Einleitung "Risiko Softdrinks -Zähne heilen nicht von selbst" (Spiegel Online 2004) und "Vorsicht, Schmelzfresser" (Stern Online 2006) waren bereits vor einigen Jahren Überschriften in den populären Medien, mit denen das Thema Zahnerosionen eine gesteigerte gesellschaftliche Aufmerksamkeit erreichte. Fällt heutzutage die Diagnose Zahnerosion im Praxisalltag, ergeben sich oftmals beim Patienten, aber auch beim Behandlungsteam selbst Fragen über die Prävention oder Therapiemaßnahmen. Mittlerweile antwortet auch die Industrie mit neuartigen Zahnpflegeprodukten, die mit speziellen Formulierungen gegen Erosionen schützen sollen. "Draufputzen statt abnutzen" oder ähnliche Werbetexte versprechen dem Endverbraucher ein geeignetes Mittel gegen Erosionen. Doch wirken die neuen Produkte tatsächlich oder hört sich ihr Wirkmechanismus nur in der Werbung gut an? Definitionen Zum Verständnis seien zunächst verschiedene Zahnhartsubstanzdefekte gegenüberstellt, um Erosionen eindeutig von anderen Läsionen abzugrenzen und zu diagnostizieren. Erosionen gehören wie die Karies in die Gruppe der säurebedingten Zahnhartsubstanzverluste. Allerdings unterscheiden sich beide Erkrankungen hinsichtlich ihrer Entstehung, Morphologie und dem klinischen Erscheinungsbild. n Karies Karies wird durch säurebildende Bakterien verursacht und zeichnet sich initial durch einen Mineralverlust im tieferen Zahnhartgewebe aus. Gleichzeitig bleibt eine teilweise intakte Oberfläche erhalten. Die demineralisierte Zahnhartsubstanz kann unter gewissen Umstän-den wieder remineralisieren. n Erosionen Im Gegensatz zur Karies handelt es sich bei Erosionen um oberflächliche Substanzverluste, die ohne die Anwesenheit von Bakterien entstehen. Der Substanzverlust schreitet dabei von außen nach innen fort. Die verantwortlichen Säuren haben einen extrinsischen (z. B. durch saure Lebensmittel) oder einen endogenen (Magensäure) Ursprung. Eine Remineralisation der Zähne ist bei Erosionen nicht möglich, es kann jedoch zur Präzipitierung von Mineralien auf der Zahnoberfläche kommen.
