Introdução: A Hanseníase é uma das doenças mais antigas mundialmente das quais se tem relato histórico, essa doença sendo causada por uma bactéria que atinge nervos periféricos, mucosas e pele e se não for tratada de forma precoce, pode causar incapacidade física e mental. Além disso, o Brasil é o único país que não alcançou a meta de erradicação da doença como problema de saúde pública. Objetivo: Compreender a importância e do papel das áreas de pobreza na construção do cenário da hanseníase. Métodos: Estudo epidemiológico de abordagem quantitativa utilizando os dados da plataforma DATASUS sobre a doença nos anos de 2020 e 2021 e filtragem da busca em municípios de extrema pobreza. Resultados: Observou-se números expressivos com relação ao total de casos nas regiões de extrema pobreza em relação às demais regiões. Conclusão: Por serem mais vulneráveis que outras regiões, as áreas de extrema pobreza precisam de monitoramento epidemiológico e ações de prevenção constantes para atingir a meta de erradicação da doença como problema de saúde pública.
