidence of stress fractures in this study are discussed.
In a prospective study, quantitative measures of the structure of the longitudinal arch of the foot were established and related to the incidence of stress fractures in the bones of the lower limbs of military recruits. In addition, the role of a semirigid orthotic device (Langer military stress orthotic) in preventing stress fractures was evaluated as a function of the structure of the longitudinal arch. Femoral and tibial stress fractures were found to be more prevalent in the presence of feet with high arches, whereas the incidence of metatarsal fractures was higher in feet with low arches. The use of an orthotic device reduced the incidence of femoral stress fractures only in the presence of feet with high arches and the incidence of metatarsal fractures only among feet with low arches. The findings suggest that the normal foot with a low arch acts as a better shock absorber than the normal foot with a high arch, and that an orthotic device may improve the shock absorbing capacity of the arch.
Full-thickness rotator cuff tears tend to increase in size in about half of patients aged 60 years or younger. Surgery should be initially considered in these patients to prevent a probable increase in size tear. Patients treated nonoperatively should be routinely monitored for tear size increase, especially if they remain symptomatic.
The prediction of patient-specific proximal femur mechanical response to various load conditions is of major clinical importance in orthopaedics. This paper presents a novel, empirically validated high-order finite element method (FEM) for simulating the bone response to loads. A model of the bone geometry was constructed from a quantitative computerized tomography (QCT) scan using smooth surfaces for both the cortical and trabecular regions. Inhomogeneous isotropic elastic properties were assigned to the finite element model using distinct continuous spatial fields for each region. The Young's modulus was represented as a continuous function computed by a least mean squares method. p-FEMs were used to bound the simulation numerical error and to quantify the modeling assumptions. We validated the FE results with in-vitro experiments on a fresh-frozen femur loaded by a quasi-static force of up to 1500 N at four different angles. We measured the vertical displacement and strains at various locations and investigated the sensitivity of the simulation. Good agreement was found for the displacements, and a fair agreement found in the measured strain in some of the locations. The presented study is a first step toward a reliable p-FEM simulation of human femurs based on QCT data for clinical computer aided decision making.
