INTRODUÇÃO: Benzodiazepínicos são medicamentos psicotrópicos de prescrição restrita utilizados para crises agudas de ansiedade, insônia e convulsões. Sua utilização é muitas vezes inadequada. O objetivo deste trabalho é identificar o perfil da usuária de benzodiazepínicos na atenção primária e as características de sua utilização. MÉTODO: Foram entrevistadas todas as pacientes maiores de 18 anos em espera para atendimento em uma unidade básica de saúde de Sorocaba (SP) em 2008. RESULTADOS: Foi encontrada utilização de benzodiazepínicos por 13,14% das frequentadoras da unidade básica de saúde, sendo maior em mulheres de 50 a 69 anos, em relacionamento estável e analfabetas. O uso encontrado foi principalmente por insônia (48,14%), de medicamentos de longa meia-vida, crônico (mais de 6 meses, 89,14%), com prescrição inicial principalmente pelo clínico geral (47,82%), com alta taxa de tentativas de interrupção (91,3%) infrutíferas (69,05%). DISCUSSÃO: A utilização é 3,3 vezes maior do que a descrita na literatura para mulheres de perfil semelhante, com prescrições em geral inadequadas, independentemente do prescritor inicial (especialista ou não). CONCLUSÃO: O uso é crônico em mulheres mais idosas, de menor escolaridade, de relacionamento estável, por motivos corretos, porém de tempo de tratamento inadequado.
Distúrbios do sono são comuns entre adultos e entre estudantes de Medicina e podem ter impacto nas atividades diárias profissionais ou acadêmicas. Modelo do estudo: Transversal. Objetivo e Método: O presente trabalho buscou avaliar a qualidade de sono de 157 estudantes de Medicina da Faculdade de Ciências Médicas e da Saúde da Pontifícia Universidade Católica de São Paulo, que se disponibilizaram após pedido nosso, sem maior filtragem. Para isso foram utilizados dois instrumentos, o Índice de Qualidade de Sono de Pittsburgh (PSQI) e a Escala de Sonolência de Epworth (ESE), ambos os questionários autoaplicáveis. Resultados: A média de duração de sono da amostra foi de 6,8 horas. Pobre qualidade de sono (PSQI > 5) foi encontrada em 44,59% dos indivíduos. A sonolência diurna excessiva mostrou-se presente em 36,3% dos avaliados. Não se mostrou diferença significativa dos escores do PSQI entre sexos, anos acadêmicos e grupos etários. A sonolência diurna foi maior dentro do gênero feminino. O uso de drogas hipnóticas foi relatado por 6,9% da amostra. Conclusão: A prevalência de distúrbios do sono encontrada entre estudantes de Medicina foi alta, sem importante predileção por grupo etário ou sexo.
The perioperative application and testing of the pacemaker was safe and could be performed in nearly all patients successfully. However, we do not consider a routine prophylactic transcutaneous placement in patients with chronic bifascicular or LBBB and additional first-degree A-V block justified. Nevertheless, appropriate drugs and temporary pacemaker equipment should be easily accessible.
SummaryThe B7-1/B7-2-CD28/CTLA-4 pathway is crucial in regulating T cell activation and tolerance. Autoantibodies to surface molecules on lymphocytes have already been described in various immune conditions, such as autoimmune diseases, infections and blood transfusions. The objective of this study was to test sera from healthy individuals and from patients for association of CD28 autoantibodies with inflammatory and non-inflammatory diseases. First, CD28 was obtained by digestion of CD28-Ig fusion protein with trypsin. The cleavage products were separated by sodium dodecyl sulphate-page gel electrophoresis. Additionally, a CD28/GST fusion protein was expressed in Escherichia coli and was used to establish an enzyme-linked immunosorbent assay for detection of autoantibodies against CD28. Sera from healthy individuals (n = 72) and patients with different inflammatory and noninflammatory skin diseases (n = 196) were tested for the presence of autoantibodies against CD28. Using mixed lymphocyte reaction (MLR), purified autoantibodies against CD28 were tested for their effects on CTLA-4-Iginduced T cell anergy. In this study, for the first time, we describe the existence of autoantibodies against CD28 in humans which are associated with atopic diseases, e.g. allergic rhinitis and asthma. These antibodies stimulate T cells and overcome the CTLA-4-Ig-induced anergy of T cells in an MLR. The existence of autoantibodies against CD28, which may have a T cell-stimulating function, has been shown. The data indicate that autoantibodies against CD28 could be a new immunological mechanism in allergic inflammation. Additionally, autoantibodies against CD28 could be an important new marker to discriminate between atopic diseases and other inflammatory skin diseases.
