INTRODUÇÃO: Nos pacientes portadores de doença renal crônica (DRC) submetidos à hemodiálise (HD), o sedentarismo compromete a capacidade funcional e se associa a aumento da mortalidade por doenças cardiovasculares. Apesar da demonstração dos benefícios da prática de exercícios físicos para estes pacientes, poucos centros de diálise desenvolvem esta atividade. O objetivo deste estudo foi descrever a experiência de cinco anos de um programa de exercício aeróbico intradialítico, com ênfase na adesão e na segurança do procedimento. MÉTODOS: Foram incluídos pacientes adultos, de ambos os sexos, que participaram de um programa de exercícios realizado no período de agosto de 2004 a agosto de 2009. O exercício foi realizado nas duas horas iniciais da HD, empregando-se um cicloergômetro horizontal, sendo constituído de aquecimento, condicionamento e resfriamento. Foram realizadas análises descritivas dos dados coletados ao longo dos cinco anos do programa. RESULTADOS: No período de cinco anos, 34 pacientes (48,26 ± 11,44 anos; 20 mulheres) participaram do programa de exercício. Foram realizadas 3.077 sessões individuais de exercício aeróbico durante as sessões de HD. O tempo médio de participação no programa foi de 15,65 ± 13,86 meses e a aderência às sessões de exercício foi de 64,87 ± 19,47%. Apesar do registro de alguns episódios de hipotensão, sem repercussões hemodinâmicas significativas, não foram observadas complicações clínicas importantes. CONCLUSÃO: A realização de exercício aeróbico durante a sessão de HD é uma prática bem aceita pelos pacientes, segura e não acompanhada de complicações graves.
End-stage renal disease (ESRD) is associated with several hemodynamic and peripheral muscle abnormalities which could slow Rationale: the kinetics of O uptake (VO ) at the on-exercise transient. Although previous studies have shown that aerobic training is able to 2 2 accelerate VO kinetics in several disease populations, little is known if these positive findings are also applicable to hemodialysis patients.2 We therefore investigated the effects of supervised aerobic training on the VO kinetics at the onset of dynamic exercise in hemodialysis 2 patients.Twelve patients and 12 age-and gender-matched non-trained patients underwent sub-and supra-anaerobic threshold (AT) Methods: exercise tests on a cycle ergometer. Exercise training was performed during the first 2 hours of hemodialysis sessions by using a stationary cycle, three times weekly, for 12 weeks.We found that training significantly accelerated VO kinetics (tau decreased from 62.45 ± 19.61s to 44.96 ± 12.6s and MRT Results: 2 decreased from 77.45 ± 23.78s to 63.13 ± 16.31s) and reduced O deficit at the sub-AT bout (p<0.05). In contrast, VO kinetics were 2 2 actually slower at the final evaluation in the non-trained patients (p<0.05). Similar beneficial findings of training were observed at the supra-AT exercise bout (tau decreased from 52.92 ± 17.39s to 40.4 ± 13.82s and MRT decreased from 69.3 ± 21.13s to 58.4 ± 14.8s, p<0.05), including a significant increase in time to exercise intolerance from 311.92 ± 223.43s to 589.17 ± 358.12s.In conclusion, a 12-weeks supervised aerobic training program was highly effective in accelerating on-exercise VO kinetics Conclusions: 2 at both moderate and severe exercise intensity domains in ESRD patients undergoing hemodialysis. These beneficial effects might be mechanistically linked to increased tolerance to dynamic exercise after training in these patients.

Aproveitamos também para lançar a I Conferência Nacional de Fisioterapia do ICHC FMUSP em comemoração ao 20° aniversário da Divisão de Fisioterapia do ICHC.Este evento científico possibilitou rever ou fazer novas amizades, ampliar nosso network e compartilhar experiências, projetos e vasto conhecimento.Com o intuito de integrar as diferentes áreas da fisioterapia, organizamos a participação de renomados palestrantes nacionais e internacionais que compartilharam suas experiências abordando questões inovadoras nas práticas profissionais, buscando a aproximação e participação ativa dos congressistas. Além disso, foram desenvolvidas diversas discussões e reflexões sobre visão de futuro, metas e estratégias para o desenvolvimento sócio-político contínuo da fisioterapia.Os trabalhos científicos foram analisados e aprovados pelo Comitê Científico e foram organizados em eixos temáticos da fisioterapia: Fisioterapia Cardiorrespiratória; Funcionalidade e Neurociência; Fisioterapia em Ortopedia, Traumatologia e Esporte; Fisioterapia nas Disfunções do Assoalho Pélvico; Fisioterapeuta Gestor e Outros.Gostaríamos de agradecer a todos aqueles que vêm contribuindo para a evolução da fisioterapia e esperamos que o material apresentado nesta edição seja base para conhecimento e disseminação de boas práticas e inovação.Até a próxima!
