Preeclampsia increases maternal and perinatal morbidity and mortality rates. Much research has been done to identify unique screening tests that would predict the risk of developing preeclampsia before the classic symptoms appear. The possible use of a screening test with high predictive accuracy in patients with high-risk or low-risk of preeclampsia remains to be investigated. At present, the search for additional tests continues. There is growing interest in the use of combinations of tests. Effective primary prevention is not possible because the causes are still unknown, but to identify and to modify susceptible risk factors might decrease the frequency of preeclampsia. A community guideline improves the screening and early detection of preeclampsia, and uniforms the referral thresholds and assessment procedures. Secondary prevention with calcium supplementation and aspirin administration during pregnancy are beneficial in low calcium intake women and in the patient at a very high risk of developing severe early onset disease. Lifestyle choices, dietary nutritional measures (antioxidant as vitamin C, vitamin E, lycopene, selenium, zinc, magnesium and the mitochondrial antioxidants nicotine, coenzyme Q(10) and melatonin; and other dietary nutritional measures as low dietary salt, omega 3 fatty acids, folic acid, garlic, nutritional advice, protein and energy supplementation, isocaloric balanced protein and protein and energy restriction for obese women) and others drugs; have not shown benefits or there is insufficient evidence to recommend clinical use. Proper antenatal care and timed delivery are of utmost importance in tertiary prevention.
Congenital ranula (CR) is a rare tumor. Few cases of prenatal diagnosis (PD) of CR have been reported in the literature and diagnosis usually is beyond 20 weeks of gestation. We report a case of a pregnant patient with 16 weeks of gestation, where ultrasound (US) visualized in her fetus, a cystic tumor protruding in his oral cavity. Basis and complementary studies, including fetal echocardiography, were normal. Serial two-dimensional (2D) and three-dimensional (3D) US studies monitored fetal growth, amniotic fluid, progressive changes in the cystic mass, and maxillary movements. At 38.5 weeks of gestation, a cesarean section was performed. At 17th neonate days old, the neonate was done the simple and complete excision. Early PD of CR is possible. In this case was done as early as at 16 weeks of gestation. This is the first CR reported case with the earliest PD (16 week of gestation). We recommend that during the first trimester US, the oral cavity must be well explored, to rule out tumors.
Cervical length or bishop score in prediction of imminent preterm delivery in symptomatic patients. REV CHIL OBSTET GINECOL 2019; 84(5): 362 -371 363 ABSTRACT OBJECTIVE: To compare the cervical length with Bishop score in prediction of imminent preterm delivery in symptomatic patients. METHODS:Women with single pregnancies of 24-35 weeks were selected, with the diagnosis of threatened preterm labor and intact membranes. Before the start of any treatment, all were submitted to the cervical digital examination and transvaginal ultrasound determination of cervical length. The main variable of the study was the frequency of imminent delivery (in the 7 days following evaluation). General characteristics, Bishop score, and cervical length values were evaluated. RESULTS: A total of 481 women were selected, 119 patients presented imminent delivery (group A) and 362 patients delivered after 7 days (group B). the interval between evaluation and delivery was 4.3 +/-1.6 days in group A and 56.3 +/-27.2 days in group B (p <0.0001). Patients in group A had lower values of cervical length compared with patients in group B (p <0.0001). This group also presented higher values of Bishop score (p <0.0001). Cervical length had a mayor area under for prediction (0.972, ...
Objective: To establish the association between pulsatility index of the uterine artery and imminent preterm delivery in symptomatic patients. Methods: Women with single pregnancies of 24 - 35 weeks were selected, with threat of preterm delivery and intact membranes. Before the start of any treatment, all were submitted to Doppler ultrasound evaluation. The main variable of the study was the frequency of imminent delivery (in the 7 days following the evaluation). General characteristics and values of the pulsatility index of the uterine artery were evaluated. Results: 481 patients were selected for the study. 119 participants presented imminent preterm delivery (group A) and 362 patients presented deliveries beyond 7 days (group B). Pulsatility index of the uterine artery at rest (2.24 +/- 0.51 compared to 1.57 +/- 0.36) and during uterine contractions (0.94 +/- 0.21 compared to 0, 75 +/- 0.12) were higher in patients in group A compared with those in group B (p <0.0001). It was observed that the pulsatility index at rest had an area under the curve of 0.843 compared to an area under the curve of 0.748 during contractions (p <0.05). The combination of both measurements showed a significantly higher value under the curve (0.892) that was significantly higher than both tests individually (p <0.05). Conclusion: Pulsatility index of the uterine artery is associated with imminent preterm delivery in symptomatic patients.
