Vest ibular disorders are common among the elderly, mainly resulting in dizziness and imbalance -symptoms which can impact daily routine activities. Aim:To study the correlation between body balance and functional capacity and a comparison of risk of falls, actual falls and the functional capacity of the elderly with chronic vestibular dysfunctions. Materials and Methods:A cross-sectional, clinical and experimental study with 50 senior citizens -60 to 86 years, with chronic peripheral vestibular dysfunction. These participants underwent body balance assessment by the Dynamic Gait Index (DGI) and functional capacity assessment by the Functional Independence Measure (FIM). The data was tested using the Spearman correlation and comparison tests, Mann-Whitney and Kruskal-Wallis, being α=5% (0.05).Results: There was a significant correlation between the total DGI score and all FIM scores, especially the total score (r=0.447; p<0.001) and loss of functional capacity in elderly patients with the highest risk of falling (p<0.001). Conclusion:There is a positive correlation between body balance and functional capacity in elderly patients with peripheral vestibular disorders, that is: the better the balance, the better the individual's functional capacity. RESUMOOs exercícios físicos têm sido associados a importantes e variados benefícios à saúde, como aqueles relacionados a função imune específica e não-específica, destacando-se, nesta última, o processo inflamató-rio. Contudo, dependendo do tipo, intensidade, freqüência e duração, os exercícios também podem causar certos prejuízos ao organismo. De fato, estudo prévio mostrou que a hipernocicepção de origem inflamatória, em ratos, foi influenciada pelo protocolo de exercícios físicos realizados em esteira ergométrica. Assim, este trabalho teve como objetivo estudar os efeitos dos exercícios físicos de baixa e alta intensidade sobre a resposta inflamatória aguda. Para isso, foram utilizados ratos machos, adultos, da linhagem Wistar, os quais foram submetidos (grupo treinado) ou não (grupo não treinado) a exercícios em esteira ergométrica. A inflamação aguda foi induzida pela injeção de carragenina-0,5% no coxim da pata posterior esquerda dos ratos, sendo o volume de edema inflamatório agudo mensurado por meio de pletismografia, antes e após 1, 2, 3, 4, 6, 8 e 24 horas da indução do processo inflamatório. A análise estatística dos resultados mostrou aumento significante no volume de edema inflamatório nos momentos H 1 , H 2 e H 3 (P<0,01) e nos momentos H 4 e H 6 (P<0,05) nos animais treinados em baixa intensidade. Entretanto, não ocorreram alterações estatisticamente significantes no volume de edema inflamatório agudo em nenhum dos momentos avaliados (P>0,05) nos animais do grupo treinado em alta intensidade em relação aos não treinados. Concluiu-se, então, que os exercícios físicos de baixa intensidade, em esteira ergométrica, aumentaram o volume de edema inflamatório agudo em ratos, provavelmente ocasionado pelo aumento na síntese e secreção de prostaglandinas e/ou aumento nos níveis plasmáticos das citocinas IL-1, IL-6 e TNF-alfa entre outros fatores. Tal fato não foi observado com os exercícios de alta intensidade, mostrando assim, a influência da intensidade, freqüência e duração dos exercícios sobre este parâmetro inflamatório.Palavras-chave: exercícios físicos, esteira ergométrica, inflamação aguda, edema, ratos. Physical exercise practice is an important habit to maintain the physical and emotional well-being of those who regularly adopt it, and can bring great benefits to health. However, depending on the type, intensity, frequency and duration of the exercises, they can also do a certain degree of harm to the organism. As a benefit, the role of physical exercise is described in the specific and non-specific immune function and, in the latter, the inflammatory process stands out. Therefore, the purpose of this study was to show the effects of low and high intensity physical exercises on hypernociception, and to evaluate the body mass and the relative weight of some organs in rats. For the study, male adult Wistar rats were used and submitted (trained group) or not submitted (non-trained group) to physical exercises on an ergometric treadmill. The acute inflammation was induced by an injection of carrageenin-0.5% into the plantar tissue of the left hind paw of each rat and the nociception was measured by the plantar test before and after 1, 2, 3, 4, 6, 8 and 24 hours. At the end of the evaluations, the rats were submitted to deep anesthesia until their euthanasia, to collect and weigh the adrenal glands, heart, spleen and kidneys, and to subsequent histopathological study of those tissues. The statistical analysis of the results showed a significant increase (P < 0.05) of the hypernoniception threshold in H 2 , H 3 , H 4 and H 6 moments in animals trained at high intensity. There was also a significant reduction (P < 0.05) in body mass besides hypertrophy of the adrenal glands and heart and an increase in the kidneys relative weight of the rats trained at high intensity, in addition to adrenal hypertrophy of the animals trained at low intensity. There were no statistically significant changes in the other parameters evaluated in this study. We concluded that physical exercises on a treadmill were capable of changing the nociception, body mass and the relative weight of some organs, but in a way that was dependent on the exercises protocol applied to the animals.
