Perfil de causa mortis em idosos internados em um serviço público de urgência e emergência: evidências clínicasProfile of causa mortis in elderly hospitalized in an emergency and emergency public service: clinical evidence Resumo O artigo buscou determinar a prevalência das causas que provocaram mortalidade em pacientes idosos internados em um serviço público de urgência e emergência. Realizou-se pesquisa por sexo, idade e causa de óbito no período de janeiro a junho de 2017. As causas de mortalidade foram classificadas segundo a Classificação Internacional de Doenças-10, sendo identificadas por meio de observação direta da ocorrência do óbito e dos registros do serviço de saúde. Foram registradas 143 ocorrências de óbito durante o estudo, com a maior proporção entre as mulheres, sendo a principal causa de mortalidade o choque séptico, seguido por outras septicemias. Os resultados demonstraram um predomínio das mortes por complicações das doenças infecciosas e podem contribuir no estabelecimento de estratégias de tratamento e promoção de saúde em populações geriátricas. We conducted a survey by sex, age and cause of death from January to June 2017. The causes of mortality were classified according to the International Classification of Diseases -10, and were identified through direct observation of the occurrence of death and of the records of the health service. There were 143 deaths during the study, with the highest proportion among women, with septic shock being the main cause of death, followed by other septicemias. The results showed a predominance of deaths due to complications of infectious diseases and may contribute to the establishment of treatment strategies and health promotion in geriatric populations. Introdução: O tumor borderline de ovário (TBO) é um tumor de origem epitelial que representa 10‒15% dos tumores ovarianos, apresenta crescimento lento e possui potencial de malignidade. O diagnóstico é histológico após cirurgia. Distinguem o TBO: atipia nuclear, estratificação epitelial, formação de projeções papilares microscópicas, pleomorfismo celular e atividade mitótica. Diferencia-se do carcinoma invasor pela ausência de invasão estromal. O marcador tumoral CA-125 é comumente negativo em estágios iniciais. O TBO na gestação é raro, com incidência significativa de estágio avançado, tumor mucinoso e bilateralidade. Objetivo: Relatar o caso de gestante portadora de massa ovariana e ascite volumosa com diagnóstico de tumor borderline. Material e Métodos: Os dados foram obtidos por meio de revisão de prontuário e de literatura específica disponível no PubMed utilizando as palavras-chave “borderline ovarian tumors” e “pregnancy”. Resultados e Conclusão: Paciente de 20 anos, primigesta, com idade gestacional de 8 semanas, encaminhada ao nosso serviço após ultrassonografia de primeiro trimestre evidenciando ovário esquerdo aderido à parede uterina posterior contendo volumosa formação cística com componente sólido medindo 12,7 × 9,5×8,4 cm e grande quantidade de líquido ascítico. Apresentando elevação isolada de CA-125 (372,8 UI/mL). Clínica de aumento do volume abdominal há dois meses, sem sinais de perda ponderal. Realizada paracentese com análise citológica de líquido citrino com ausência de malignidade. Paciente submetida à laparotomia exploradora com 13 semanas e 3 dias, sendo identificado, no ato operatório: ascite volumosa citrina (3 L), útero aumentado compatível com idade gestacional e massa retrouterina de origem ovariana esquerda com 15 cm, exofítica, sem cápsula, fragmentando-se mediante manipulação, aderida ao útero e a anexo direito. Mediante encontrado na cirurgia, optou-se por cirurgia citorredutora com exérese macroscópica total da lesão com ooforectomia esquerda, anexectomia direita (devido aderências) e preservação do útero. Procedimento sem intercorrências e ultrassonografia realizada após este, com visualização de batimentos cardíacos fetais. Laudo histopatológico evidenciou tumor seroso borderline micropapilar de ovário esquerdo, sendo ausentes cápsula e invasão estromal. Sem alterações em tecido ovariano direito. Gestação em curso em acompanhamento pré-natal. Proposto seguimento pelo CA-125 e exames de imagem, com proposta de reestadiamento durante cesariana (se houver indicação obstétrica) ou pós-natal em caso de parto vaginal. Os TBO diagnosticados na gestação exibem características agressivas, sendo o diagnóstico e o tratamento precoces essenciais. O tratamento é baseado no estadiamento intraoperatório e é possível realizar cirurgia conservadora com preservação de útero e anexo contralateral. O seguimento deve ser realizado com exames de imagem, dosagem de CA-125 e é recomendado planejar novo estadiamento cirúrgico.
Study question Does the analysis of the metabolites of the embryonic culture medium can predict the sex of the embryo? Summary answer The presence and quantity of some metabolites in the culture medium can predict the sex of the human embryos. What is known already Advances in analytical techniques for metabolomics have brought the possibility of better tools for the characterization of molecules. Embryonic metabolism can be used as a good indicator of viability, regardless of the morphology of the blastocysts, since differences were observed in the metabolic activities between the days of embryo development and in the rates of live births. Study design, size, duration 16 patients had their embryos biopsied between the months of January to July 2019 in a human reproduction laboratory. All cases had PGT-A indication and after the biopsy, the embryos were frozen. The culture medium samples were individually prepared for metabolites extraction according to the Bligh and Dyer protocol. Controlled ovarian stimulation and dose adjustments according to the response of each patient. The metabolomics analysis was performed by mass spectrometry. Participants/materials, setting, methods Follicular puncture were performed 35 hours after r-hCG. The eggs were kept in individual culture until the blastocyst stage. The blastocysts biopsy was performed (20). After the culture medium was sent to the 337 metabolites analysis by mass spectrometry. 14 molecules with the highest score on the PLS-Da was submitted to the ROC curves showing the power of metabolic analysis to predict the sex of euploid embryos. Besides, we performed the functional enrichment analysis. Main results and the role of chance After the genetic analysis by PGT-a, we obtain 20 euploid embryos, being 12 female embryos and 08 male embryos. Comparing the quantitative target metabolomic analysis of the 337 metabolites in the embryo culture medium, we observed the Asymmetric dimethylarginine, FAD, Malic Acid, Serotonin, increased in female embryos and Adenosine monophosphate, L-Alanine, L-Arginine, Cysteamine, DL-Dopa, Flavin Mononucleotide, Methionine sulfone, Nicotinic acid, L-Tyrosine, Uracil in male embryos. Through the ROC curve, we can verify AUC = 0.937. This result suggests that the metabolomic analysis of the culture medium is valid to be used as a complement of PGT-A to know embryo sex diagnostic. The functional enrichment analysis shows the Asymmetric dimethylarginine and Malic Sulfone metabolism as the principal function alter by female embryos.
