Os transtornos alimentares são quadros psiquiátricos que cursam com intenso sofrimento psíquico e diversas complicações clínicas. A avaliação de pacientes com transtornos alimentares costuma exigir uma anamnese minuciosa, exame físico e psíquico detalhado e exames complementares. Nessa avaliação, objetiva-se estabelecer um diagnóstico nosológico do transtorno alimentar e das comorbidades, uma estratificação do risco clínico e psiquiátrico, para então estabelecer um planejamento terapêutico. Após essa avaliação inicial, de acordo com a gravidade do caso, o clínico deve decidir o nível de tratamento recomendado, que pode ser desde a hospitalização até o atendimento ambulatorial regular. Outras decisões, como a via de realimentação, o valor calórico planejado para a realimentação e as metas de recuperação, também devem ser pré-definidas. Sumarizamos, neste artigo, alguns aspectos relacionados à avaliação, comorbidades e orientações gerais de tratamento a partir de três diretrizes internacionais de manejo e tratamento de transtornos alimentares, a saber: duas britânicas, a guideline de transtornos alimentares do National Institute for Health and Care Excellence (NICE) e o Management of Really Sick Patients with Anorexia Nervosa (MARSIPAN); e uma australiana, elaborada pelo Royal Australian and New Zealand College of Psychiatrists (RANZ-CP).
Introdução: Entre os Transtornos Alimentares (TA), a Bulimia Nervosa (BN) e o Transtorno da Compulsão Alimentar (TCA) compartilham características clínicas e históricas e suas primeiras descrições médicas e categorizações diagnósticas ocorreram no século passado. Objetivos: revisar brevemente os aspectos históricos da BN e TCA, bem como revisar suas categorizações ao longo das edições da Classificação Internacional de Doenças (CID) e do Manual Diagnóstico e Estatístico dos Transtornos Mentais (DSM). Metodologia: Realizou-se uma revisão em todas as edições das classificações da CID e do DSM, além de livros-texto de referência de psiquiatria clínica e de publicações sobre a história e a evolução desses diagnósticos, além de uma breve revisão das atualizações sobre tratamento desses transtornos. Resultados: apresentaram os critérios diagnósticos da BN e TCA a cada edição da CID e DSM, desde a primeira vez em que foram categorizadas, e as progressivas mudanças foram comentadas. Por fim, apontou-se atualização breve dos tratamentos de ambos os transtornos. Conclusão: É imprescindível, porém não suficiente, ao clínico conhecer os critérios diagnósticos mais atuais das patologias. É importante compreender também a evolução do conceito e assim obter uma visão mais abrangente e mesmo crítica dos critérios, a partir da consciência de sua provisoriedade.
