Leydig cells are coupled in vivo by numerous gap junctions. In vivo and in vitro cells were immunolabeled by connexin 43 (Cx43) but not by Cx26 or Cx32 antibodies; immunoblotting confirmed specificity of Cx43 labeling. Pairs of Leydig cells dissociated from mouse testis were studied by dual whole cell voltage clamp, and a high incidence of dye (n = 20) and electrical coupling (n = 60; > 90%) was found. Coupling coefficients were near 1 and junctional conductance (gj) averaged 7.2 +/- 1.2 nS (SE, n = 40). Large transjunctional voltage (Vj) decreased gj; currents decayed exponentially with time constants of seconds that decreased at greater Vj. The residual conductance at large Vj was at least approximately 40% of the initial conductance. Exposure of cell pairs to saline solutions saturated with CO2 (n = 15) or containing 2 mM halothane (n = 15) or 3.5 mM heptanol (n = 15) rapidly and reversibly reduced gj. In eight cell pairs, gating of single junctional channels was observed during halothane-induced reduction in gj. Most gating events at Vj < 40 mV were fit by a Gaussian distribution with a mean of approximately 100 pS. With Vj > 40 mV, smaller transitions of approximately 30 pS were also recorded, and the frequency and duration of the approximately 100-pS transitions decreased. Also, approximately 70-pS transitions between 30- and 100-pS conductances were observed in the absence of 70-pS transitions to or from the baseline, indicating that the 30-pS conductance was a substate induced by large Vj.
Resumo O objetivo deste artigo é compreender o significado da experiência vivenciada por crianças institucionalizadas em abrigo, por meio do brinquedo terapêutico. Estudo descritivo, de abordagem qualitativa, realizada em uma instituição de abrigo não governamental, filantrópica, situada no município de Santos, São Paulo, Brasil. Para a amostra, selecionaram-se cinco crianças de quatro a 11 anos de idade, que concordaram em participar da pesquisa, com autorização do seu responsável legal. A observação durante uma sessão de brinquedo terapêutico dramático foi a estratégia usada para a coleta dos dados, que foram analisados por meio da técnica de análise de conteúdo de Bardin. Seis categorias emergiram dos dados, evidenciando aspectos do cotidiano da criança no abrigo e na escola, bem como a falta que sente de sua família e a maneira como lida com o medo no abrigo. Ressalta-se, ainda, a tentativa constante de obter a aprovação do adulto e o prazer que sente em brincar com ele. O brinquedo terapêutico possibilitou à criança refletir sobre suas vivências, além de momentos de catarse. Para o adulto, foi possível compreender como é para ela viver em um abrigo e os desafios que tem que enfrentar neste contexto.
Model for the systematic implementation of therapeutic play in pediatric hospital units a Modelo de implementação sistemática do brinquedo terapêutico em unidades pediátricas hospitalaresModelo de implementación sistemática del juego terapéutico en unidades hospitalarias de pediatria
Introduction: The benefits of the application of therapeutic play (TP) in pediatrics are widely reported in the literature. Evidence supports its use in the care of hospitalized children with a positive impact on reducing anxiety and fear, not only for the child, but for her family members. This study sought to investigate the perception of professionals from a reference group for the application of TP (BrinquEinstein) in relation to its implementation in a pediatric hospital unit. Objectives: To understand how professionals who belong to the BrinquEinstein group evaluate the process of systematic implementation of TP in pediatric hospital units; outlining the facilitating factors and barriers for this implementation to occur from the perspective of these professionals. Method: Exploratory study, with a qualitative approach, developed in the pediatric and intensive care units of a general hospital, extraport, in the city of São Paulo. Professionals from different categories who work in these units and who belong to BrinquEinstein participated in the sample. Data were collected through individual semi-structured and audio-recorded interviews, being analyzed using the Inductive Thematic Analysis proposed by Braun & Clark. Results: From the analysis of the interviews, two themes emerged that represent the perspective of BrinquEinstein members about the TP implementation process: “experiencing a transforming process” and “establishing future perspectives”. Conclusion: For the professionals interviewed, it is essential that the use of TP becomes a routine practice in the different contexts of child health care. Managers and institutions play a fundamental role in the implementation of this practice, in the identification of needs and in the search for solutions to make it a reality.
Resumo Existe consenso na literatura sobre a importância do uso do brinquedo terapêutico (BT), recomendando-se a sua sistematização no planejamento do cuidado pediátrico, contudo há lacunas no conhecimento sobre como sistematizar esse cuidado e implementa-lo em unidades hospitalares. Objetivos Propor um modelo de implementação sistemática do BT para unidades pediátricas hospitalares e descrever as etapas desse processo. Método estudo descritivo, de abordagem qualitativa, desenvolvido em unidades de internação e terapia intensiva pediátricas, apoiado pela ferramenta PDCA (Plan, Do, Check e Action). Os dados foram coletados por meio da observação da dinâmica dos atendimentos da unidade e entrevista com onze dos profissionais do grupo de referência de BT e, a seguir procedeu-se a análise temática. Resultados a implementação do BT evidenciou resultados positivos, seja na perspectiva dos integrantes do grupo de referência, seja na percepção de aumento da frequência na prática de realização do BT ou, ainda, pelo reconhecimento da família e da instituição. Conclusão e implicações para a prática As etapas percorridas no processo de implementação do BT em unidades pediátricas fornecem subsídios para direcionar profissionais de diferentes instituições a implementar de forma sistemática esta prática lúdica.