ResumoAs doenças vestibulares são comuns na população idosa e, associadas a várias comorbidades, aumentam o uso de medicamentos e o risco de reações adversas. O objetivo foi investigar os excipientes farmacêuticos contidos em medicamentos utilizados por idosos com vestibulopatias, para identificar aqueles com maior risco de causar reações alérgicas. Foram utilizadas informações de 104 idosos da comunidade, com queixas vestibulares e em uso de medicamentos; os excipientes farmacêuticos de todos os medicamentos foram identificados e separados os mais frequentes, dentre os quais, foram destacados aqueles com potencial para induzir reações alérgicas. Os resultados obtidos mostraram que as mulheres foram o gênero predominante (85%) e maior número de idosos está na faixa etária de 66 a 70 anos (30%). Os sintomas vestibulares mais relatados pelos idosos foram tontura, zumbido e perda auditiva, e 91% da amostra fazia uso de polifarmacoterapia. Os excipientes mais frequentemente encontrados nos medicamentos, incluindo os antivertiginosos mais usuais, foram a lactose (35%), os corantes (26%) e os conservantes (21%), entre os quais, os que causam maior risco de reações alérgicas: lactose, parabenos, amarelo crepúsculo, eritrosina, amarelo de tartrazina, benzoato de sódio e cloreto de benzalcônio. O cuidado nas prescrições e no uso de medicamentos pode contribuir para reduzir o risco de reações adversas a certos excipientes farmacêuticos, especialmente em grupos etários mais vulneráveis e polimedicados, como os idosos com vestibulopatias e naqueles mais predispostos às reações alérgicas e intolerâncias.Palavras-chave: Excipientes Farmacêuticos. AbstractVestibular disorders are common in the elderly people and, associated with multiple comorbidities, increase the use of medicines and the risk of adverse reactions. The aim was investigate the pharmaceutical excipients contained in medicines used by elderly patients with vestibular disorders to identify those with greater risk for developing allergic reactions. It were used information of 104 elderly patients with vestibular complaints and using medications. The pharmaceutical excipients of all medicines were identified and the most frequents were separated, between them those with potential to cause allergic reactions. The results obtained showed that women were the predominant gender (85%) and a greater number of elderly people aged 66-70 years (30%). Vestibular symptoms most frequently reported by the elderly were dizziness, tinnitus and hearing loss, and 91% of the sample were using polypharmacy. The most frequently excipients found in the medicines were lactose (35%), dyes (26%) and preservatives (21%), among which, those causing greater risk of allergic reactions were lactose, parabens, sunset yellow, erythrosine, yellow tartrazine, sodium benzoate and benzalkonium chloride. This study aims to verify the effects of Senna occidentalis (So) on hematological, inflammatory, and immunological responses in young rats as their immune system may be more sensitive to toxicity than that of the adult. So seeds used in the experiments were obtained from the Research Centre for Veterinary Toxicology-CEPTOX at Pirassununga, São Paulo, Brazil. Thirty male Wistar rats, all 21 days old, were divided into groups: So4 was fed a ration containing 4% So seeds; a control group received commercial ration; and a peer-fed (PF) group received the same amount of ration as consumed by the So4 group but with no So. Food consumption and weight gains were evaluated for 28 consecutive days. The rats of So4 group showed decreases in food consumption, weight gains, and weight of the thymus and an increase in spleen weight compared to controls. The So4 rats had microcytic and hypochromic anemia characterized by a reduction in mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), and mean corpuscular hemoglobin concentration (MCHC). The immune system analysis revealed that the So4 animals had a decrease in percentage of phagocytic neutrophils. The results showed that S. occidentalis seeds lead to injury to both the lymphoid organs and the hematopoietic system.
<p>A polifarmacoterapia para tratamento de múltiplas doenças é uma realidade na população idosa. O objetivo deste trabalho foi avaliar as consequências do uso de múltiplos fármacos em idosa com vestibulopatia. O estudo foi previamente aprovado pelo Comitê de Ética em Pesquisa (Protocolo 475.835/2013) e realizado com base em informações de idosa de 64 anos, com labirintopatia vascular e queixas de tontura, zumbido e desequilíbrio, além de relato de queda. A idosa é hipertensa, apresenta insuficiência arterial e venosa crônica, aterosclerose de artérias coronárias e hérnia discal lombar. Passou por cateterismo e angioplastia. Faz uso contínuo e concomitante de nove medicamentos, cada um contendo apenas um fármaco, a saber: Captopril, Propranolol, Hidroclorotiazida, Ácido acetilsalicílico, Pravastatina, Bezafibrato, Meloxicam, Omeprazol e Hidróxido de alumínio. Os fármacos foram classificados pela Anatomical Therapeutic Chemical (ATC) e as interações analisadas pelo The Medical Letter Drug Interactions Program. Todos os fármacos fizeram algum tipo de interação, com exceção do Bezafibrato, que não pôde ser analisado. As principais consequências das diferentes interações foram: diminuição do efeito anti-hipertensivo e diurético (em diferentes interações); redução do efeito hipotensor (em diferentes interações); bloqueio do efeito terapêutico do Ácido acetilsalicílico sobre a mortalidade após infarto do miocárdio; piora da insuficiência cardíaca congestiva; arritmias cardíacas; aumento do risco de insuficiência renal; indução e agravamento da insuficiência renal (provável efeito aditivo); hiponatremia; hipotensão; maior risco de toxicidade gastrintestinal; possível toxicidade do Ácido acetilsalicílico; aumento do efeito hiperglicêmico da Hidroclorotiazida; redução da absorção e do efeito do Captopril, Propranolol e Ácido acetilsalicílico. A polifarmacoterapia utilizada resultou em diferentes interações medicamentosas, cujas consequências farmacológicas podem ter repercussões clínicas importantes para a saúde desta idosa e de outros idosos que utilizam as mesmas medicações, sobretudo os que apresentam vestibulopatias.</p>
