Preeclampsia increases maternal and perinatal morbidity and mortality rates. Much research has been done to identify unique screening tests that would predict the risk of developing preeclampsia before the classic symptoms appear. The possible use of a screening test with high predictive accuracy in patients with high-risk or low-risk of preeclampsia remains to be investigated. At present, the search for additional tests continues. There is growing interest in the use of combinations of tests. Effective primary prevention is not possible because the causes are still unknown, but to identify and to modify susceptible risk factors might decrease the frequency of preeclampsia. A community guideline improves the screening and early detection of preeclampsia, and uniforms the referral thresholds and assessment procedures. Secondary prevention with calcium supplementation and aspirin administration during pregnancy are beneficial in low calcium intake women and in the patient at a very high risk of developing severe early onset disease. Lifestyle choices, dietary nutritional measures (antioxidant as vitamin C, vitamin E, lycopene, selenium, zinc, magnesium and the mitochondrial antioxidants nicotine, coenzyme Q(10) and melatonin; and other dietary nutritional measures as low dietary salt, omega 3 fatty acids, folic acid, garlic, nutritional advice, protein and energy supplementation, isocaloric balanced protein and protein and energy restriction for obese women) and others drugs; have not shown benefits or there is insufficient evidence to recommend clinical use. Proper antenatal care and timed delivery are of utmost importance in tertiary prevention.
Objetivo. Comparar el ángulo uterocervical con la longitud cervical en la predicción de parto pretérmino inminente en pacientes sintomáticas. Diseño. Estudio de casoscontroles. Institución. Hospital Central “Dr. Urquinaona”, Maracaibo, Venezuela. Participantes. Pacientes con parto pretérmino en los siguientes 7 días (grupo A) y embarazadas con parto pretérmino más allá de los 7 días (grupo B). Métodos. Al momento del diagnóstico, las pacientes fueron evaluadas utilizando ecografía transvaginal y fueron seguidas hasta el parto. Principales medidas de resultado. Características generales, ángulo uterocervical, longitud cervical, parto pretérmino inminente y eficacia pronóstica. Resultados. Se incluyó 326 pacientes: 75 mujeres presentaron parto pretérmino inminente (grupo A) y 251 pacientes fueron consideradas como controles (grupo B). Las pacientes del grupo A presentaron valores significativamente más altos del ángulo uterocervical y más bajos de longitud cervical comparado con las pacientes del grupo B (p < 0,0001). El ángulo uterocervical mostró un valor de área bajo la curva de 0,864, mientras que el valor del área bajo la curva de la longitud cervical fue 0,985. La diferencia de la capacidad de discriminación entre las áreas bajo la curva de cada prueba fue significativa (p < 0,0001). Conclusión. El ángulo uterocervical no es superior a la longitud cervical en la predicción de parto pretérmino inminente en pacientes sintomáticas.
Congenital ranula (CR) is a rare tumor. Few cases of prenatal diagnosis (PD) of CR have been reported in the literature and diagnosis usually is beyond 20 weeks of gestation. We report a case of a pregnant patient with 16 weeks of gestation, where ultrasound (US) visualized in her fetus, a cystic tumor protruding in his oral cavity. Basis and complementary studies, including fetal echocardiography, were normal. Serial two-dimensional (2D) and three-dimensional (3D) US studies monitored fetal growth, amniotic fluid, progressive changes in the cystic mass, and maxillary movements. At 38.5 weeks of gestation, a cesarean section was performed. At 17th neonate days old, the neonate was done the simple and complete excision. Early PD of CR is possible. In this case was done as early as at 16 weeks of gestation. This is the first CR reported case with the earliest PD (16 week of gestation). We recommend that during the first trimester US, the oral cavity must be well explored, to rule out tumors.

RESUMEN OBJETIVO: Comparar la longitud cervical con el puntaje de Bishop en la predicción de parto pretérmino inminente en pacientes sintomáticas. MÉTODOS: Se seleccionaron mujeres con embarazos simples de 24 -35 semanas, con amenaza de parto pretérmino y membranas integras. Antes del inicio de cualquier tratamiento, todas fueron sometidas a examen digital del cuello uterino y determinación ecográfica transvaginal de la longitud cervical. La principal variable de estudio fue la frecuencia de parto inminente (en los 7 días siguientes a la evaluación). Se evaluaron las características generales, puntaje de Bishop y valores de la longitud cervical RESULTADOS: Fueron seleccionadas 481 mujeres, 119 participantes presentaron parto pretérmino inminente (grupo A) y 362 pacientes presentaron partos más allá de los 7 días (grupo B). El intervalo entre la evaluación y el parto fue de 4,3 +/-1,6 días en el grupo A y 56,3 +/-27,2 días en el grupo B (p < 0,0001). Las pacientes del grupo A tenían valores más bajos de longitud cervical comparado con las pacientes del grupo B (p < 0,0001). Este grupo también presentó valores más elevados de puntaje de Bishop (p < 0,0001). La longitud cervical tenía un área bajo la curva mayor para la predicción (0,972, intervalo de confianza 95%, 0,772 -1,000) comparado con el puntaje de Bisho (0,825, intervalo de confianza 95%, 0,783 -0,870; p = 0,0137). CONCLUSIÓN: La longitud cervical es más útil en la predicción de parto pretérmino inminente en pacientes sintomáticas comparado con el puntaje de Bishop.
