Background/Aims Facial trauma arising from traffic accidents is one of the most prevalent injuries in urban trauma centers. The aim of this study was to evaluate the frequency, types, and costs associated with osteosynthesis material used in facial fractures of traffic accident victims seen at the Hospital da Restauração, Recife/PE, Brazil, from January to December 2014. Materials and methods Data were collected from the medical records of hospitalized patients submitted to surgery under general anesthesia for the treatment of facial fractures during the period. The data were analyzed descriptively by calculating absolute frequencies, percentages, and means. Statistical analysis was performed using SPSS v.21.0. Results Among the patients, 73.1% were victims of motorcycle accidents that led to higher costs as they were the most prevalent injuries. Mandibular fractures were the most prevalent injury (53.5%). A total of 477 osteosynthesis materials were used, of which 76.51% were a 2.0‐mm plate system, 15.51% were a 1.5‐mm plate system, 3.14% were a 2.4‐mm plate system, 3.77% titanium meshes, 0.83% lag screws, and 0.2% steel wires. The use of these materials for the treatment of facial fractures resulted in a cost of US $51.285,00 for the Brazilian public health system. Conclusion The increase in the number of traffic accidents implies significant numbers of victims with facial fractures, as well as considerable costs associated with the use of osteosynthesis materials for public health services.
Introdução: Os osteomas são proliferações de tecido ósseo, neoplasia benigna clinicamente consiste em massa bem delimitada que pode ser séssil ou pediculada, assintomática e que dependendo da sua localização e tamanho, pode acarretar danos a estruturas adjacentes, alterações funcionais e estéticos dependendo da sua localização. Objetivo: Discutir um relato de caso de osteoma periosteal compacto em região mandibular. Relato de Caso: Paciente N.B, do sexo feminino, 33 anos, foi atendida no Hospital da face (Recife – PE) queixando-se de aumento de volume endurecido à palpação em região submandibular esquerda. Ao exame físico, apresentava uma lesão endurecida, séssil e indolor, com tempo de evolução de aproximadamente 1 ano. Após exame clínico, foi solicitada a tomografia de face para avaliação complementar. A imagem tomográfica evidenciou a presença de uma lesão hiperdensa em região de corpo mandibular esquerda. Mediante o aspecto da lesão, optou-se pela excisão cirúrgica através de acesso extra-oral submandibular. Após excisão, a peça foi encaminhada para estudo anátomo-patológico que identificou a lesão como um Osteoma compacto. Discussão: A anamnese e exame físico são de suma importância para o diagnóstico do osteoma, por se tratar de uma patologia assintomática. A conduta inicial do tratamento é o acompanhamento, e caso o osteoma acarrete transtornos funcionais ou estéticos, o tratamento eleito é a ressecção cirúrgica. Conclusão: O osteoma é uma neoplasia benigna, que em alguns casos, a intervenção cirúrgica é necessária devido ao comprometimento estético e funcional, como relatado no presente caso.
Introdução: Os tumores, neoplásicos ou não neoplásicos, que acometem a articulação temporomandibular (ATM) são incomuns e, clinicamente, os sintomas manifestam-se de forma semelhantes aos diversos transtornos que acometem a ATM, tais quais: dor, limitação de abertura bucal e alterações oclusais. Objetivo: avaliar os principais aspectos relacionados a etiopatogenia, diagnóstico e modalidades de tratamento dos principais tumores e pseudotumores que acometem a ATM, através de uma revisão narrativa da literatura atual. Metodologia: Foi realizada uma busca em fontes indexadas ao Google acadêmico e na Medline (via Pubmed), através dos seguintes descritores e meshes: Articulação temporomandibular; Transtornos da articulação temporomandibular; Neoplasias maxilares; Neoplasias ósseas e Deformidades dentofaciais. Resultados e discussão: Foram encontrados artigos sobre a etiopatogenia, modalidades diagnósticas e os aspectos relacionados ao tratamento dos principais tumores que acometem a ATM, tais quais o osteocondroma, a condromatose sinovial, tumor tenossinovial de células gigantes, os osteomas, condromas e condroblatomas, além das variedades malignas osteossarcoma e condrossarcoma. Considerações finais: os exames complementares são fundamentais para o diagnóstico diferencial dessas patologias, visto a inespecificidade dos sintomas normalmente associados, além de serem importantes para traçar o quadro evolutivo das lesões e para o planejamento cirúrgico que envolve a maior parte dos casos. Quando necessária, a ressecção cirúrgica desses tumores, muitas vezes está associada a reconstrução da articulação e a correção da deformidade dento-facial residual, promovendo uma melhor função articular, diminuindo os sintomas, em alguns casos melhorando a estética, e por fim devolvendo qualidade de vida para os pacientes acometidos.
IntroductionPseudoaneurysm is a vascular injury typically caused by rupture of arteries with extravasation of blood. The involvement of this entity in facial arteries after firearm aggression is extremely rare, and they need treatment as early as possible, thus avoiding irreversible damage to patients.Study designA 40-year-old male victim of gunshot attack with an entrance orifice in the right posterior cervical region with ascending trajectory, lodging in the ipsilateral zygomatic-orbitary complex, which was submitted to removal of the bone fragments and the bullet. In the intraoperative period, the patient developed profuse hemorrhage and, after complementary examinations, he was diagnosed with pseudoaneurysm of the internal maxillary artery, which was treated by selective endovascular embolization.ResultsThe patient was hemodynamically stable, with no complaints and was discharged after 48 hours, without postoperative bleeding recurrences. He had no more complications after 8 months of follow-up.ConclusionsThe main forms of treatment and diagnosis of vascular lesions are reviewed, and embolization is demonstrated as a technically safe procedure with few complications. Objetivo: O propósito do presente estudo foi discutir, através de uma revista da literatura, a etiologia, e os protocolos de tratamento mais indicados para peri-implantite. Métodos: a pesquisa foi realizada nas seguintes base de dados: Scielo e Medline via Pubmed. Como estratégia de busca, foram utilizados os seguintes descritores “Decs”: ‘implantes dentários’, ‘Peri-implantite’, ‘Periodontite’ e ‘Osseointegração’ no Scielo, bem como os meshes no Pubmed: ‘dental implants’, ‘peri-implantitis’ e ‘osseointegration’. Por fim, 24 estudos relacionados ao assunto foram selecionados. Resultados: Os implantes osseointegrados vêm sendo cada vez mais utilizados nas reabilitações orais. Apesar da alta taxa de sucesso e longevidade desse tratamento, alguns casos podem ser acometidos por doenças peri-implantares. A peri-implantite é uma doença infecciosa bacteriana que afeta tecidos moles e duros em torno do implante, podendo ocasionar a perda da osseointegração. Vários fatores são apontados como causadores da perda óssea peri-implantar. Por ser de caráter multifatorial, pode exacerbar a perda de altura óssea, dificultar a cicatrização após cirurgia, e causar a falha do implante. Considerações finais: Os métodos de tratamentos das doenças peri-implantares são os mais diversos possíveis, incluindo modalidades cirúrgicas e não cirúrgicas, além de terapias fotodinâmicas e regenerativas. Por ser uma doença relativamente nova, ainda não há consenso entre os profissionais sobre qual o melhor protocolo de tratamento. Faz-se necessária a realização de novos estudos que possam viabilizar a elaboração de protocolos clínicos para controle e tratamento da peri-implantite.
