Improving psychological practice in mental health services in the Brazilian Unified Health System (Sistema Único de Saúde) requires a critical analysis of core concepts of the psychiatric reform, such as 'social reinsertion'. This analysis, oriented by the dialectics of exclusion/inclusion, showed that this concept is impregnated with the adaptation paradigm and asylum view which prevents its effective implantation. The results suggest it is necessary to include social aspects in the discussion of mental health, articulating it with networks of social work and recuperating the revolutionary aspects of the psychiatric reform, thus demarcating the political nature of professional practices.
RESUMO.Este estudo teve como objetivo conhecer as histórias de ex-"moradores" de hospital psiquiátrico na visão de seus familiares. Por meio de uma pesquisa de campo de caráter exploratório entrevistamos alguns familiares de ex-"moradores" de hospital psiquiátrico, os quais descreveram o trajeto percorrido. As histórias possibilitaram delinear os caminhos que levaram essas pessoas a se tornarem "moradoras" de hospital psiquiátrico, o que esse fato lhes acarretou e o que lhes possibilitou retornar ao lar. A recorrência e longevidade da internação psiquiátrica dos "moradores" provocaram perda de papéis sociais e de laços afetivos, além de sinais de cronificação do transtorno mental, reforçando que o modelo manicomial acarreta agravos que poderiam ser evitados com o tratamento em consonância com os preceitos da atenção psicossocial. Neste modelo não há lugar para o "morar" em um hospital psiquiátrico tradicional, mas leitos em hospital geral para permanência pelo menor tempo possível, como determinam a Lei 10.216, de 06 de abril de 2001, e a Portaria 3.088, de 23 de dezembro de 2011. Palavras-chave: Saúde mental; família; reforma psiquiátrica. THE "RESIDENCE" IN PSYCHIATRIC HOSPITAL : STORIES TOLD BY FAMILIES OF EX-"RESIDENTS"ABSTRACT. This study aimed at getting to know stories of the ex-"residents" of psychiatric hospital in their families´view. Through a field research of exploratory character, families of ex-"residents" of psychiatric hospital were interviewed and they described the route taken by them. The stories allowed to trace the path that led these people to become "residents" of psychiatric hospital, what this fact caused to them and what allowed them to return home. The recurrence and longevity of psychiatric hospital admission of "residents" caused loss of social roles and of affective bonds, and also cronicity signs of mental disorders, reinforcing that the asylum model entails in aggravations that could be avoided with treatment in accordance with the precepts of psychosocial care. In this model, there is no place to "reside" in traditional psychiatric hospital, but in general hospital beds and in the shortest time possible, as determined by Law 10216 of April 6, 2001 and by Decree 3088 of 23 December 2011.
O objetivo deste estudo é apresentar considerações de alguns médicos higienistas sobre os serviços abertos de saúde mental desenvolvidos no Brasil no início do século XX. Analisamos publicações dos Archivos Brasileiros de Hygiene Mental e outros materiais e verificamos que a possibilidade de cuidar em liberdade (defendida pela atual Reforma Psiquiátrica) já era vislumbrada pelos higienistas, em um período no qual a assistência era, hegemonicamente, pautada no isolamento em instituições psiquiátricas. Fund., São Paulo, 19(4), 737-750, dez. 2016 Este trabalho enfoca os serviços abertos de saúde mental no Brasil, a partir do resgate das considerações debatidas pelos higienistas no início do século XX. De forma sucinta, podemos pontuar que os higienistas 1 eram médicos e intelectuais em geral, integrantes da Liga Brasileira de Higiene Mental (LBHM), entidade civil fundada em 1923, que tinha entre as suas fi nalidades a prevenção das enfermidades mentais, o amparo aos egressos dos manicômios e a "melhoria progressiva nos meios de assistir e tratar os doentes nervosos e mentais em asilos públicos, particulares ou fora deles" (Liga Brasileira de Hygiene Mental, 1925, p. 223). Pautados nos conceitos das ciências naturais, os higienistas buscaram solucionar os problemas sociais da época -desde doenças físicas até os costumes e comportamentos considerados desviantes pela sociedade burguesa -por meio da higienização física e mental da população, propondo intervenções em diversos âmbitos, tais como na educação e na saúde. A base psiquiátrica desses profi ssionais continha uma tendência a associar certos problemas psiquiá-tricos a questões culturais mais gerais relativas ao povo brasileiro, atribuindo-os, por exemplo, à situação social dos imigrantes ou à constituição étnica dos brasileiros (Costa, 2007). Ao transporem de forma direta os preceitos das ciências naturais às ciências sociais, os higienistas concebiam, muitas vezes, as diferenças sociais como resultantes da natureza humana e não do modo como os homens constroem as relações entre eles. Entretanto, tal concepção não deve ofuscar as contribuições desses intelectuais que se dedicaram à saúde mental e consideraram, dentro daquele contexto, propostas que poderiam melhorar a assistência nesta área.1 Para mais informações sobre os higienistas, sugerimos os textos de Costa (2007) e Boarini (2003). 739HISTÓRIA DA PSIQUIATRIA Rev. Resumo Este estudo apresenta os organismos comunitários em saúde mental (OCSM) do Quebec, Canadá. Especificamente, traça reflexões considerando sua origem, inserção no sistema público de saúde, princípios, características e formatos. Os OCSM são serviços autônomos, originados na comunidade, que oferecem atividades sem fins lucrativos no setor da saúde e assistência social. Realizou-se estudo descritivo, de abordagem qualitativa, com revisão não sistemática da literatura. Os resultados apontaram o surgimento dos OCSM no contexto de desospitalização de pacientes, mas com déficit de recursos substitutivos ao hospital psiquiátrico. Apontaram também sua consolidação como alternativa à psiquiatria tradicional, reconhecimento e financiamento parcial, pelo governo, como parte da assistência à saúde mental. Constatou-se que os OCSM compartilham certos princípios, mas concretizam-se em vários formatos, destacando-se por estimular a defesa dos direitos e a convivência em comunidade de pessoas em sofrimento psíquico. Contudo, observaram-se limitações quanto ao alcance e à interação entre OCSM e estabelecimentos públicos, e questiona-se o papel do Estado frente a tais organismos. Conclui-se apontando aproximações e distanciamentos entre características da assistência em saúde mental promovida pelos OCSM no Quebec e pelo Brasil; e indica-se a continuidade das discussões, visando à sedimentação de políticas e práticas em saúde mental, que garantam o cuidado na comunidade.
Resumo: Objetiva-se relatar e discutir a experiência de participação em um encontro de um grupo de música, direcionado para pessoas em sofrimento psíquico, em Montreal -Canadá. Trata-se de um estudo descritivo-qualitativo, do tipo relato de experiência, com utilização do registro em diário de campo e análises pautadas na literatura sobre a temática. Discutese que um encontro guiado pela música ultrapassa a lógica da transmissão do conhecimento musical ou do foco no conteúdo emocional dos participantes, permitindo uma experiência terapêutica, transformadora e crítica. Conclui-se, pela ampliação de espaços como o relatado na experiência, que possam contribuir com o fortalecimento de um novo modelo de cuidado em saúde mental, garantindo que os que sofrem psiquicamente sejam de fato ouvidos e possam exercer sua cidadania.
