El angiomiolipoma cútaneo es la variante de partes blandas de los PEComas o tumores derivados de células epiteliodes perivasculares. Compuesto por múltiples variantes, la más conocida el angiomiolipoma renal (AML), comparten un mismo patrón histológico. Pueden presentarse en múltiples localizaciones como el tracto gastrointestinal, genitourinario o en los tejidos blandos. Son neoplasias histológicamente caracterizadas por abundantes vasos de pared fina acompañados de células perivasculares redondeadas y con citoplasma claro. A nivel inmunohistoquímico, coexpresan marcadores melanogénicos (HMB45, Melan-A o tirosina) junto con marcadores musculares (SMA, actina, miosina, calponina y h-caldesmon).. Sin embargo, su positividad errática y no patognomónica unida a la baja prevalencia, dificulta la identificación y la consideración dentro del diagnóstico diferencial. En este artículo se presenta un caso de angiomiolipoma cutáneo a nivel del pie derecho junto con un repaso breve de este tipo de tumores.
Las úlceras neuroisquémicas son las lesiones más predominantes en el pie diabético en las que tanto la neuropatía como la isquemia contribuyen a esta manifestación clínica. Es una enfermedad de elevada prevalencia y morbilidad en nuestro medio, especialmente en el pie diabético. En este artículo se expone el caso de un paciente con una amputación del tercer dedo del pie izquierdo no resuelta tras un año de evolución. El paciente recibió una propuesta de amputación transmetatarsiana como solución para su problema. Él rechazó esa posibilidad y decidió buscar una segunda opinión. La colaboración de distintos profesionales especialistas en cirugía vascular, cirugía plástica, enfermería y podología, unido a técnicas novedosas como la terapia de presión negativa o curas con factores de crecimiento, permitieron la preparación del lecho para una cobertura exitosa, evitando una amputación transmetatarsiana. Las técnicas quirúrgicas que se realizaron fueron una amputación parcial de la falange distal del primer dedo (syme) como tratamiento de la osteomielitis que sufría en la falange distal y un fillet flap en el segundo dedo que cubrió el defecto de la lesión producida por la amputación, y posterior evolución tórpida del tercer dedo. Este tipo de técnicas pueden ser muy resolutivas si se aplican con una indicación correcta y pueden evitar tratamientos más agresivos que pueden condicionar la calidad de vida del paciente.

Introducción: Las toxicodermias de origen medicamentoso constituye un problema dermatológico común que puede producir un gran impacto en el paciente que lo padece, ya que afecta a la apariencia y calidad de vida. En el año 2025 se calcula que habrá en el mundo 300 millones de diabéticos. En nuestro país se ha demostrado un aumento del consumo de antidiabéticos orales del 56,1% desde el año 2.000 al 2.014. Existen muy pocos estudios sobre toxicodermias inducidas por hipoglucemiantes orales. Materiales y métodos (o Historia Clínica): Presentamos dos casos de toxicodermia producidos por antidiabéticos orales: un caso de una erupción inducida por metformina en un paciente de 51 años sin antecedentes personales de alergias y un caso de balanopostitis en un paciente de 70 años asociada al uso de canaglifocina. Discusión: Los antidiabéticos orales se dividen en tres grandes grupos que son los clásicos (biguanidas, sulfonilureas, meglitinidas, tiazolidindionas y los inhibidores de la alfaglucosidasa), los de efecto incretina (inhibidores de la DPP-4 y agonistas de los receptores GLP-1) y los inhibidores del cotransportador sodio-glucosa tipo 20 (SGLT2). Cabe destacar que, en España, la metformina es el hipoglucemiante oral más usado en términos absolutos y el más consumido. Hemos realizado una puesta al día de las diferentes toxicodermias asociadas al uso de estos fármacos cuya incidencia es baja en comparación con el alto consumo de estos. En general, las toxicodermias descritas asociadas al uso de antidiabéticos orales son muy poco frecuentes, de presentación clínica variable, y no suelen ser de carácter grave. Acerca de los diferentes grupos, la metformina, como es de esperar por ser el más utilizado, es el fármaco que más toxicodermias documenta, aunque siguen siendo raras. Las reacciones cutáneas por sulfonilureas descritas se deben fundamentalmente a la primera generación de sulfonilureas siendo las lesiones maculopapulosas las más frecuentes. Los inhibidores de la DPP-4 son el grupo farmacológico que mayor número de incidencias publicadas presenta y las más graves, siendo enfermedades ampollosas autoinmunes en su mayoría. En cuanto a los inhibidores de SGLT2, actualmente existe una alerta por el posible riesgo de aparición de gangrena de Fournier asociada a su uso. El manejo de estos pacientes requiere la retirada del fármaco sospechoso y tratamiento de los síntomas. Conclusión: (1) Es importante reconocer este tipo de reacciones cutáneas para un correcto manejo del paciente. (2) El consumo de antidiabéticos orales (AO) ha sufrido un aumento considerable en los últimos años a favor de la metformina y los inhibidores de la DPP-4. (3) Los inhibidores de la DPP-4 son el grupo farmacológico que más incidencias ha registrado, siendo las enfermedades ampollosas autoinmunes la mayoría de los casos.
